Sterilization of infected root-canal dentine by topical application of a mixture of ciprofloxacin, metronidazole and minocycline in situ.

The aim of this study was to observe the potential of a mixture of ciprofloxacin, metronidazole and minocycline to kill bacteria in the deep layers of root canal dentine in situ. After the crowns of extracted teeth had been removed, the drug combination (0.5 mg of each drug), or sterile saline, as the control, was placed in the root canals which had been previously irrigated ultrasonically with G4M EDTA. The penetration and bactericidal efficacy were estimated by various procedures as follows. (1) A cell suspension of E. coli was placed into small cavities prepared parallel to the root canals

Rotary versus reciprocating shaping files in bacterial elimination from long oval canals.

A comparison of the abilities of rotary versus reciprocating files to eliminate viable Enterococcus faecalis populations from the long oval root canals of extracted human teeth. Fifty teeth were contaminated and randomly distributed into two groups (n = 25 each): BT-RaCe group and WaveOne group. Two microbial samples were obtained from each tooth before (S1) and after (S2) instrumentation. The CFUs from the S1 and S2 measurements were calculated and compared between the groups. Both groups showed significantly fewer CFUs in the S2 samples (P < 0.001). In the S2 intragroup comparison, BT-RaCe r

Effect of vapor lock on root canal debridement by using a side-vented needle for positive-pressure irrigant delivery.

INTRODUCTION: This study examined the effect of vapor lock on canal debridement efficacy by testing the null hypothesis that there is no difference between a "closed" and an "open" system design in smear layer and debris removal by using a side-vented needle for irrigant delivery. METHODS: Roots in the closed system were sealed with hot glue and embedded in polyvinylsiloxane to restrict fluid flow through the apical foramen during cleaning and shaping. For the open system, the apical foramen was enlarged and connected to the external environment via a channel within the polyvinylsiloxane to p

Mandibular second molar with C-shaped canal morphology and five canals: report of a case.

This article reviews a case involving endodontic treatment of a mandibular second molar with C-shaped canal morphology and five separate canals. The C-shaped canal variation of morphology is unusual and can lead to difficulties during treatment. Preoperative radiographs are limited in their ability to detect this entity; for a favorable treatment outcome, knowledge of the C-shaped canal's existence and morphology is essential.

Endotoxic activities of lipopolysaccharides of microorganisms isolated from an infected root canal in Macaca cynomolgus.

Lipopolysaccharides (LPS) prepared from a strain of Bacteroides oralis, a strain of Fusobacterium necrophorum, and a strain of F. nucleatum, all isolated from an infected root canal in monkey (Macaca cynomolgus), were examined for endotoxic activities using primary skin reactions in rabbits and induction of leukocyte chemotaxis in rats. LPS of B. oralis showed considerably lower ability to cause skin inflammation than LPS of the fusobacteria. However, the leukotactic effect of the LPS preparations as determined by the wound chamber method in rats was approximately of the same proportion. In bo

Comparative efficacy of ultrasonic, diode laser, and manual irrigant activation around an apically separated instrument: an in vitro study.

BACKGROUND: The fracture of a nickel-titanium instrument in the apical third of a root canal presents a significant clinical challenge, as it creates a compacted barrier of dentinal debris and smear layer that is shielded by a vapor lock, severely limiting irrigant efficacy. This study aimed to compare the efficacy of Ultrasonic Activation (UAI), Diode Laser-Activated Irrigation (LAI), and Manual Dynamic Agitation (MDA) in overcoming this barrier by evaluating both irrigant penetration and direct debris removal. METHODS: Ninety human mandibular first molars with standardized, non-bypassable a

Irrigants for non-surgical root canal treatment in mature permanent teeth.

BACKGROUND: Root canal treatment is carried out on teeth in which irreversible pulpitis has led to necrosis of the dental pulp. As a treatment option it is an alternative to dental extraction. Mechanical preparation and irrigation with antiseptic or antibacterial solutions destroys bacteria and cleans the infected root canal. Irrigants should be effective in deactivating bacteria in the entire root canal space without causing any adverse tissue reactions. Sodium hypochlorite (NaOCl) and chlorhexidine are commonly used but there is uncertainty as to which solution, concentration or combination

Mandibular first premolar with four canals.

A case of endodontic treatment of a mandibular first premolar exhibiting a total of four distinct root canals and four apical foramina is described. This occurrence in mandibular first premolar has rarely been reported in the endodontic literature. Endodontic treatment that considers the anatomic variation of root canal morphology is important to ensure a favorable healing outcome, and its identification could be enhanced by careful examination using a dental operating microscope. Obturation of root canals using a warm vertical compaction technique with a highly-radiopaque root canal sealer, s
