Codevelopment of a model of care for adults living with cystic fibrosis-related diabetes.

BACKGROUND: Cystic fibrosis (CF) related diabetes affects up to half of all adults with CF and is associated with higher morbidity and mortality. Our aim is to codevelop an ideal model of care that integrates diabetes technology and better meets the needs of adults living with the condition to improve attendance, engagement, service satisfaction, and clinical outcomes. METHODS: Using qualitative research methods, we evaluated disease perceptions, barriers, and enablers to optimal CF-related diabetes management and service delivery. Integration of continuous glucose monitoring (CGM) was also e

The role of alcohol consumption in the lives of older Australian women: qualitative insights and an agenda for further research, policy and practice.

BACKGROUND: Alcohol consumption presents a threat to the health and wellbeing of women. The alcohol industry often pushes back at global efforts to prioritise the prevention of alcohol harms to women. Qualitative researchers have investigated how younger and midlife women conceptualise their alcohol consumption, but there is very limited research relating to older women (those 60 years and over). METHODS: Using data collected from an online qualitative survey, this paper explored the factors that influence how older Australian women drinkers (n = 144. 60-88 years) conceptualised the role of a

Algorithmic hyperindividualisation and the transformation of vulnerability in substance use prevention.

Generative artificial intelligence is entering substance use prevention through applications that tailor health information to the individual in real time. These systems promise to reach vulnerable groups more precisely than mass communication, and they rest on individual-level models of behaviour change. This article asks what happens to the concept of vulnerability once it becomes something such a system can read and act upon. It draws on the public health literature on vulnerability as a multilevel construct, the theory of fundamental causes, Rose's prevention paradox, and critical work on

Mobile App-Supported Self-Management for Chronic Low Back Pain: Realist Evaluation.

BACKGROUND: As the world's population ages, the prevalence of chronic low back pain (CLBP) is increasing, placing a substantial burden on individuals and health care systems. Mobile health (mHealth) apps offer a potentially scalable solution to support self-management, but little is known about how, why, for whom, and under what circumstances such tools work in real-world settings. OBJECTIVES: This study aimed to test and refine 3 program theories-developed through a previous realist review-on how mobile apps support CLBP self-management. The goal was to understand the key contextual factors

Strategies for older people living in care homes to prevent urinary tract infection: the StOP UTI realist synthesis.

BACKGROUND: Urinary tract infection is the most diagnosed infection in older people. It accounts for more than 50% of antibiotic prescriptions in care homes and is a frequent reason for care home residents being hospitalised. OBJECTIVE: This realist review developed and refined programme theories for preventing and recognising urinary tract infection, exploring what works, for whom and in what circumstances. DESIGN: The review used realist synthesis to explore existing literature on the detection and prevention of urinary tract infection, complemented by stakeholder consultation. It applies

Acceptance of Machine Learning for Medication Selection in Epilepsy to Inform Clinical Trial Design: Co-Design Survey Study.

BACKGROUND: Antiseizure medications (ASMs) are the mainstay of epilepsy treatment; however, there is currently no reliable way to predict which medication will be most effective for an individual patient. Machine learning (ML) approaches are increasingly being explored to support personalized ASM selection, but successful implementation will depend on acceptance by both people with epilepsy and treating neurologists. Understanding factors influencing acceptability of ML in clinical decision-making is therefore critical to support engagement, trust, and adherence. The extended unified theory of

Construction of a digital literacy education support model and development and validation of a scale for physical education majors in China.

INTRODUCTION: Physical education majors have a dual identity as current learners and future educators. Their digital literacy is not only related to their adaptation to digital learning environments but also directly influences the development of their future competence in physical education teaching, training organization, and management practice. Therefore, it is necessary to systematically identify and strengthen educational support for their digital literacy development. However, existing studies have mainly focused on digital literacy levels or single influencing factors, with insufficien

A methodology to model causal relationships on offshore safety assessment focusing on human and organizational factors.

INTRODUCTION: Focusing on people and organizations, this paper aims to contribute to offshore safety assessment by proposing a methodology to model causal relationships. METHOD: The methodology is proposed in a general sense that it will be capable of accommodating modeling of multiple risk factors considered in offshore operations and will have the ability to deal with different types of data that may come from different resources. Reason's "Swiss cheese" model is used to form a generic offshore safety assessment framework, and Bayesian Network (BN) is tailored to fit into the framework to c

Digging Deeper into Precision/Personalized Medicine: Cracking the Sugar Code, the Third Alphabet of Life, and Sociomateriality of the Cell.

Precision/personalized medicine is a hot topic in health care. Often presented with the motto "the right drug, for the right patient, at the right dose, and the right time," precision medicine is a theory for rational therapeutics as well as practice to individualize health interventions (e.g., drugs, food, vaccines, medical devices, and exercise programs) using biomarkers. Yet, an alien visitor to planet Earth reading the contemporary textbooks on diagnostics might think precision medicine requires only two biomolecules omnipresent in the literature: nucleic acids (e.g., DNA) and proteins, kn

Identifying Subgroups in Acceptance and Intended Use of Digital Technologies and the Role of Economic, Cultural, Social, and Person Capital: Latent Class Analysis.

BACKGROUND: Digital health technologies offer promising opportunities to support physical health. However, their acceptance, use, and associated benefits are not equally distributed across society. While existing research has mainly focused on traditional socioeconomic indicators, broader sociological influences, including economic, cultural, social, and person capital, may provide a more comprehensive understanding of these inequalities. Yet, too little is currently known about how different subgroups, based on their economic, cultural, social, and person capital, relate to intentions to acce

Associations between sensory changes and psychological problems after moderate-to-severe traumatic brain injury: A network analysis.

