Neurological Symptoms, Not Signs,<br />Are Common in Early HIV Infection.

UNLABELLED: Objective. To examine the cross-sectional prevalence of neurological symptoms and signs in a large cohort of human immunodeficiency virus (HIV)-seropositive men, and determine the relationship of the symptoms to disease stage, immunologic markers, and independent variables from neuropsychological (NP) testing and psychiatric interview. METHODS: One hundred-nine controls and 386 HIV-infected volunteers enrolled in the HIV Neurobehavioral Research Center (HNRC) longitudinal study. The majority, without acquired immune deficiency syndrome (AIDS), were screened for alcohol/substance a

Aluminum Microcomb Electrodes on Silicon Wafer for Detecting Val66Met Polymorphism in Brain-Derived Neurotrophic Factor.

OBJECTIVE: Brain-derived neurotrophic factor (BDNF) dysregulation is widely related with various psychiatric and neurological disorders, including schizophrenia, depression, Rett syndrome, and addiction, and the available evidence suggests that BDNF is also highly correlated with Parkinson's and Alzheimer's diseases. METHODS: The BDNF target sequence was detected on a capture probe attached on aluminum microcomb electrodes on the silicon wafer surface. A capture-target-reporter sandwich-type assay was performed to enhance the detection of the BDNF target. RESULTS: The limit of detection was

The Fire Service for the Removal of a Metallic Penile Constricting Device: A Ready Help When All Else Fail.

The placement of a constricting device around the penis is a urologic emergency. Though injuries from constricting penile devices are generally rare, they may be associated with serious complications. There is no standard modality for the removal of penile constricting devices and the management of the patient can therefore prove to be a formidable challenge to the urologist. Timely intervention is always important in preventing complications especially penile gangrene. Depending on the type of device used along with the duration and severity of penile constriction caused, significant resource

Too Much to Bear: Psychometric Evidence Supporting the Perceived Burdensomeness Scale.

BACKGROUND: People who feel they have become a burden on others may become susceptible to suicidal ideation. When people no longer feel capable or productive, they may assume that friends and family members would be better off without them. AIM: The present study was designed to assess preliminary psychometric properties of a new measure, the Perceived Burdensomeness (PBS) Scale. METHOD: Depressed psychiatric patients (N = 173) were recruited from a veterans affairs medical center. Patients were assessed with a structured diagnostic interview and self-report measures assessing perceived burd

[Neural prostheses and neuromodulation : Research and clinical practice in therapy and rehabilitation].

Stimulation of the nervous system with the aid of electrical active implants has changed the therapy of neurological diseases and rehabilitation of lost functions and has expanded clinical practice within the last few years. Alleviation of effects of neurodegenerative diseases, therapy of psychiatric diseases, the functional restoration of hearing as well as other applications have been transferred successfully into clinical practice. Other approaches are still under development in preclinical and clinical trials. The restoration of sight by implantable electronic systems that interface with t

Triple jeopardy for HIV: substance using Severely Mentally Ill Adults.

Severely Mentally Ill (SMI) adults have disproportionately high HIV seroprevalence rates. Abuse of alcohol and other substances (AOD) and lifetime exposure to trauma by others are particularly potent risk factors, which, in combination with psychiatric disabilities, create triple jeopardy for HIV infection. This study examined the predictive utility of demographic characteristics; history of physical, emotional, or sexual abuse; extent of drug and alcohol abuse; knowledge about HIV/AIDS; sexual self-efficacy; and condom attitudes toward explaining the variance in a composite of HIV high-risk b

Neuropsychiatric adverse events of bupropion treatment: a brief update.

The clinical use of bupropion as an aid for smoking cessation has raised some concern in clinicians and public opinion. Clinical and post-marketing surveillance studies indicate a favourable side effect profile, but spontaneous reports of suspected adverse reactions to medical authorities indicate adverse neuropsychiatric events to be more frequent than previously thought. Except for the risk of seizures, adverse neuropsychiatric events have not been examined systematically. Insomnia has been reported quite frequently. In the placebo-controlled trials, depression scores were unaffected by bupr

Alpha EEG activity and subcortical pathology in HIV infection.

Quantitative electroencephalographic (QEEG) investigations of patients with HIV-1-infections were made to detect early signs of HIV brain involvement. QEEG data were recorded in a prospective controlled cohort study together with standardised clinical, immunological and neuropsychological tests. Subject groups were HIV-seronegative controls, HIV-seropositive subjects with asymptomatic infection, and HIV-seropositive subjects with symptomatic infection exclusing AIDS defining illnesses. Marked increase in background alpha amplitude preceded cognitive and neurological impairment in the symptomat

[Drug-induced dystonia misinterpreted as hysteria].

Psychic distress is often expressed in the form of physical pain or disease, but the converse also occurs. Illnesses with an organic aetiology are sometimes misdiagnosed as psychogenic. We describe three patients who developed rare forms of acute drug-induced dystonia when treated with antipsychotic drugs. All three cases were initially misdiagnosed as "hysteria" because the patients had psychiatric illnesses and because the symptoms were bizarre and became worse when the patients became very anxious. Furthermore, if the patients were helped to relax the symptoms disappeared for a moment. One

[Sleep disorders and psychiatry. Keys for trans-nosological symptomatology, genetic epidemiology and dynamic pathogenesis].

Sleep studies in psychiatry collect various disorders in the category of "dysomnia", and sleep EEG studies today can be routinely performed using appropriate autonomous devices. Three major needs emerge from a broad approach of the topic: a transnosological view on symptomatology--dysomnia gets its meaning from the context, a genetic epidemiology--dysomnia appears as a risk factor for psychiatric disorders and a marker of depressive vulnerability, and as a dynamic pathogeny. A wide consensus arises in the field of polysomnographic studies claiming a clear distinction between two opposite sleep

Performance of patients with early HIV-1 infection on the Stroop Task.

