Ruptured liver abscess presenting as pneumoperitoneum caused by Klebsiella pneumoniae: a case report.

BACKGROUND: Spontaneous gas-forming pyogenic liver abscess (GFPLA) is a rare complication with a high fatality rate in spite of aggressive management. Clinical spectrum of GFPLA can mimic hollow viscus perforation as it usually accompanied by pneumoperitoneum and peritonitis. Up to now, GFPLA has not been well studied in Vietnam. CASE PRESENTATION: We reported here a case with pneumoperitoneum caused by ruptured liver abscess in a 41-year-old man with a history of treated duodenal ulcer and uncontrolled type II diabetes mellitus. He had an epigastric pain associated with a high fever. Patient

Body temperature evaluation during induced pneumoperitoneum with CO₂: an experimental study in pigs.

BACKGROUND: In prolonged laparoscopic procedures, hypothermia is frequently observed. The possible influence of the vasodilating action of CO(2), due to its increased levels in the blood during the laparoscopic procedures, has yet to be studied. The objective of this study was, therefore, to evaluate body temperature patterns in pigs subjected to pneumoperitoneum with CO(2). METHODS: Thirty male pigs were allocated into three groups of ten animals each: group I, anesthetic procedure and abdominal puncture only; group II, the same as for group I and insufflation with CO(2); and group III, the

Fatal pneumoperitoneum caused by nasopharyngeal oxygen delivery after transoesophageal echocardiography for cardiac surgery.

We report a case of fatal post-operative pneumoperitoneum in a patient who had undergone urgent mitral valve surgery. In the absence of a proven cause of the pneumoperitoneum (refusal by the family of an autopsy), we can only propose a hypothesis for its origin. The most probable one is that forceful or sustained retrograde flexion of the transoesophageal echocardiographic probe created a lower oesophagus or gastric rupture and that oxygen flow administered by the nasal cannula went straight to the abdominal cavity, leading to tension pneumoperitoneum.

Pneumoperitoneum--rare complication in end stage renal disease patient on automated peritoneal dialysis.

Peritoneal dialysis (PD) is the first option for patients in end stage renal disease (ESRD). Several complications such as peritonitis, exit-site or tunnels infections are encountered during PD. Other complications such as pain, proteic malnutrition, hyperglycemia, hypertension, cardiac failure are described in patients on continuous ambulatory peritoneal dialysis (CAPD) or APD (automated peritoneal dialysis). Rare complications are incapsulated sclerosing peritonitis, hemoperitoneum or pneumoperitoneum. We present the case of a female patient, 66 years old, on cyclic continuous peritoneal dia

Long-distance air medical transport of extremely low birth weight infants with pneumoperitoneum.

OBJECTIVE: Long-distance air transport (LDAT) of infants with pneumoperitoneum for pediatric surgical evaluation has not been previously reported. We examined whether extremely low birth weight (ELBW) infants with and without pneumoperitoneum would tolerate transport differently. STUDY DESIGN: A retrospective cohort study was conducted comparing ELBW infants diagnosed with pneumoperitoneum to other ELBW infants transported>2000 miles by a trained team from a US Department of Defense tertiary care neonatal intensive care unit in Okinawa, Japan. RESULT: Between 2000 and 2006, 49 air transports

Intraoperative diagnostic pneumoperitoneum in pediatric patients with unilateral inguinal hernias: the Goldstein test.

Over a four-year period 256 infants and children presenting with unilateral inguinal hernias underwent intraoperative diagnostic pneumoperitoneum to evaluate the contralateral groin. Sixty one patients had positive results and underwent contralateral hernia repair. Of the 195 negative tests 3 presented later with either a hernia, communicating hydrocele or cord hydrocele (false negative rate of 1.5%). This test represents a safe, economical, rapid, and accurate means to evaluate the contralateral groin in the pediatric patient presenting with a unilateral inguinal hernia.

[Pneumoperitoneum induced by artificial respiration and its diagnosis by peritonel lavage (author's transl)].

Ventilation with increased end-exspiratory pressure may, especially in cases of blunt chest trauma, result in pneumoperitoneum although there are no intestinal injuries. The case histories of four persons who developed this complication are reviewed. Differentiation between such a complication and a condition requiring laparotomy is often difficult because other injuries, particularly a cerebrocranial trauma, may obscure the clinical symptoms. Diagnostic peritoneal lavage may help towards differentiating between a ventilation-induced pneumoperitoneum and an "acute abdomen" caused by perforatio

Post-cesarean pneumoperitoneum beyond expected revealing a sealed cecal perforation: a case report.

BACKGROUND: Small volumes of intraperitoneal air can follow cesarean delivery, but persistence or progression beyond the immediate postoperative period is unusual and may indicate hollow-viscus injury. CASE PRESENTATION: A 30-year-old gravida 5, para 3, abortus 1 Palestinian female with prior right ovarian cystectomy underwent an uncomplicated elective lower-segment cesarean section and was discharged on postoperative day 2. She re-presented on postoperative day 4 with colicky abdominal pain, distension, and subjective fever. The examination showed tachycardia and a distended, tympanitic abdo

Beyond pneumoperitoneum: technical feasibility and predictive factors for gasless laparoscopic cholecystectomy in dogs.

This study evaluated the safety and feasibility of gasless laparoscopic cholecystectomy in dogs, identifying factors associated with surgical time and survival. Twenty-five dogs with symptomatic gallbladder disease-predominantly gallbladder mucocele (72.0%) and cholelithiasis-underwent cholecystectomy using a mechanical abdominal wall lift system. Variables including gallbladder volume, age, body condition score, and intraoperative complications were recorded. Statistical analyses employed Pearson's chi-squared, Spearman's correlation, regression analysis for surgical time predictors, and Kapl

Management of residual spaces after lung resections and air leaks: Ultrasound and pneumoperitoneum.

Prolonged chest tube drainage is one of the most common postoperative complications of pulmonary resections; it is related to complications such as residual pleural spaces or continuous alveolar air leaks. We retrospectively evaluated the efficacy of artificial intraoperative pneumoperitoneum in the treatment of such complications after lung resections. The presence of a residual space associated with prolonged air leaks can be difficult to treat, exposes the patient to a high risk of infection, prolongs hospitalization, and in some cases mandates reoperation. Between October 2016 and March 20

Morphologic Change of In Vivo Porcine Liver Under 13 mm Hg Pneumoperitoneum Pressure.

BACKGROUND: Clinically, the total and residual liver volume must be accurately calculated before major hepatectomy. However, liver volume might be influenced by pneumoperitoneum during surgery. Changes in liver volume change also affect the accuracy of simulation and augmented reality navigation systems, which are commonly first validated in animal models. In this study, the morphologic changes in porcine livers in vivo under 13 mm Hg pneumoperitoneum pressure were investigated. MATERIALS AND METHODS: Twenty male pigs were scanned with contrast-enhanced computed tomography without pneumoperit
