Sepsis in the baboon: factors affecting resuscitation and pulmonary edema in animals resuscitated with Ringer's lactate versus Plasmanate.

Septic shock and the formation of pulmonary edema were studied in 19 baboons. Four animals served as controls. Four were subjected to deep septic shock by infusion of live E. coli and then deliberately killed while in deep shock. Four were subjected to septic shock, resuscitated with Ringer's lactate (RL), and then killed 11/2 hours after resuscitation was started. Seven were subjected to shock and resuscitation attempted with Plasmanate (PL). Resuscitation with RL was successful for 11/2 hours in all four RL-animals. Resuscitation with PL was successful for 11/2 hours in three of the 7 PL-ani…

The resuscitation team members' lived experiences regarding futile cardiopulmonary resuscitation in children and newborn infants: A phenomenological study.

Futility is difficult to define in the context of Cardiopulmonary Resuscitation , as it refers to the low likelihood of success. Healthcare providers face challenges when performing pediatric/neonatal Resuscitation. This study aimed to explore the lived experiences of resuscitation team members regarding futile Cardiopulmonary Resuscitation in children/newborns. A qualitative descriptive phenomenological approach was used with purposive sampling, resulting in 11 members of the Cardiopulmonary Resuscitation team for children/newborns (7 nurses/4 physicians). Data were collected through in-depth…

Artesunate preserves post-resuscitation myocardial and neurologic function in a rat model of cardiac arrest and cardiopulmonary resuscitation.

BACKGROUND: To investigate the effects of Artesunate (Art) on post-resuscitation myocardial and neurologic function, survival duration, and the underlying mechanisms in a rat model of cardiac arrest (CA) and cardiopulmonary resuscitation (CPR). METHODS: Thirty healthy male Sprague-Dawley rats were randomly allocated into three groups: Sham, Control(CA/CPR + vehicle),and ART(CA/CPR + Art). The latter two groups were further divided into survival and non-survival subgroups. CA were induced via 6-minute ventricular fibrillation, followed by 8 min of CPR. After the return of spontaneous circulati…

Pyridostigmine improves post-resuscitation myocardial dysfunction in a rat model of ventricular fibrillation-induced cardiac arrest and resuscitation.

BACKGROUND: Post-resuscitation myocardial dysfunction is one of the major causes of death in post-cardiac arrest patients. Pyridostigmine (PYR) shows protective effects on myocardial ischemia-reperfusion injury following myocardial infarction and left heart failure by improving autonomic nervous function. This study aimed to investigate the effects of low-dose pyridostigmine on post-resuscitation myocardial dysfunction. METHODS AND RESULTS: A rat model of cardiac arrest and resuscitation was induced by ventricular fibrillation (VF). The rats were randomly categorised into the following four g…

Pediatric Extracorporeal Cardiopulmonary Resuscitation: Development of a Porcine Model and the Influence of Cardiopulmonary Resuscitation Duration on Brain Injury.

Background The primary objective was to develop a porcine model of prolonged (30 or 60 minutes) pediatric cardiopulmonary resuscitation (CPR) followed by 22- to 24-hour survival with extracorporeal life support, and secondarily to evaluate differences in neurologic injury. Methods and Results Ten-kilogram, 4-week-old female piglets were used. First, model development established the technique (n=8). Then, a pilot study was conducted (n=15). After 80% survival was achieved in the final 5 pilot animals, a proof-of-concept randomized study was completed (n=11). Shams (n=6) underwent anesthesia on…

Beyond basic resuscitation: What are the next steps to improve the outcomes of resuscitation at birth when resources are limited?

Implementation of basic neonatal resuscitation in low- and middle-income settings consistently saves lives on the day of birth. What can be done to extend these gains and further improve the outcomes of infants who require resuscitation at birth when resources are limited? This review considers how resuscitation and post-resuscitation care can advance to help meet the survival goals of the Every Newborn Action Plan for 2030. A brief summary of the evidence for benefit from basic neonatal resuscitation training in low- and middle-income countries highlights key aspects of training, low-dose hig…

Is emergency department resuscitative thoracotomy futile care for the critically injured patient requiring prehospital cardiopulmonary resuscitation?

