Dangling Lens Phacoemulsification: A Novel Technique for Near Complete Zonular Dehiscence.

PURPOSE: To demonstrate a novel technique for the management of dangling lenses with near complete zonular dehiscence. METHODS: A capsule-sparing combined anterior and posterior surgical approach for dangling lenses with severe zonular dehiscence is described. The surgical record of a patient who underwent phacoemulsification with "dangling lens technique" and in-the-bag intraocular lens (IOL) implantation was evaluated. RESULTS: This technique was able to preserve the lens capsule, allowing freedom of IOL choice and an optimally reconstructed anterior segment. Placement of sutured capsular …

Occult Descemet's membrane detachment after phacoemulsification surgery mimicking pseudophakic bullous keratopathy.

We herein report two cases of occult Descemet's membrane detachment (DMD) after phacoemulsification surgery, which initially presented as persistent corneal edema and had been considered as pseudophakic bullous keratopathy. The patients were thus scheduled to receive corneal transplantation. For Case 1, DMD was incidentally detected by slit-lamp examination 2 months postoperatively, only when part of the cornea became clearer. For Case 2, anterior segment optical coherence tomography demonstrated extensive DMD, which had lasted 5 months after the operation. DMDs in both patients had been succe…

Endothelial Cell Loss after Phacoemulsification according to Different Anterior Chamber Depths.

Purpose. To compare the loss of corneal endothelial cells after phacoemulsification according to different anterior chamber depths (ACDs). Methods. We conducted a prospective study on 135 eyes with senile cataracts. Eyes with nuclear density grades of 2 to 4 were divided into three groups according to ACD: ACD I, 1.5 < ACD ≤ 2.5 mm; ACD II, 2.5 < ACD ≤ 3.5 mm; or ACD III, 3.5 < ACD ≤ 4.5 mm. Intraoperative mean cumulative dissipated energy (CDE) was measured. Clinical examinations included central corneal thickness (CCT) and endothelial cell count (ECC) preoperatively and 2 months postoperativ…

Closure technique for leaking wound resulting from thermal injury during phacoemulsification.

UNLABELLED: We describe a method of wound closure with a conjunctival flap in cases of wound burn. A fornix-based conjunctival flap is raised to adequately cover the wound. Then, a single horizontal mattress suture with 10-0 monofilament nylon is passed to appose the wound. The conjunctival flap is hinged to the corneal end of the loop so the conjunctival flap covers the wound when the suture is tied. The wound is thus sealed by conjunctiva with optimum suture tension. This suture was applied in 5 patients with clear corneal incisions who developed wound burn during phacoemulsification. All ha…

Clear corneal incision leakage after phacoemulsification--detection using povidone iodine 5%.

The purpose of this work was to study the incidence of clear corneal wound leakage at the conclusion of standard co-axial phacoemulsification in a prospective observational series of 100 consecutive cataract cases in a single surgeon's institutional practice. At the conclusion of standard co-axial phacoemulsification using a 2.75 mm temporal single plane clear corneal incision with a 1 mm clear corneal side-port incision, the wounds were hydrated and checked for water-tightness. Povidone iodine 5% (P-I) was then evenly dripped over the cornea and the wounds were inspected visually. Any leakage…
