Moving toward precision communication in pediatric oncology clinical trials: An outline of a conceptual framework to support caregiver decision-making.

Pediatric oncology clinical trials (POCTs) are the cornerstone of therapeutic progress in childhood cancer. Yet, poor accrual and retention remain persistent challenges contributing to early termination of POCTs, limiting the advancement of novel therapies. These challenges reflect a convergence of system, protocol, clinician, and family-level factors, among which caregivers' psychosocial decision-making processes represent a potentially modifiable but underexplored contributor. Caregivers making POCT-related decisions must navigate complex medical information under conditions of heightened em

The centrality of healthcare and education interactions - An Interpretive Phenomenological Analysis of experiences of parents of children with Ehlers-Danlos Syndrome.

BACKGROUND: Ehlers-Danlos Syndrome (EDS) is a rare group of connective tissue disorders and, as such, the diagnosis can often be delayed. While emerging research indicates that there may be adverse psychosocial consequences for the child, little is known about the processes behind such outcomes, including the psychosocial impact of this rare disease on family life. AIMS: To extend our understanding, we examined the lived experiences of parenting a child with EDS. METHODS: Four parents recruited from a specialist child development clinic participated in semi-structured interviews. Data were a

Similarities and Differences Between the Extravascular and the Subcutaneous Implantable Cardioverter-Defibrillator: Sensing, Discrimination, and Complications (Part 2).

PURPOSE OF REVIEW: The subcutaneous ICD (S-ICD) and the extravascular ICD (EV-ICD) both avoid intravascular hardware, but they solve sensing and detection differently. This review compares their sensing architectures, detection and discrimination algorithms, therapy capabilities, and system-specific complications, along with practical considerations including extraction, MRI compatibility, and patient-reported outcomes. RECENT FINDINGS: Inappropriate shock rates have declined substantially for both platforms through algorithmic refinements, two-zone programming and SMART Pass for S-ICD, and i

Reflections on "Building Back Better" Child and Adolescent Mental Health Care in a Low-Resource Postemergency Setting: The Case of Sierra Leone.

Over the past three decades, Sierra Leone has experienced two major humanitarian crises: an armed conflict (1991-2002) and an Ebola virus disease outbreak (2014-2015). In addition to these country-wide crises, the capital Freetown experienced a mudslide affecting thousands of people in 2017. In response to these emergencies, donors and aid organizations showed an increased interest in supporting and implementing mental health and psychosocial support interventions. Despite these efforts, the mental health infrastructure of the country remains frail. Specifically, systemic improvements in the i

The Multidimensional Impact of Traditional Orthopaedic Casting and the Role of Emerging Immobilization Technologies: A Narrative Review.

BACKGROUND: Traditional orthopaedic casting has remained the cornerstone of non-surgical fracture management for more than a century. Although plaster and fiberglass casts reliably stabilize fractures, they are associated with physical, psychological, emotional, social, and economic burdens that extend beyond bone healing. Children, older adults, and individuals with pre-existing vulnerabilities may be disproportionately affected. Despite increasing recognition of these complications, existing orthopaedic literature has historically prioritized radiographic healing and biomechanical stability,

Navigating the Emotional and Practical Challenges of Newborn Screening for Late-Onset Pompe Disease: Insights From Parental Perspectives.

Pompe disease (PD), an autosomal recessive lysosomal disorder, results in glycogen accumulation in muscle cells, leading to progressive muscle weakness and respiratory insufficiency. Newborn screening (NBS) has improved outcomes for infantile-onset PD by enabling early diagnosis and intervention with enzyme replacement therapy. NBS also identifies late-onset PD (LOPD) cases, wherein children have a wide clinical spectrum and may remain asymptomatic for years, placing families in uncertainty as "patients-in-waiting." This study explores parental experiences following an LOPD diagnosis through N

Self-Esteem and Body Satisfaction After Radical Orchiectomy With or Without Testicular Prosthesis: A Retrospective Study at a Mexican Tertiary Center.

Radical orchiectomy remains the standard treatment for localized testicular cancer. However, the resulting anatomical loss may negatively affect body image and self-esteem, especially in young men. The use of testicular prostheses has been proposed as a reconstructive option to mitigate these psychosocial effects, though evidence on its impact on global self-esteem remains limited, particularly in Latin American populations.  Methods: We conducted a retrospective, analytical study among adult patients with non-metastatic testicular cancer who underwent radical orchiectomy at a tertiary public

Psychosocial aspects of cancer pain.

Pain, and especially cancer pain, is not a pure nociceptive, physical experience, but involves different dimensions of man, such as personality, affect, cognition, behavior and social relations. Cancer pain is best conceptualized as the convergence of multiple activated systems with feedback mechanisms to a complex, multidimensional model. The psychosocial aspects of this multidimensional model will be analyzed with special emphasis on results from recent research. Although most research has been conducted on the role of affect and cognition in cancer pain, data on other factors such as person

Early Supportive Versus Standard Care in Phase I Cancer Trials: A Randomized Controlled Trial.

CONTEXT: Patients enrolled in Phase I clinical trials often face substantial physical and psychological challenges. Although supportive and palliative care interventions have been previously investigated in cancer, evidence regarding their early and structured integration in Phase I populations remains limited. Multidisciplinary supportive approaches may improve symptom control and patient-centered outcomes in this context. OBJECTIVES: To evaluate the impact of a structured early supportive care (ESC) program on overall quality of life, encompassing physical, psychological, and social dimensi

Mapping Caregiver Needs' Assessment Tools for Family and Friend Caregivers: A Rapid Scoping Review.

BACKGROUND: Family and friend caregivers provide essential support across health and social care systems but remain inconsistently identified, assessed, and supported in routine practice. Although numerous caregiver needs' assessment instruments exist, many focus on burden, distress, or preparedness rather than explicitly eliciting caregiver-defined support needs, limiting their utility for care planning, care transitions, and system integration. METHODS: We conducted a rapid scoping review to identify and characterize caregiver needs' assessment tools developed for family and friend caregive

Palliative care in cardiovascular medicine.

Cardiovascular disease remains the leading global cause of morbidity and mortality. Although advances in prevention, diagnostics, and disease-modifying therapies have prolonged survival, many individuals now live longer with high symptom burden, functional decline, and complex decisional needs. Palliative care (PC) for adults with advanced cardiovascular disease can improve quality of life, support caregivers, and align treatments with patient values and goals. Core elements include symptom management, effective communication, shared decision-making, advance care planning, and integration of p