BACKGROUND: Documented prehospital asystole justifies termination of resuscitation, but recently it has been proposed to extend this policy to patients in the field with pulseless electrical activity. Consequently, we questioned whether resuscitative thoracotomy is warranted in the critically injured patient who fails to respond to prehospital CPR. STUDY DESIGN: A prospective database of all emergency department resuscitative thoracotomies (EDT) performed at our Level I trauma center has been maintained since January 1977. These registry data were augmented by a review of prehospital paramedi…

Emergency Burn Resuscitation: Where Evidence Meets Practice.

The initial management of a critically ill burn patient sets the trajectory for their entire hospital course. Understanding standards of care is essential, but moving beyond rote formulas for airway management, fluids, and analgesia is critical to ensuring appropriate care and preventing iatrogenic harm. This article provides an evidence-based guide for emergency physicians on when and how to pivot from standard care to nuanced, life-saving interventions that address the dynamic pathophysiology of severe burns.

Benefits of cardiopulmonary resuscitation in cancer patients.

PURPOSE: According to meta-analytic data, the prognosis of a cancer patient post-cardiopulmonary resuscitation (CPR) is relatively similar to the general population. However, preselection of patients, the details of CPR, patient-specific characteristics, and post-CPR care are poorly described. The aim of this study is to identify prognostic factors in order to recognize cancer patient profiles more likely to benefit from CPR. METHODS: This is a retrospective study on a series of patients with solid or hematological malignancies who received CPR between January 2010 and December 2020 in a canc…

[Damage Control Resuscitation and Damage Control Surgery].

Trauma triggers complex physiological responses with primary and secondary effects vital to understanding and managing trauma impact. "Damage Control" (DC), a concept adapted from naval practices, refers to abbreviated initial surgical care focused on controlling bleeding and contamination, critical for the survival of severely compromised patients. This impacts anaesthesia procedures and intensive care. "Damage Control Resuscitation" (DCR) is an interdisciplinary approach aimed at reducing mortality in severely injured patients, despite potentially increasing morbidity and ICU duration. Curre…

Pneumoperitoneum following cardiopulmonary resuscitation: An unusual case.

INTRODUCTION AND IMPORTANCE: High-quality cardiopulmonary resuscitation (CPR) is the foundation of cardiac arrest resuscitation. Pneumoperitoneum due to gastric perforation is a rare surgical complication of CPR that, if left untreated, can result in significant morbidity and mortality. CASE PRESENTATION: We present a 51-year-old male patient with sealed perforation who received an urgent but non-diagnostic exploratory laparotomy after initial esophageal intubation and resuscitation in cardiac arrest, despite significant evidence of surgical pneumoperitoneum. CLINICAL DISCUSSION: It is unusu…

Chest compressions and medications during neonatal resuscitation.

Prolonged resuscitation in neonates, although quite rare, may occur in response to profound intractable bradycardia as a result of asphyxia. In these instances, chest compressions and medications may be necessary to facilitate return of spontaneous circulation. While performing chest compressions, the two thumb method is preferred over the two finger technique, although several newer approaches are under investigation. While the ideal compression to ventilation ratio is still uncertain, a 3:1 ratio remains the recommendation by the Neonatal Resuscitation Program. Use of feedback mechanisms to …

Improving Ventilation Rates During Pediatric Cardiopulmonary Resuscitation.

BACKGROUND: Excessive ventilation at rates of 30 breaths per minute (bpm) or more during cardiopulmonary resuscitation (CPR) decreases venous return and coronary perfusion pressure, leading to lower survival rates in animal models. A review of our institution's pediatric CPR data revealed that patients frequently received excessive ventilation. METHODS: We designed a multifaceted quality improvement program to decrease the incidence of clinically significant hyperventilation (≥30 bpm) during pediatric CPR. The program consisted of provider education, CPR ventilation tools (ventilation reminde…
