A schwannoma of the larynx is a rare benign tumor that usually presents as a submucosal mass in the pyriform sinus and the aryepiglottic space, and this type of schwannoma constitutes a diagnostic and therapeutic challenge for otolaryngologists. We present here two cases of supraglottic schwannomas that were misdiagnosed as laryngoceles. Hypopharyngeal stenosis after total laryngectomy has been reported as high as 40%. Seven cases of acute suppurative thyroiditis are described. Six patients had a recurrent painful swelling of the left anterior neck and one was seen at her first episode of the disease. A barium meal revealed a fistula originating from the apex of the left pyriform sinus in all cases. The fistula, a remnant of the fourth pharyngeal pouch, thus seems to be a common route of infection in acute suppurative thyroiditis, allowing bacterial infection to begin in the perithyroidal space and spread to the thyroid gland. Complete extirpation of the fistula is required for a permanent cure.

BACKGROUND: Piriform fossa sinus tracts (PFSTs) are a recognized cause of recurrent deep neck infections in the pediatric population. Conventional management has historically required open resection, but over recent years minimally invasive endoscopic approaches to obliterate the pharyngeal opening of the sinus have been performed in many centers. However, there is a lack of clear evidence regarding the success rate and safety of these approaches.

Stricture oesophagus is an unusal complication of thyroid surgery. There is only one case of such nature recorded in literature, which shows that the surgeon who operated may not have had any previous experience of head and neck surgery. Locating the stricture was difficult, more so planning the treatment modality was problematic. Informed consent for oesophagectomy with gastric pull through was taken. However when we explored the neck the stricture was seen just below the level of pyriform sinus. It could be successfully excised without end to end anastomosis.

This retrospective analysis of 100 patients revealed a postoperative pharyngocutaneous fistula rate of 15.3% following total laryngectomy, and 21.4% following partial pharyngolaryngectomy. All fistulae were benign. None resulted in mortality. The most significant risk factor for the development of fistulae is prior radiotherapy, especially high dose (greater than 5000 rads).
