Per-oral Endoscopic Myotomy Biopsies of Achalasia Patients Reveal Schwann Cell Depletion in the Muscularis Propria.

Achalasia is a neurodegenerative condition resulting in abnormal lower esophageal sphincter relaxation and impaired upstream esophageal body peristalsis.1 The pathophysiology and natural history of achalasia remain unclear, and evaluation of the histopathogenesis of achalasia has traditionally been challenging because the esophageal wall muscularis propria is not typically accessible via routine endoscopic biopsies.

Investigation on bowel regulation and constipation relief based on the microbial fermentation solution of cassia seed.

ETHNOPHARMACOLOGICAL RELEVANCE: As digestive health issues rise and interest in natural therapies grows, traditional herbs like Cassia Seed are gaining attention for their antioxidant, laxative, and digestive benefits. AIM OF THE STUDY: This study aimed to optimize the fermentation conditions of Cassia seed using microbial technology to enhance the content of anthraquinone compounds, thereby augmenting its pharmacological effects, particularly in promoting intestinal peristalsis and alleviating constipation. MATERIALS AND METHODS: Fermentation of Cassia Seed was conducted under controlled mi…

A randomized double blind comparison of atosiban in patients with recurrent implantation failure undergoing IVF treatment.

BACKGROUND: Patients with recurrent implantation failure (RIF) may have more uterine contractions. Several observational studies suggested t … mbryos previously transferred, number of previous failed embryo transfers, frequency of endometrial peristalsis on embryo transfer day (≥ 3 waves/min) or serum estradiol (E2) on the day of hCG above the median level. And, there was no correlation between the serum E2 level on the day of hCG and the frequency of endometrial peristalsis on embryo transfer day. …

Abnormal development of lung innervation in experimental esophageal atresia.

BACKGROUND/AIM: Patients with esophageal atresia and tracheo-esophageal fistula (EA-TEF) have chronic respiratory tract disease and deficien … protein by immunoblot. Embryonic lung primordia harvested on E13 were cultured for 72 h and airway peristalsis was assessed under an inverted microscope. PGP 9.5 expression was then examined in explants by whole-mount immunohistochemistry and RT-PCR. Values were compared with non-parametric tests. RESULTS: Neural networks were present in both EA-TEF and con …

Brainstem and vagal dysfunction in pharyngoesophageal and esophageal dysphagia.

Dysphagia affects approximately 16% of the US population and is classified as pharyngoesophageal and esophageal, the former being more clini … ngeal, and laryngeal paresis to life-threatening bilateral paralysis, and loss of normal esophageal peristalsis. Because conventional manometry detects only the contraction phase of peristalsis, distension-contraction plots are needed to uncover distension-phase abnormalities in patients when pressure recordings appear normal, both in the pharynx and esophagu …

A New 3D Colon on a Chip to Decipher the Influence of Mechanical Forces on the Physiological Cellular Ecosystem.

The gut epithelium ensures nutrient absorption and barrier protection, functions tightly linked to its 3D architecture and dynamic mechanical activity. To dissect how mechanical forces influence intestinal physiology, we developed a stretchable 3D colon-on-chip that integrates tunable topography, stiffness and peristalsis-like motion within a physiologically relevant microenvironment. The model employs 3D scaffolds composed of either pure collagen I or mechanically reinforced interpenetrated network (IPN) made of collagen I and PEGDA. Tensile tests mimicking peristalsis revealed that both hydr…

A jejunostomy tube was placed to prevent further aspiration. Gastrointestinal tract obstruction in the fetus.

Surgical advice is often sought when a prenatal diagnosis of gastrointestinal tract obstruction is made. We reviewed our experience with 17 such cases during a 4-year period. Eight fetuses had complete proximal obstruction. Seven of the 8 did well after maternal transport to a perinatal center and prompt neonatal surgery. Six fetuses had distal obstruction with dilated bowel and increased peristalsis. Two died after birth (1 with severe associated anomalies and 1 with short-bowel syndrome), and the other 4 did well. Three fetuses had a false-positive diagnosis of in utero meconium peritonitis.…

Effect of intraesophageal bile and acid (HCl) perfusion on the action of the lower esophageal sphincter.

We examined the effect of acid and bile on the lower esophageal high pressure zone (LEHPZ) in a combined manometric, pH testing and morpholo … s within 4 weeks. LEHPZ pressures returned to almost normal values after this recovering period but peristalsis was still completely discoordinated. Swallowing waves were of exaggerated magnitude and length. From our findings it is evident that bile and acid change LEHPZ competence. Bile and acid seem to have a different effect on motor function of the LEHPZ …

Esophageal Infusion of Menthol Does Not Affect Esophageal Motility in Patients with Gastroesophageal Reflux Disease.

Menthol is thought to trigger gastroesophageal reflux disease (GERD) symptoms by influencing esophageal peristalsis and lower esophageal sphincter (LES) function. We evaluated the effect of esophageal menthol infusion on esophageal motility and the LES in healthy volunteers and in patients with GERD. High resolution manometry (HRM) catheter with attached thin tube for menthol infusion was placed transnasally. Protocol which included baseline recording, 16 water swallows (5 ml, 10 ml, and 15 ml) and the multiple rapid swallows was performed before and after esophageal infusion of menthol (3 mM,…

[Pelvioureteral function after ureterotomy--experimental study simulating endopyeloureterotomy].

BACKGROUND: Endopyeloureterotomy has been established as a valuable procedure for ureteropelvic junction or upper ureteral stenosis. In order to evaluate ureteropelvic function after endopyeloureterotomy, I examined electromyographic change in ureteral peristalsis, measured urine bolue volume, and monitored ureteral compliance in dogs following ureterotomy. METHODS: Twelve adult mongrel dogs were anesthetized with pentobarbital, and the upper part of the left ureters was incised longitudinally for a distance of 2 cm. After the incision of the left ureter, a polyurethane splint was inserted th…

Spontaneous retrograde migration of a ureteric calculus: a rare phenomenon.

INTRODUCTION: While spontaneous anterograde passage of ureteric stones is well documented, retrograde migration-movement of the stone back t … de migration of ureteric calculi is a rare phenomenon. Proposed mechanisms include reverse ureteral peristalsis, proximal ureteral dilation, antiperistaltic waves due to irritant stimuli, and the effects of non-steroidal anti-inflammatory drugs (NSAIDs) on ureteric peristalsis. Awareness of this occurrence is important for appropriate diagnosis and management …

[Esophageal Motility Disorders - What's New?].

Esophageal motor disorders are an important cause of dysphagia but can also be associated with retrosternal pain and heartburn as well as regurgitation. In extreme cases, patients are not able to eat appropriately and lose weight. Repetitive aspiration can occur and may cause pulmonological complications. Achalasia represents the most important and best-defined esophageal motor disorder and is characterized by insufficient relaxation of the lower esophageal sphincter in combination with typical disturbances of esophageal peristalsis. Additional defined motor disorders are distal esophageal spa…
