The forest and the trees: relational and specific factors in addiction treatment.

AIMS: Increased expectations for the use of evidence-based methods in addiction treatment have fueled a debate regarding the relative importance of 'specific' versus 'common' factors in treatment outcome. This review explores the influence of these factors on addiction treatment outcome. METHODS: The authors review and link findings from four decades of research on specific and general factors in addiction treatment outcome research. FINDINGS: Although few would argue that what one does in addiction treatment is immaterial, outcome studies tend to find small to no difference when specific tr

Practical tips for sexual counseling and psychotherapy in premature ejaculation.

INTRODUCTION: A number of milestones in the treatment of premature ejaculation (PE) have occurred over the past five decades, including the development of various behavioral and cognitive techniques as well as pharmacotherapies that modify neurophysiological processes involved in ejaculation. Nevertheless, the notion that sexual responses such as PE are influenced by physiological, psychobehavioral, cultural, and relationship factors is as valid now as it was 50 years ago, and therefore, interventions should consider all such domains in the development of effective treatment strategies. AIM:

Expressed emotion: a call for partnership among relatives, patients, and professionals.

The emotional climate within the home is a powerful predictor of relapse and remission in schizophrenic disorders. Research has focused on "expressed emotion" (EE), a measure of relatives' expressed attitudes about the patient. Recently, EE has become a focus of controversy. Correcting a misperception that EE research blames families of schizophrenic patients, we clarify the concepts and data base that underlie EE research, provide an operational definition of EE, and briefly review the history of EE research in schizophrenia and other disorders. Then, methodological and conceptual issues in t

Healthcare professionals' accounts of challenges in managing motor neurone disease in primary healthcare: a qualitative study.

Motor neurone disease (MND) is a progressive neurological disease causing muscle wasting, gradual paralysis and respiratory failure, with a life expectancy of 2-4 years. In order to better understand how MND is managed in the community, we conducted a qualitative study to explore the challenges healthcare professionals encounter when managing MND in primary healthcare. Based on data from 15 semi-structured interviews with primary healthcare professionals in Norway, we found that MND is viewed as a condition that requires exceptional effort and detailed planning. Healthcare professionals report

CE: Inside an Ebola Treatment Unit: A Nurse's Report.

In December 2013, the first cases of the most recent outbreak of Ebola virus disease (formerly known as Ebola hemorrhagic fever) emerged in the West African nation of Guinea. Within months the disease had spread to the neighboring countries of Liberia and Sierra Leone. The international humanitarian aid organization Médecins Sans Frontières (MSF; known in English as Doctors Without Borders) soon responded by sending staff to set up treatment centers and outreach triage teams in all three countries. In August 2014, the World Health Organization declared the outbreak an international public heal

[Pilot Study of "Japanese Model of Asheville Project"~How Did It Work?].

For prevention of the aggravation of diabetic nephropathy, a treatment method that combines self-care with medical guidance is becoming increasingly important, leading to the development of programs for lifestyle modification for the patients. To assess the effectiveness of such programs, we have conducted a feasibility study of a patient self-care support program with medical collaboration by registered pharmacists in community pharmacies involving patients with diabetic nephropathy who are under treatment at medical institutions, including our hospital. This study evaluated the two primary m

Factors shaping return to work: a qualitative study among heart failure patients in Denmark.

PURPOSE: For people of working-age diagnosed with heart failure, return to work (RTW) is often a significant rehabilitation goal. To inform vocational rehabilitation strategies, we conducted a qualitative study aiming at exploring patient experienced support needs, and barriers and facilitators to RTW. MATERIALS AND METHODS: Ten men and eight women with heart failure (48-60 years) were interviewed in Denmark during 2022. A thematic analysis was conducted using the Sherbrooke model as framework. RESULTS: Multiple factors operating at different levels shaped participants' RTW processes. Person

"The one that got away"- therapists' experiences when patients suddenly drop out from psychotherapy: a thematic analysis.

OBJECTIVE: The objective of the study was to examine how therapists in clinical practice experience sudden dropouts from psychotherapy. METHOD: We interviewed 12 licensed psychotherapists regarding sudden dropouts. The interviews were analysed using thematic analysis (TA). RESULTS: Three main themes and seven subthemes were identified. The first main theme was: Struggling to understand and explain the dropout. The subthemes were: "the patients' problem was too difficult", "trouble cooperating" and "problematic emotional bond". The second main theme was: Continuing when something is wrong. Th

The interplay of clients' role expectations with working alliance: A multilevel analysis.

Research on clients' psychotherapy expectations has predominantly focused on the associations of clients' role expectations with working alliance and psychotherapy outcomes. However, limited research has examined their moderating role in multilevel alliance-outcome associations. Utilizing naturalistic data from a Chinese training clinic (6,400 observations nested within 1,016 clients and 176 therapists), the present study conducted multilevel analyses to examine the moderation effects of clients' role expectations, including expectations about client involvement, counselor expertise, and facil

Assessing the perioperative communication needs of a patient with learning disabilities: An holistic case study approach.

