[Phototherapy in the treatment and prevention of serious icterus in the neonate].

Ten years ago, treatment of serious icterus in the neonate was made more effective by the addition of a new method: the employment of the hypobilirabinaemising effect of light. The mechanism of degradation lying behind this action is not vet fully clear; there can be no doubt, however, that the method is convenient, practical and harmless. Personal results obtained in 1 yr of work in this field are reported. 113 subjects (77% of whom were immature) were treated. The ratio of treated subjects to number of births was 7% (ratio between exsanguinotransfusion and birth values, 0.7%). The means cons

High- versus low-dose phototherapy for neonatal jaundice.

RATIONALE: Phototherapy is the mainstay of treatment for neonatal hyperbilirubinaemia. Increasing the dosage or intensity of phototherapy by various means is commonly practised for infants with high levels of serum bilirubin. It is unclear whether these measures of increasing phototherapy dosage are beneficial and safe for neonates. OBJECTIVES: To assess the effects of high-dose versus low-dose phototherapy on bilirubin level and associated clinical outcomes that constitute the major conditions in kernicterus spectrum disorder (KSD), such as acute bilirubin encephalopathy and kernicterus, as …

Validating the early phototherapy prediction tool across cohorts.

BACKGROUND: Hyperbilirubinemia of the newborn infant is a common disease worldwide. However, recognized early and treated appropriately, it typically remains innocuous. We recently developed an early phototherapy prediction tool (EPPT) by means of machine learning (ML) utilizing just one bilirubin measurement and few clinical variables. The aim of this study is to test applicability and performance of the EPPT on a new patient cohort from a different population. MATERIALS AND METHODS: This work is a retrospective study of prospectively recorded neonatal data from infants born in 2018 in an ac…

Noninvasive monitoring of bilirubin photoisomer excretion during phototherapy.

Lumirubin is the most prevalently excreted hydrophilic bilirubin photoisomer in phototherapy for neonatal jaundice caused by excess hydrophobic unconjugated bilirubin (ZZ-bilirubin). We developed a simple method to estimate the amount of lumirubin by monitoring the reverse photoisomerization of lumirubin to ZZ-bilirubin. Although lumirubin formation was long considered irreversible, exposure to blue light in the presence of the fluorescent protein UnaG, which binds specifically and tightly to ZZ-bilirubin, enables the reverse photoisomerization of lumirubin. This reaction was first detected us…

Association Between Neonatal Phototherapy Exposure and Childhood Neoplasm.

OBJECTIVE: To study the association between phototherapy for the treatment of neonatal jaundice and the risk of childhood neoplasms. STUDY DESIGN: This population-based retrospective cohort study included all infants born at ≥32 weeks of gestation at a single medical center between 1988 and 2018. The incidence of neoplastic diseases was compared between infants exposed to phototherapy and those unexposed. Kaplan-Meier curves and log-rank tests were used for cumulative incidence comparison, and multivariable Cox and Weibull survival analysis were used to adjust for confounding or clinically si…

Long-term hazards of neonatal blue-light phototherapy.

Blue-light phototherapy has been an essential therapeutic tool in the management of neonatal jaundice for decades. Rarely, it is accompanied by acute dermatological and systemic side-effects, but fortunately these are reversible and can be adequately and promptly treated in routine neonatal practice. In contrast, much less is known about the potential long-term side-effects of neonatal blue-light phototherapy (NBLP). Many of the data that are currently available on how NBLP influences melanocytic naevus (MN) development are controversial. The results of recent well-designed epidemiological sur…

[Scleredema diabeticorum: successful treatment with UVA-1 phototherapy].

Scleredema adultorum is characterized by induration of the skin on the neck, shoulders and upper back caused by increased accumulation of collagen and aminoglycans in the dermis. The induration may progress and lead to pronounced restriction of mobility. Scleredema diabeticorum is one type of scleredema adultorum associated with diabetes mellitus. Multiple therapies have been tried, but most of them have not proven to be consistently effective. We report two cases of scleredema diabeticorum treated successfully with UVA-1- as well as physiotherapy and topical corticosteroids; this approach led

Narrowband UVB phototherapy for early-stage mycosis fungoides.

BACKGROUND: Narrowband UVB (NB-UVB) phototherapy has been shown to be effective for the treatment of various dermatoses. OBJECTIVE: We evaluated the effect of NB-UVB in the treatment of early stage mycosis fungoides (MF). METHODS: The response of 24 patients (12 stage IA, 12 stage IB) with patch stage MF to thrice weekly NB-UVB was assessed. Twelve patients had skin phototypes I-III, and 12 had types IV-VI. Seven patients had hypopigmented MF. Mean follow-up period was 29.0 weeks. RESULTS: Thirteen patients (54.2%) had a complete response (CR), 7 (29.2%) had partial response (PR), and 4 (16…