HIV-seropositive and seronegative control subjects performed a standard paper-and-pencil version and an experimental reaction time version of the Stroop Color-Word Naming Task. Results indicated that both symptomatic and asymptomatic HIV-positive subjects showed an exaggerated Stroop effect compared to controls, but this increase was only apparent on the RT version of the task. Analysis of components of the effect indicated that HIV-positive subjects showed increased inhibition compared to controls but normal facilitation. These results suggest that HIV-related cognitive slowing has an attenti

The Use of Cannabis-Based Medicine in Selected Neurological Disorders.

Cannabis-based medicine (CBM) is used in a wide variety of different neurological disorders. While the use of CBM in the treatment of pain, AIDS wasting, loss of appetite, and spasticity is well established, CBM application in movement disorders and neurodegenerative disorders is still an emerging topic. The purpose of this chapter is to summarize current evidence behind the use of CBM in selected neurological diseases, mainly movement and neurodegenerative disorders. The best evidence for efficacy of CBM is for Tourette syndrome resulting in an improvement of tics and psychiatric comorbiditie

Avoidant/Restrictive Food Intake Disorder: Review and Recent Advances.

Avoidant/restrictive food intake disorder (ARFID) is an eating disorder recently codified in DSM-5 that affects individuals of all ages. A proliferation of ARFID research has emerged over the years, and this review provides a brief overview of the current understanding of ARFID epidemiology, symptoms, comorbid conditions, assessment, and treatment. The review highlights recent research updates regarding ARFID among adults, putative neurobiological mechanisms underlying ARFID, and new treatment trials. Findings from this review demonstrate that ARFID is as prevalent as other eating disorders, e

Retinal injury from handheld lasers: a review.

Retinal photic injury induced by handheld lasers is a burgeoning public health concern due to the wider accessibility of high-powered devices. Retinal damage from thermal energy can cause potentially severe and permanent vision loss in children and young adults who are particularly vulnerable because of comorbid behavioral, learning, and psychiatric impairments. Understanding the spectrum of specific clinical and imaging features of such laser injuries aids in prompt and accurate diagnosis. Multimodal retinal imaging is important for the identification of the outer retinal abnormalities that c

Huntington's Disease: Relationship Between Phenotype and Genotype.

Huntington's disease (HD) is an autosomal dominant inherited neurodegenerative disease with the typical manifestations of involuntary movements, psychiatric and behavior disorders, and cognitive impairment. It is caused by the dynamic mutation in CAG triplet repeat number in exon 1 of huntingtin (HTT) gene. The symptoms of HD especially the age at onset are related to the genetic characteristics, both the CAG triplet repeat and the modified factors. Here, we reviewed the recent advancement on the genotype-phenotype relationship of HD, mainly focus on the characteristics of different expanded C

Stiff-person syndrome presenting as eating disorder: a case report.

OBJECTIVE: To report a case of a 35-year-old female initially treated for anorexia nervosa who was found to have Stiff-Person Syndrome (SPS). METHOD: Case report. RESULTS: The patient reported panic attacks at meal times that were found to consist of tetanic contractions of the axial musculature. Swallowing initiated reflexive painful muscle spasms that consequently resulted in cibophobia and significant weight loss. Her serum tested positive for anti-glutamic acid decarboxylase antibodies, and she subsequently improved with appropriate treatment for SPS. DISCUSSION: SPS has not been previo

HIV, cognition and women.

Although the incidence of HIV in the United States is higher among men compared to women, the global proportion of women versus men who are infected has been approximately 50% since the late 1990s. Women have been under-represented in neuropsychological studies of HIV. A small number of studies have reported a significantly higher prevalence of neurocognitive impairment among HIV+ women compared to HIV- controls regardless of symptom status and with or without an AIDS diagnosis. Impairment was most evident on psychomotor tasks. The risk of neuropsychological impairment was increased among HIV+

Treatment of opioid-dependent pregnant women: clinical and research issues.

This article addresses common questions that clinicians face when treating pregnant women with opioid dependence. Guidance, based on both research evidence and the collective clinical experience of the authors, which include investigators in the Maternal Opioid Treatment: Human Experimental Research (MOTHER) project, is provided to aid clinical decision making. The MOTHER project is a double-blind, double-dummy, flexible-dosing, parallel-group clinical trial examining the comparative safety and efficacy of methadone and buprenorphine for the treatment of opioid dependence in pregnant women and

Dermatitis artefacta.

BACKGROUND: Dermatitis artefacta (DA) is difficult to diagnose by clinical findings alone. Histopathologic analysis may rule out organic disorders and in some cases establish, in conjunction with microanalytic techniques, a definite diagnosis. OBJECTIVE: We report a young woman with DA in the form of recurrent abscesses whose diagnosis was aided by the identification of subcutaneous foreign material through histologic and microanalytic methods. METHODS: Histologic examination showed foreign body giant cell reaction with polarizable material, which was identified as cellulose by energy disper

Blood-stained letter received by HIV/AIDS reference laboratory in 1993--a historical case report.

We describe an interesting historical case of a blood-stained letter received in 1993 by Slovenian HIV/AIDS Reference Laboratory. According to the statement of the sender, the letter was spotted with HIV-infected blood. The polymerase chain reaction (PCR) amplification with two gag and env primer sets excluded the presence of HIV proviral DNA in the spot punches obtained from the dried blood spots. Although the presence of HV DNA was confirmed in 5 positive control blood spots using the same sets of primers, we still doubted in the accuracy of the HIV negative result. Fortunately, we obtained