Patients should be at the heart of everything we do. The aim should always be to achieve healthcare outcomes by involving patients fully in their own care, with decisions made in partnership with clinicians, rather than by clinicians alone: 'no decision about me, without me' (DH 2012). This article details a plan of care for an 18 year old male patient with a moderate level of learning disability who was scheduled for a tonsillectomy at a local NHS trust hospital. It focuses on the management of the patient's anxiety, nausea and pain. In accordance with the Health and Care Professions Council'

Comprehensive care in Huntington's disease: a physician's perspective.

Huntington's disease is a slowly progressive neurodegenerative disorder with wide-ranging effects on affected individuals and their families. Until a cure is found for the disease, patients and their families will continue to need care over years, even generations. The ideal care for HD is provided by a team, led by a physician, with input from rehabilitation therapists, nurses, psychologists, genetic counselors, social workers, and other health care providers. The goals of care are to maximize the quality of life at all points through the course of the disease, in part by anticipating problem

Assessment factors associated with premature psychotherapy termination.

Characteristics of patients accepted for both intensive psychotherapy and psychoanalysis were rated during their initial assessments. Twenty cases that terminated prematurely (most within the first month) were compared with twenty cases that continued in therapy. While neither specific diagnosis, type of insight therapy, nor gender of the patient or therapist was a reliable predictor of premature termination, it was found that psychodynamic and environmental assessment factors significantly differed between these two groups. In those patients who eventually dropped out, specific ego deficits,

Women and medicalisation: an empirical examination of the extent of women's dependence on medical technology in the early detection of breast cancer.

Recent criticisms of the thesis of medicalisation focusing at the level of doctor-patient interaction have suggested that patient dependency on or addiction to modern scientific medicine and in particular medical scientific technology is overstated. However, empirical research to examine this is difficult to find. The study presented here throws some light on this issue as it looks at women's beliefs and feelings about breast screening and breast self-examination and the prospect of attending for breast screening or a class which teaches BSE. The results show that the generalised concept of pa

How patients perceive the therapeutic communications skills of their general practitioners, and how that perception affects adherence: use of the TCom-skill GP scale in a specific geographical area.

BACKGROUND: To study: (1) the structure and test-retest reliability of a measure of how patients perceive the therapeutic communications skills of their general practitioners (TCom-skill GP), and (2) the associations of that scale with socio-demographic and health-related characteristics, and adherence. METHODS: A total of 393 people who lived in the same geographic area and invited to attend a preventive medical centre for a check up were asked to complete a self-administered questionnaire concerning TCom-skill GP (15 items), socio-demographic and health-related characteristics, and to answe

Palliative care and orthopedics: what is on the horizon?

Patient-centered palliative care involves effective and empathetic communication between the physician and patient, with the ultimate goal of providing improved quality of life for individuals who have terminal illnesses. Although several nonoperative and operative treatment options are available to improve the patient's quality of life, providing palliative care requires more than technical expertise and knowledge. Orthopedic surgeons need to be supportive facilitators who inform patients of the available treatments, risks, and benefits and incorporate these treatments into the wishes of pati

Don't keep women waiting on a diagnosis.

In December 1999, the administration at Grant/Riverside Methodist Hospitals in Columbus, OH, launched a benchmark study to determine how long it took for a woman diagnosed with a breast abnormality to find out if she had breast cancer. It was determined that it took an average of 27 days, better than the national average of 42 days, but far below the gold-standard programs around the country that took 24 hours to five working days. To remedy the situation, Patti Dunn, RN, BSN, OCN, was asked to oversee the process and create a system that coordinated care across the continuum to better meet th

Adverse Events During Dental Care for Children: Implications for Practitioner Health and Wellness.

Adverse events during dental care for children inevitably happen, despite precautionary procedures. Remedial measures have primarily focused upon affected children and their parents/caregivers. The purpose of this paper was to summarize the effects of adverse events upon practitioners involved in the incident who have been termed second victims. Affected practitioners may suffer negative emotions impinging upon their professional performance as well as deleterious personal health consequences, including substance abuse and depression/suicidal ideation. Peer support has been identified as an im

The Continuum of Disclosure: Exploring Factors Predicting Tentative Disclosure of Child Sexual Abuse Allegations During Forensic Interviews and the Implications for Practice, Policy, and Future Research.

When a child sexual abuse investigation ensues, many children do not disclose readily to professionals. Defining disclosure beyond the disclosure versus nondisclosure dichotomy is essential, yet little research exists on factors associated with a continuum of disclosure, including active and tentative disclosure. Through the coding of 196 forensic interviews using content analysis and subsequent regression analysis, findings suggest that children of color, children abused by adults, unintentional initial disclosure, and those lacking family support were more likely to tentatively disclose in t