BACKGROUND: Moderate-to-severe traumatic brain injury (msTBI) is frequently accompanied by persistent sensory changes that can substantially impair quality of life. Theory suggests that these changes may not occur in isolation but instead be closely intertwined with psychological difficulties (eg, anxiety). A better understanding of the interrelationships between sensory changes and psychological problems may help identify mechanisms underlying symptom persistence and inform rehabilitation efforts. METHODS: We conducted a psychometric network analysis on cross-sectional questionnaire data fro

Clinical and Subclinical Narcissism as Risk Factors for Suicidality: Revisiting Conceptual Links.

BACKGROUND: Narcissism-as either clinical or subclinical-has long been discussed as a risk factor for suicide. Whereas traditional studies link narcissism with grandiosity, entitlement, and deficits in empathy, newer research has emphasized the intricate interactions among this construct and vulnerability to mental illness, emotion dysregulation, as well as the effects of social stress. The inconsistency in the impingement of narcissistic features on suicidal ideation highlights for us rethink and reformulate our conceptual understanding for therapies. OBJECTIVE: The purpose of this article i

Shifting participatory approach when ideology meets reality: a grounded theory study based on project leaders' experiences with peer-led sex education programs for and by persons with intellectual disabilities and/or autism.

BACKGROUND: This study explored peer-led sex education for individuals with intellectual disabilities and/or autism (ID/ASD) from the perspective of project leaders within Swedish non-governmental organizations (NGOs). The purpose of this Grounded Theory study was to develop a conceptual model that characterizes the ways in which peer-led sex education is implemented by Swedish NGOs. This was done by exploring what the concept of peer-led sex education means to NGO project leaders, and how they experience, explain and reason about the application of peer education in their daily operations. Parental Intention to Support the Use of Computerized Cognitive Training for Children With Genetic Neurodevelopmental Disorders.

Children with genetic neurodevelopmental disorders (NDDs) such as Down syndrome, Prader-Willi syndrome, and Fragile X syndrome may show a range of cognitive impairments, including impairments in executive functions (EF). EF are related to general intelligence, academic achievement, and literacy and mathematical skills. EF deficits are linked to a variety of clinically and socially important behaviors. Therefore, methods for improving EF in children with NDDs could be beneficial. One method for improving EF is through cognitive training. Research on commercial brain training programmes and vide

Cross-cultural adaptation and psychometric validation of the Kannada version of the Extended Effort Assessment Scale (EEAS-K) in adults with hearing loss.

INTRODUCTION: The Extended Effort Assessment Scale (EEAS) is a recently developed questionnaire for measuring listening effort, but a validated version for Kannada speakers is lacking. This study aimed to translate, culturally adapt, and psychometrically validate the EEAS for the Kannada-speaking adults with hearing loss. METHOD: The EEAS was translated into Kannada (EEAS-K) following rigorous forward and back translation, expert committee review and cognitive debriefing. The final EEAS-K was then field-tested with 100 native Kannada-speaking adults with hearing loss (predominantly non-hearin

Integrating noncommunicable disease care in a public primary health care facility in North Lebanon: A qualitative study of implementation in a humanitarian crisis.

Integrated care for non-communicable diseases (NCDs) may enhance care delivery, clinical impact, user-satisfaction, and access. However, evidence on integrated NCD care in humanitarian settings remains scarce. This qualitative study accompanied the first year of implementation (March 2023 to March 2024) of an integrated care programme at an International Committee of the Red Cross (ICRC)-supported, Ministry of Public Health-affiliated primary healthcare centre (PHC) in North Lebanon. A preceding theory of change (TOC) workshop on combining NCD PHC, mental health and psychosocial support (MPHSS

Nursing their way not our way: working with vulnerable and marginalised populations.

AIMS AND OBJECTIVES: This paper uses the findings of two studies to explore the nature of nurses practice when working with vulnerable and marginalised populations, particularly with regard to the attributes of holism and individualised care. A secondary analysis of the findings of two separate studies was undertaken, one on the elderly with delirium and the other with indigenous Maori women. BACKGROUND: Often considered vulnerable and marginalised, elderly and indigenous experiences of health services are often fraught with their health needs not always identified or addressed. Nursing claim

Sense of Coherence in patients with stable schizophrenia: multidimensional determinants from a cross-sectional study of 438 inpatients in Guangzhou, China.

INTRODUCTION: Schizophrenia, a chronic psychiatric disorder, profoundly affects social functioning and quality of life. The Sense of Coherence (SOC), a core construct in salutogenic theory, plays a pivotal role in stress coping and recovery outcomes. While SOC has been studied in general populations, its determinants in stabilized schizophrenia inpatients remain underexplored. This study comprehensively investigated the Sense of Coherence (SOC) profile among stabilized schizophrenia inpatients by assessing its current status, exploring the interrelationships between SOC and demographic factors

Knowledge-seeking and knowledge sharing of health services across social networks and communities: a scoping review.

INTRODUCTION: Lay people's knowledge influences healthcare service utilisation, but the literature on people's knowledge-seeking and sharing about different healthcare services across social networks is patchy and not well integrated. This scoping review was undertaken to map how different studies report healthcare service -related (healthcare) knowledge-seeking or sharing in social circles and to identify evidence gaps for further research. METHOD: Levac's enhanced scoping review framework was adapted to develop a comprehensive electronic search strategy. Four electronic databases-Medline, W
