Transoral Endoscopic-Assisted Partial Arytenoidectomy as a Treatment for Laryngeal Paralysis in Eight Cats.

Laryngeal paralysis is an uncommon but potentially life-threatening condition in cats due to upper airway obstruction. Surgical arytenoid lateralization is generally considered the treatment of choice; however, complication rates in cats appear higher than in dogs, and alternative techniques may be desirable, particularly in emergency situations. The aim of this study was to evaluate the clinical outcome of transoral partial arytenoidectomy for the treatment of laryngeal paralysis in cats. Eight client-owned cats diagnosed with laryngeal paralysis underwent transoral partial arytenoidectomy. D

Thyrotoxic Periodic Paralysis in a Samoan Male With Metabolic Acidosis: A Case Report and Review of the Literature.

Thyrotoxic periodic paralysis (TPP) is a rare disorder characterized by muscle paralysis, thyrotoxicosis, and hypokalemia. It commonly manifests as paralysis of both proximal and distal upper and lower limbs, and if left untreated, may progress to respiratory failure or cardiac arrhythmias. It is most common in Asian males and is frequently precipitated by strenuous exercise, high carbohydrate diet, stress, corticosteroid therapy, or alcohol. Early diagnosis of TPP is crucial as the condition may be reversible with oral or IV potassium replacement therapy, and management of the underlying hype

Hemidiaphragmatic Paralysis Due to Cervical Spondylosis: A Case Report.

INTRODUCTION: C4 radiculopathy due to cervical spondylosis has rarely been reported as a cause of hemidiaphragmatic paralysis. CASE REPORT: A 70-year-old man presented with hemidiaphragmatic paralysis due to right C3-C4 foraminal stenosis. The diagnosis was made preoperatively from findings on plain chest radiographs, respiratory function tests, and electrophysiologic tests. All the patient's test results and symptoms improved immediately after surgical treatment for cervical spondylosis. CONCLUSIONS: Although it may be difficult to make a correct diagnosis based only on radiological finding

Mounier-Kuhn syndrome and bilateral vocal cord paralysis.

Mounier-Kuhn syndrome is a rare disorder of unknown cause that is characterized by atrophy of the elastic and smooth muscle of the tracheobronchial tree leading to tracheobronchomegaly and bronchiectasis. The syndrome is likely underdiagnosed, because the patients usually present with common respiratory symptoms such as productive cough and usually labeled as chronic obstructive pulmonary disease. Diagnosis is established on the basis of radiologic findings. Association with bilateral vocal cord paralysis has not been described. Treatment is mainly supportive. Symptomatic patients may require

Fulminant Guillain-Barré Syndrome Post Hemorrhagic Stroke: Two Case Reports.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated inflammatory peripheral polyneuropathy characterized by ascending paralysis. Most GBS cases follow gastrointestinal or chest infections. Some patients have been reported either following or concomitant with head trauma, neurosurgical procedures, and rarely hemorrhagic stroke. The exact pathogenesis is not entirely understood. However, blood-brain barrier damage may play an essential role in triggering the autoimmune activation that leads to post-stroke GBS. Here, we present two cases of fulminant GBS following hemorrhagic stroke to rem

Paralytic rabies following cat scratch and intra-dermal anti-rabies vaccination.

Only few reports of failure of intradermal postexposure prophylaxis for rabies following cat scratch exist in the published literature. We are reporting such a case in a 15-year-old girl. The child had category III cat scratch on her face. She presented with progressive paralysis, finally developing quadriplegia and respiratory paralysis. Typical hydrophobia and aerophobia were absent. She received intra-dermal anti-rabies cell culture vaccine. She did not receive anti-rabies immunoglobulin. The girl succumbed on the 10(th) day of weakness. Diagnosis of rabies was confirmed by isolation of rab…

Combined treatment with antitoxin and 3,4-diaminopyridine improves survival outcomes after lethal botulinum neurotoxin challenge.

Botulinum neurotoxins (BoNTs) are the most potent toxins known, causing life-threatening flaccid paralysis by blocking acetylcholine release … human life. Thus, there is an urgent need for rapidly acting symptomatic therapies that can reverse respiratory paralysis and sustain survival until resolution of toxin effects. We previously identified the FDA-approved, voltage-gated potassium channel blocker 3,4-diaminopyridine (3,4-DAP) as a fast-acting symptomatic treatment in preclinical botulism models. Here, we …

Acute Neuromuscular Disorders in the Pediatric Intensive Care Unit.

BACKGROUND: The neuromuscular disorders encountered in the pediatric intensive care unit (PICU) encompass a broad spectrum of pathologies. These include acute disorders (eg, Guillain-Barre syndrome), acute-on-chronic disorders (eg, myasthenia gravis), progressive disorders (eg, muscular dystrophy), and disorders that develop in the PICU (eg, critical illness myopathy/polyneuropathy). Familiarity with the presenting features of these disorders is of paramount importance in facilitating timely diagnosis. METHODS: We conducted a retrospective review of the medical records of patients admitted to

EIF2AK2-related hypomyelinating leukoencephalopathy presenting with congenital bilateral vocal cord paralysis and a spinal cord lesion.

Heterozygous EIF2AK2 variants reportedly cause leukoencephalopathy, developmental delay, and episodic neurologic regression syndrome. We present, to our knowledge, the first reported patient with EIF2AK2-related hypomyelinating leukoencephalopathy involving congenital bilateral vocal cord paralysis. She had respiratory distress soon after birth and underwent tracheostomy. Neonatal brain magnetic resonance imaging showed normal myelination, but severe delayed myelination, accompanied by the rare finding of dorsal medulla and cervical spinal cord signal changes, was apparent at 8 months after tr

Diagnosis of Severe Acute Respiratory Syndrome Coronavirus 2 Infection in a Patient Presenting With Guillain-Barré Syndrome.

