Despite longstanding evidence of racism's harmful impacts on health, most health professionals lack training in structural justice and equity. To address this, leaders at Boston Medical Center developed the Davidoff Executive Fellowship in Health Equity. This report describes its design, early outcomes, and influence on broader institutional equity initiatives.

At the interpersonal level, experiences of bias, stigma, and racism from staff in healthcare and treatment settings were common. BACKGROUND: Evidence about the association between structural racism and mortality in the United States is limited. We examined the association between ongoing structural racism, measured as inequalities in adulthood income between White and Black children with similar parental household income (economic mobility gap) in a recent birth cohort, and Black-White disparities in death rates (mortality gap) overall and for major causes.
METHODS: Sex-, race/ethnicity-, and county-specific data were used to examine sex-specific associations between economic mobility and mortality gaps for all causes …

RESULTS: Among 1,055,338 patients, 18.4% (n=194,002) experienced complications, including 4.4% (n=46,356) major and 16.0% (n=169,361) minor complications. Complication rates increased proportionally across VVI …

The COVID-19 pandemic has had devastating health, economic, and social effects on people throughout American society. At the same time, racism spread alongside the virus. As China was blamed for COVID-19, Asian Americans became targets of prejudice and discrimination. In this study we document the racism experienced by Asian Americans during the pandemic and explore its link to mental health. Using the National Couples' Health and Time Study's Asian American oversample, we investigate the relationship between discrimination and COVID stress and mental health outcomes during the pandemic (anxie…

The year 2020 brought unprecedented challenges and renewed focus on racial disparities and inequities in the United States. For racial and ethnic minority groups, and in particular African Americans, racial disparities have been a constant presence and threat from the time of slavery through the present day. These racial disparities, sanctioned and maintained by institutional racism, manifest in all aspects of life for African Americans-segregated and unequal education and housing systems, health and mental health care disparities, disproportionally elevated incarceration rates, and, as painfu…

RESULTS: Cross-sectionally, discrimination during the COVID-19 pandemic was associated with greater sleep difficulties. Feelings of vulnerability in light of Asian hate crimes were additionally associated with greater sleep difficulties. Neither d …

Health crises have a disproportionate impact on communities that are marginalized by systems of oppression such as racism and capitalism. Benefits of advances such as in the prevention and treatment of HIV disease are unequally distributed. Intersecting factors including poverty, homophobia, homelessness, racism, and mass incarceration expose marginalized populations to greater risks while limiting access to resources that buffer these risks. Similar patterns have emerged with COVID-19. We identify comparable pitfalls in our responses to HIV and COVID-19. We introduce health justice as a frame…

Syphilis control programs and research received fewer resources and attention compared to HIV and other sexually transmitted infections (STIs) in the pre-pandemic era. The neglect of syphilis within comprehensive STI control efforts may be related to diagnostic (poor diagnostics), historical (legacies of racism in research), public health (limited partner services), and social problems (limited public engagement). At the same time, there are increasingly compelling reasons to prioritize syphilis control programs and research by harnessing lessons learned and advances during COVID-19. The closu…

Bystander cardiopulmonary resuscitation (BCPR) significantly improves survival rates for out-of-hospital cardiac arrest (OHCA), yet racial disparities persist. Black and Hispanic individuals are markedly less likely to receive BCPR than White individuals, contributing to preventable deaths and inequities in survival outcomes. Structural factors such as educational barriers, implicit bias, and systemic racism contribute to these disparities. This article examines key contributors to racial disparities in BCPR administration and presents targeted, evidence-based solutions. Educational barriers, …

Non-Hispanic Black (NHB) people have a 2.5-fold higher risk of maternal mortality when compared to non-Hispanic White (NHW) people. Neonates of NHB people are more likely to be born preterm and small for gestational age, which may be driven by structural racism. The placenta is very sensitive to the maternal environment and may play a critical role in the translation of environmental stressors to pregnancy outcomes. Our aim was to assess the placental miRNA expression profile in both NHB and NHW people and the association between differentially expressed miRNAs and pregnancy outcomes. Placenta…

OBJECTIVE: The presence of the COVID-19 and racism pandemics underscored the need for a basic model of advocacy for psychologists and budding psychologists. We offer a new model of action to be easily replicated, particularly during times of crisis, with aims to impact change locally and beyond the therapeutic space.
METHOD: The D.C. Psychological Association (DCPA) actively responded to the double pandemics of COVID-19. Using a posthoc qualitative analysis four steps were identified: organizing, providing support and education, and engaging in advocacy (O-SEA Approach), which all centered th…

We assessed tecovirimat treatment equity for 3,740 mpox patients in New York, New York, USA, during the 2022 mpox emergency; 32.4% received tecovirimat. Treatment rates by race/ethnicity were 38.8% (White), 31.3% (Black/African American), 31.0% (Hispanic/Latino), and 30.1% (Asian/Pacific Islander/other). Future public health emergency responses must prioritize institutional and structural racism mitigation.

Accurate pulse-oximeter readings are critical for clinical decisions, especially when arterial blood-gas tests - the gold standard for determining oxygen saturation levels - are not available, such as when determining COVID-19 severity. Several studies demonstrate that pulse oxygen saturation estimated from photoplethysmography (PPG) introduces a racial bias due to the more profound scattering of light in subjects with darker skin due to the increased presence of melanin. This leads to an overestimation of blood oxygen saturation in those with darker skin that is increased for low blood oxygen

Despite heightened awareness of the detrimental impact of hate speech on social media platforms on affected communities and public discourse, there is little consensus on approaches to mitigate it. While content moderation-either by governments or social media companies-can curb online hostility, such policies may suppress valuable as well as illicit speech and might disperse rather than reduce hate speech. As an alternative strategy, an increasing number of international and nongovernmental organizations (I/NGOs) are employing counterspeech to confront and reduce online hate speech. Despite t

There are persistent and profound racialised inequalities in maternal and reproductive health in the UK. Yet in multiple settings, these disparities have been blamed on class or ethnicity, individuals and communities rather than the structures within which they live. In this study, we draw on narratives told within a 'slow-stitch' craft workshop, organised in southern England for racialised women with reproductive trauma, to show how processes of racialisation and racism shape experiences of maternal and reproductive healthcare. Experiences of reproductive trauma were multiple and cumulative. …
