We detected peripyloric lymph node metastasis in 2 of 24 patients with ductal adenocarcinoma of the pancreas who underwent Kausch-Whipple's procedure. The resection would have been palliative if these patients were treated by pylorus-preserving pancreatoduodenectomy.

We have noted that sonographically positive cases of hypertrophic pyloric stenosis (HPS) demonstrate a distortion of normal anatomy in which the elongated pylorus displaces the duodenum and lies adjacent to and below the gallbladder. Ten consecutive cases of HPS diagnosed by ultrasound were reviewed. In each case the pylorus was imaged subjacent to the gallbladder. Between 1986 and 1993 fifty-two patients with ductal adenocarcinoma of the pancreatic head underwent pancreatoduodenectomy, 34 in a standard Whipple technique (Whipple), 18 since 1990 preserving the pylorus (PPPD). Operating time was significantly longer for Whipple compared to PPPD (5.5 +/- 1.4 vs. 3.8 +/- 1.0; p < 0.01). The authors purpose was to demonstrate that pyloromyotomy can be successfully carried out by nonspecialist surgeons. Eighty three cases admitted over a twelve-year period to the Department of Pediatrics of Montecelo Hospital, a district general hospital, were reviewed. The time-honored surgical approach to resectable tumors of the head of the pancreas or distal common bile duct has been a Whipple operation for adults and children. In recent years, pylorus-preserving pancreatoduodenectomy has been used for adults. We investigated whether duodenal reflux through the pylorus is involved in the development of gastric cancer. Male Wistar rats weighing 230-250 g were subjected to three types of operative procedures: (i) allowing reflux through the pylorus; (ii) allowing reflux through a gastrojejunal stoma; and (iii) gastrotomy. Plasma levels of gastrin, adrenocorticotropin (ACTH) and somatotropin (STH) were determined 24 hours after pylorus ligation and after pylorus ligation and vagotomy by radioimmunoassay. Pylorus ligation increases the serum levels of these hormones, while vagotomy inhibits the production of these ones. We conclude that the antiulcerogenetic effect of surgical vagotomy is closely related with the changes in the level of these hormones.

We performed a study concerning the relationship between hypertrophic pyloric stenosis (HPS), atopy and cow's milk protein allergy (CMPA). Pylorus ligation stimulates the gastric acid output by a long vago-vagal reflex in innervated animals and by an intramural reflex in chronically vagotomized animals. The mechanism of action of cholecystokinin octapeptide (CCK-OP) on tonic and phasic contraction of antral, pyloric, and duodenal smooth muscles was studied with a novel perfusion manometric system in isolated esophagogastroduodenal preparations of the rat.
