Priapism: pathophysiology and the role of the radiologist.

Priapism is defined as a penile erection that persists for 4 h or longer and is unrelated to sexual activity. Its identification is important as lack of timely treatment (particularly of the low flow/ischaemic subgroup) can result in persisting erectile dysfunction as a consequence of irreversible corporal fibrosis. This review describes the physiology and anatomy of the normal erection, the aetiology and pathophysiology of the different types of priapism, and the role of the radiologist in the management of the condition. The treatment of iatrogenic priapism following intracavernosal injectio…

Medical and surgical management of priapism.

Priapism is a pathological condition of penile erection that persists beyond, or is unrelated to, sexual stimulation. Pathologically and clinically, two subtypes are seen-the high flow (non-ischaemic) variety and the low flow (ischaemic) priapism. The low flow type is more dangerous, as these patients are susceptible to greater complications and the long term recovery of erectile function is dependent on prompt and urgent intervention. Many of the causes of priapism are medical, including pharmacological agents, and as such, priapism should be considered as a medical and surgical emergency.

Priapism and quetiapine: a case report.

Priapism is a "persistent erection not accompanied by sexual desire or stimulation, usually lasting more than six hours and typically involving only the corpora cavernosa." Here we report on a gay male patient from our HIV/AIDS mental health clinic who developed serious priapism on quetiapine and recreational amphetamine. Gay men are at high risk for amphetamine use, and as such, this potential association between priapism, quetiapine, and amphetamine use should be considered in making prescription decisions with these patients.

Idiopathic priapism in a young boy.

We report a case of recurrent priapism in a boy. There were no recurrences during or after oral treatment with Bricanyl (terbutaline).

Priapism--a review of the medical management.

Priapism is characterised by the presence of prolonged, often painful penile erection in the absence of a sexual stimulus. This rare condition has a range of aetiologies, but is most common following self-administration of injection therapy for impotence. Priapism may be classified into high- and low-flow states. Low-flow priapism is an emergency ischaemic condition requiring prompt recognition and treatment to avoid devastating long-term complications of erectile dysfunction. Wide-ranging medical therapies are covered in this review. Diagnostic and treatment algorithms are suggested in light …

Priapism as a complication of heparin therapy.

One case of priapism occurred during heparin therapy for a previous surgical operation to the knee is reported. The association between heparin and priapism is often recognized; abnormal platelet aggregation could play a role in the pathogenesis of this side effect. A part from the kind of surgical treatment performed, priapism as a complication of heparin therapy might be functionally more severe than priapism due to other etiologies. Our observation would confirm this statement.

New aspects in the treatment of priapism.

Recent advances in the understanding of erectile physiology have improved the prompt diagnosis and treatment of priapism. During initial assessment, the physician must distinguish between veno-occlusive low flow (ischemic) and arterial high flow (nonischemic) in order to choose the correct treatment option for each type of priapism. Patient history, physical examination, penile haemodynamics and corporeal metabolic blood quality assist the distinction between static and dynamic priapism. Normally, priapism is effectively treated with intracavernous vasoconstrictive agents or surgical shunting.…

Priapism: an unusual presentation of appendicitis.

Priapism, a persistent abnormal penile erection that is painful and unassociated with sexual interest, is known to be associated with a wide range of etiologies in children. This is a report of a case of appendicitis in an eight year old presenting with priapism. There is no mention in the pediatric literature of priapism associated with appendicitis. Possible appendicitis should be included in the differential diagnosis of priapism.

Alternative approaches to the management of priapism.

Herein we describe the use of intracavernous methylene blue (MB), a guanylate cyclase inhibitor, or internal pudendal artery embolization for the treatment of priapism. Eleven patients with priapism were treated from 1993-1996. Etiologies of priapism included PGE1/papaverine (3), trazodone (2), and sickle cell disease (1), in the other five cases the causes the cause was unknown. The average duration of priapism was 27 h for all patients (6-72 h). Five patients who failed intracavernous MB or an alpha-adrenergic agonist, underwent unilateral or bilateral pudendal artery embolization. The avera…

[Management of priapism induced by PGE1].

OBJECTIVE: To review the literature on priapism following intracavernous injection of PGE1, its management and the physiopathological mechanisms involved. METHODS/RESULTS: The literature on PGE1-induced priapism available on Medline is reviewed and two cases treated at our hospital are described. The diagnostic and therapeutic algorithms are presented. The relationship between drug use and priapism is difficult to establish since patients with the same history who received the same dose of Alprostatil did not develop priapism. CONCLUSIONS: Although infrequent, treatment must be instituted ur…

Intracavernous methoxamine in the treatment of priapism.

Methoxamine is an alpha-adrenergic drug, its unique pharmacokinetics and mechanism of action on alpha-1 receptors lead to consider it, similarly to phenylephrine, as a first-choice drug for treating drug-induced or veno-occlusive priapism. The experience obtained with its use in the management of three cases of priapism lasting over 7 h and one case of sustained rigid erection caused during anesthetic induction are reported.

Intracorporeal phenylephrine in the treatment of priapism.

PURPOSE: The effectiveness and complications of intracorporeal phenylephrine without aspiration or irrigation as a treatment for priapism were assessed. MATERIALS AND METHODS: Nine consecutive patients who presented with priapism were treated with 0.5 mg. phenylephrine diluted in 2 cc normal saline injected directly into the corpus cavernosum. Blood pressure and pulse were measured before and after injection, and monitored every 15 minutes. If no detumescence was noted after 15 minutes the injection was repeated. Neither penile blood aspiration nor irrigation was performed. RESULTS: Of 9 pat…

Oral terbutaline for the treatment of priapism.

The widespread use of penile injections in the management of erectile dysfunction has led to an increased incidence of priapism. It has been suggested that terbutaline, a beta-agonist, might be beneficial in treating priapism in this setting. We randomized 24 patients with prolonged erections to a prospective, double-blind, placebo-controlled trial. We did not find any benefit of oral terbutaline over placebo in these patients.

[Role of echography in neoplastic priapism].

The author, observing one case of neoplastic priapism, poiting on the role of ultrasonography in this disease. Its easy and quick execution, the possibility to repeat it, its low cost, its diagnostic sensibility in evaluating the infiltration of the cancer, permit us to consider it a screening technique of primary importance.

Treatment of pharmacological priapism with phenylephrine.

Pharmacological prolonged erections or priapism due to anesthesia were treated in 36 patients with intracorporeal injections of phenylephrine. Detumescence was obtained rapidly in every case and no marked side effect was reported. Considering the possible toxicity of other adrenergic agents, phenylephrine is strongly recommended in the treatment of priapism.

Penile prosthesis in the management of priapism.

Priapism is the abnormal persistence of erection, painful and not sexually induced. Many cases resolve spontaneously, but if it persists for a long time there is anoxic injury and fibrosis of the corporal tissues and the sequelae is impotence. The main problem with priapism is pain and the probability of a resultant impotence. We have managed 2 patients with recurrent priapism with insertion of penile prosthesis with satisfactory results.