Guillain-Barré syndrome (GBS) is a rare neurologic disorder in which the host immune system damages peripheral nerve cells. Although classically described as an ascending paralysis, GBS can present in a myriad of ways. As peripheral nerve conduction becomes impaired, symptoms can range from mild weakness to a life-threatening paralysis of the respiratory muscles. Patients frequently experience prolonged recovery times, with residual effects often lasting up to a year or longer. The exact cause of the disorder is not fully understood, but its development most often follows infection with numero

[Ultrasonographic assessment of hemidiaphragm paralysis secondary to interscalene block].

BACKGROUND: In shoulder surgery, interscalene brachial plexus block has an incidence of 100% hemidiaphragm palsy due to phrenic nerve block. Controling the hemidiaphragm becomes a security ventilation parameter. OBJECTIVE: identify and evaluate with ultrasound hemidiaphragm paralysis after interscalene block. METHODS: This prospective study included 50 patients scheduled for shoulder surgery with interscalene block using neurostimulation. Diaphragmatic movement was evaluated by ultrasound prior to placement of block and the end of the surgical procedure to make the comparison between the two

Clinical and Etiological Spectrum of Hypokalemic Periodic Paralysis in a Tertiary Care Hospital in Pakistan.

Introduction Hypokalemic periodic paralysis (HPP) is characterized by muscle weakness secondary to low serum potassium levels. It may be primary in origin or there may be secondary causes like thyrotoxic periodic paralysis, renal or suprarenal causes, or non-renal causes like gastroenteritis. Aim To study the etiology, clinical manifestations, and outcome after therapy of patients with hypokalemic paralysis. Methodology The study was conducted from January 2016 to December 2016. Patients fulfilling the diagnostic criteria for hypokalemic paralysis, i.e., flaccid muscle weakness involving two o

Botulin toxin: a weapon in terrorism.

Clostridium botulinum, the causative agent of botulism is an anaerobic, spore forming gram-positive bacillus. C. botulinum causes three types of botulism; foodborne botulism, wound botulism, and infant botulism. Most strains of the bacterium produce a potent, muscle-paralyzing neurotoxin. Respiratory failure secondary to paralysis of the respiratory muscles can lead to death unless appropriate therapy is promptly initiated. Due to the severity and potency of this neurotoxin, its importance as a biological weapon is of major concern to public health officials.

Dyspnea induced by hemidiaphragmatic paralysis after ultrasound-guided supraclavicular brachial plexus block in a morbidly obese patient.

RATIONALE: Hemidiaphragmatic paralysis (HDP) is a frequent complication of the brachial plexus block, caused by unintentional blockade of ipsilateral phrenic nerve. HDP did not rise enough alarm and attention to most anesthesiologists, because most patients with no coexisting comorbid diseases are asymptomatic and able to tolerate it. However, it may cause severe respiratory complication for patients with preexisting compromised cardiorespiratory function. PATIENT CONCERNS: A 67-year-old woman with morbidly obesity was planned to receive opening reduction and internal fixation of right humera

Infantile-onset spinal muscular atrophy with respiratory distress-1 diagnosed in a 20-year-old man.

Spinal muscular atrophy with respiratory distress (SMARD1) presents within the first 13months of age with low birth weight, progressive length dependent motor neuropathy, and respiratory failure from diaphragmatic paralysis. SMARD1 is caused by mutations in IGHMBP2, encoding the immunoglobulin μ-binding protein 2. Because of the severity of the disorder, many infantile-onset SMARD1 patients do not live past the first decade of life. This report documents the clinical course of a 20-year-old man diagnosed with SMARD1.

Respiratory distress after a Nadbath block.

Dysphagia and respiratory distress developed in three patients after administration of a Nadbath block. This complication may have resulted from inadvertent paralysis of the vagus, ipsilateral glossopharyngeal, and spinal accessory nerves causing pooling of oropharyngeal secretions and laryngospasm. Respiratory distress after a Nadbath block may be managed by properly positioning the patient. Short hypodermic needles (less than 12 mm), small volumes of anesthetic solution (less than 3 ml), and omission of hyaluronidase may reduce the incidence of this complication.

Central respiratory depression induced by acetylcholinesterase inhibition: involvement of anaesthesia.

We have studied the effects of anaesthesia on the changes in central respiratory activity following the inhibition of acetylcholinesterase in chronically implanted cats. The organophosphate paraoxon was administered to the brainstem respiratory centres by intracerebroventricular (i.c.v.) injection (3 mg) into the IVth ventricle, thus avoiding peripheral effects such as paralysis of respiratory muscles. Paraoxon had opposite effects on respiratory activity depending on whether the cats were anaesthetized or not: it induced respiratory depression and sometimes respiratory arrest during pentobarb

Respiratory distress and Arnold-Chiari malformation.

Fourteen children with Arnold-Chiari malformation had history of respiratory distress, apnea, vocal cord paralysis or inability to swallow. Postmortem examination in 12 disclosed vascular lesions in the tegmentum of the medulla oblongata. The length of survival of these children was markedly shorter than of those without such history and anatomic findings. It is suggested that the malformation results in changes in the vascular supply of the herniated portion of the brainstem. Stretching of the arteries may result in irreversible damage to the brainstem with subsequent life-threatening disturb
