AIM: report a new case of primary necrosis of the ligamentum teres hepatis and discuss preoperative diagnosis strategy and therapeutic options.
OBSERVATION: This is a patient of 76 years, hypertensive, who was admitted for an acute abdomen. The diagnosis of acute pancreatitis was referred to the poor clinical examination and amylase to six times normal. Abdominal CT scan showed a normal pancreas appearance and hypodense infiltration extended along the round ligament of the liver to the anterior abdominal wall that did not take the contrast. Three cases of endometriosis infiltrating the round ligament are presented. The initial diagnosis was irreducible hernia, since this rare nosologic entity often causes unusual preoperative symptoms and diagnostic problems. A case of hepatic torsion is presented with the following clinical features: 1) complete dislocation of the liver into the left upper quadrant, 2) 180 degrees clockwise torsion of the liver with severe ischemia, 3) hypotension secondary to vena caval obstruction, 4) ascites, 5) congenital absence of falciform and triangular ligaments, 6) obstruction of the descending colon by the round ligament, 7) intestinal malrotation and massive colonic dilation. The patient was treated by de-torsion of the liver, hepatopexy (by suturing the round ligament to the right abdominal wall), and colon resection.

BACKGROUND: Accessory liver lobe (ALL) is a rare liver malformation. An ALL develops due to malformation of the endodermal caudal foregut and segmentation of the hepatic bud in the third week of gestation. Most ALLs are asymptomatic and are detected incidentally during abdominal surgery. The incidence of ALL is < 1% in patients who undergo abdominal surgery. However, some ALLs twist and cause acute abdomen. We experienced a pediatric case of ALL torsion in a patient who underwent elective laparoscopic surgery.

Cysts of a permeable vaginal process or cysts of Nuck are a rare entity in women and have a similar embryologic and etiologic origin as that of inguinal hernias. They should be considered a differential diagnosis when an inguinal mass is present or during a diagnostic laparoscopy in which a peritoneal defect or open inguinal ring is found. We report the case of a woman with a cyst of Nuck that presented as a recurrent inguinal hernia. Discussion is centered on the embryologic, anatomic, clinical and therapeutic aspects of this condition.
