The double-kiss mini-crush (DKMC) technique has been successfully deployed in the past for the treatment of complex coronary lesions even for left main lesions. Our case report consists of a proof-of-principle that the DKMC technique can be successfully translated as well to the field of complex renal artery lesions. Insightful thinking out-of-the "coronary" box in concert with skillful off-label application of coronary stenting procedures may open the gate for unprecedented opportunities for the treatment of difficult-to-tackle in-stent restenosis in the renal circulation.

Takayasu's arteritis is described to be the single most important cause of renovascular hypertension. Anaesthetising a child with Takayasu's arteritis for auto renal transplantation is a challenge as it is complicated by severe uncontrolled hypertension, end-organ dysfunction resulting from hypertension, stenosis of major blood vessels affecting regional circulation, and difficulties encountered in monitoring arterial blood pressure. A balanced anaesthetic technique, maintenance of stable haemodynamics with monitoring is required for a successful outcome. We describe the anaesthetic management

Hepatorenal syndrome (HRS) is a form of functional renal failure occurring in patients with advanced liver disease. Hypoperfusion of the kidney, due to renal vasoconstriction, is the main feature of HRS. Conversely, the extrarenal circulation is characterized by low systemic resistance, especially occurring in splanchnic vessels, and arterial hypotension. It has been postulated that renal vasoconstriction is induced either by a hepatorenal reflex related to the diseased liver or by arterial vasodilation and the subsequent baroreceptor-mediator activation of systemic vasoconstrictor factors. Th…

The frequently occurring condition of renal failure in heart failure (HF) has been termed the cardiorenal syndrome. However, the importance of renal insufficiency in HF has only been embraced in the last decade, and therefore, the pathophysiology of cardiorenal failure is still poorly understood. The main driving force of renal failure in HF is probably hemodynamic derangement, with both reduced renal perfusion and increased venous pressure as the most important driving forces. Different cardiorenal connectors may modulate this relationship. Furthermore, renal failure is not only limited to re

Acute renal failure due to angiotensin-converting enzyme inhibitors (ACEI) usually results from a marked fall in renal perfusion pressure in patients with renovascular disease. We report herein one case of acute kidney transplant artery thrombosis in a patient with renovascular hypertension. The thrombosis occurred immediately after a single dose of ACEI for evaluation of a renal transplant artery stenosis by renal scintigraphy. The dramatic fall of arterial pressure observed in our patient probably played an important role in the thrombosis. The risk of the administration of a single dose of

The current approach to renovascular hypertension at the Lahey Clinic Medical Center consists of wider application of revascularization procedures than was possible a decade ago. Surgery is more frequently advised in patients over 50 years of age, in patients with atherosclerotic disease, and in patients with azotemia to restore renal function. Revascularization is suggested in some patients with a totally occluded renal artery, and alternatives to aortorenal bypass are advocated when the aorta is diseased. Bench surgery is used when appropriate, and concomitant aortic replacement and renal re

Renovascular hypertension is non essential hypertension, wherein anatomically evident arterial occlusive disease and increased blood pressures are related as cause and effect. The hypertension is due to renal ischemia. Angiodysplasia is an uncommon angiopathy associated with heterogeneous histological changes that may affect the carotid circulation and the visceral and peripheral arteries.

A specific medical therapy for 'the hepatorenal syndrome' (HRS) is not available. However, the increasing knowledge of the mechanisms involved in the development of HRS leads to new therapeutic approaches to this syndrome of functional renal failure. Recognition and prevention of precipitating factors, the correction and treatment of prerenal failure and acute tubular necrosis are particularly important in the treatment of patients with liver cirrhosis, ascites and hepato-renal failure. Vasoconstrictor agents (ornipressin, dopamine) are administered to achieve an improvement of the systemic ci

A case of rarely occurring malformation of the embryonal period of development, a diploid variant of sirenomelia in one of the twins, is described. Roentgenology revealed a paired set of bones of the extremities. There was aplasia of the external sex organs with formed gonads, atresia of the urethra and anus, aplasia of the renal arteries, hypoplasia of the right and aplasia of the left kidney, colonic-urinary bladder fistula. Operative intervention (epicystostomy, colostomy) was applied. The baby died on the 8th day with renal insufficiency and disorders of the cerebral circulation.

Diabetic kidney disease (DKD) and osteoporosis are closely linked, yet the underlying mechanisms remain incompletely understood. DKD mouse and rat models were established via combinatorial treatment with a high-fat diet and streptozotocin, which not only induced progressive renal dysfunction, but also triggered systemic osteoporotic changes, including reduced bone mineral density, trabecular thinning and impaired bone microarchitecture. Using single-cell sequencing, we demonstrate that DKD elevates the expression of Sfrp2 (secreted frizzled related protein 2) in glomerular mesangial cells (MCs

Although renovascular hypertension is associated with substantial cardiovascular morbidity, ultimately it is a curable disease. Early identification and appropriate treatment of renovascular hypertension may save years of antihypertensive therapy, reduce the morbidity associated with long-standing hypertension, and help to minimise the risk of renal failure. However, the main problem is to identify patients with renovascular disease suitable for treatment. This requires alertness in the clinician, and renographic screening of renal function or duplex-ultrasound scanning of renovascular circula

A case is described in which ureteral vascular impressions called "notching" or "scalloping sign" are caused by an ureteral artery supplying a pheochromocytoma. Vascular impressions on the ureter are recognized in different conditions. Most of them are due to collateral circulation following renal artery obstruction. The differential diagnosis is discussed.

We performed renal function studies in dogs with chronic renovascular hypertension produced by complete occlusion of a renal artery. In addition, we evaluated in anesthetized dogs the acute effects of a novel angiotensin converting enzyme inhibitor, CGS 16,617, on renal function and plasma neurohormones (epinephrine, norepinephrine and vasopressin) 4 weeks after initiation of 2 kidney, 1 clip hypertension. CGS 16,617 effectively decreased blood pressure in renal hypertensive animals. This response was associated with suppression of angiotensin II indicating effective converting enzyme inhibiti

This review outlines the available data from the work of our group on renal hemodynamics, function, and oxygenation in patients who are critically ill with acute renal dysfunction, such as those with postoperative AKI, those in early clinical septic shock, in patients undergoing cardiac surgery with cardiopulmonary bypass, or in patients undergoing liver transplantation. We also provide information on renal hemodynamics, function, and oxygenation in patients with chronic renal impairment due to congestive heart failure. This review will argue that, for all of these groups of patients, the comm

There is strong evidence that the renal medullary circulation plays a key role in long-term blood pressure control. This, and evidence implicating sympathetic overactivity in development of hypertension, provides the need for understanding how sympathetic nerves affect medullary blood flow (MBF). The precise vascular elements that regulate MBF under physiological conditions are unknown, but likely include the outer medullary portions of descending vasa recta and afferent and efferent arterioles of juxtamedullary glomeruli, all of which receive dense sympathetic innervation. Many early studies

Intrarenal hemodynamics were estimated in patients with primary aldosteronism (PA) using the Gomez's equations and by analyzing their renal function curve. The study was performed in 6 patients with PA for 2 weeks; they were given a regular sodium diet (12-15 g/day) in the 1st week and a sodium restricted diet (1-3 g/day) in the 2nd week. Blood pressure and urinary sodium excretion (UNaV) were measured on the last three days of each stage. Glomerular filtration rate (GFR) and renal plasma flow rate (RPF) were also measured on the regular sodium diet. Afferent (RA) and efferent (RE) arteriole r

Hypocalcemia and hyperphosphatemia are the pathognomonic biochemical features of hypoparathyroidism, and result directly from lack of parathyroid hormone (PTH) action on the kidney. In the absence of PTH action, the renal mechanisms transporting calcium and phosphate reabsorption deregulate, resulting in hypocalcemia and hyperphosphatemia. Circulating calcium negatively regulates PTH secretion. Hypocalcemia causes neuromuscular disturbances ranging from epilepsy and tetany to mild paresthesia. Circulating phosphate concentration does not directly regulate PTH secretion. Hyperphosphatemia is su

Acute renal failure is a serious complication of the resuscitation and post-traumatic critical care of the severely injured patient. Renal ischemia secondary to shock is the most common cause of post-traumatic renal failure. The unrecognized or untreated renal ischemic state can lead to nephrotoxic damage in the post-traumatic patient. Prevention of acute failure by rapid resuscitation and restoration of circulating volume to prevent ongoing renal ischemia and prerenal azotemia is imperative for reducing the incidence of post-traumatic renal failure. Once post-traumatic renal failure is establ
