A comparison of rural and urban anticoagulation management of atrial fibrillation in a southwest Missouri health system.

The purpose of this study is to determine if an opportunity exists to improve anticoagulation therapy for non-hospitalized, chronic atrial fibrillation patients cared for by St. John's Health System physicians. Clinical chart review consisted of 200 patients in both the urban and rural practice groups. Urban practices were found to have 95 percent of the cases receiving warfarin, 95 percent confidence interval (90-100). Rural practices were found to have 97 percent of the cases anticoagulated, 95 percent confidence interval (88-99).

Elective abortion as a primary health service in rural India: experience with manual vacuum aspiration.

India allows abortion up to 20 weeks of pregnancy but places restrictions on abortion facilities and providers. Abortion services are especially deficient in rural areas. Although vacuum aspiration is safer, sharp curettage continues to be used by providers as they lack relevant training. This paper describes the provision of first trimester abortion services using manual vacuum aspiration (MVA) in a rural clinic in the state of Rajasthan over a four-year period. Non-use of anaesthesia increased safety and allowed women to return early to a normal routine. Of 534 women, none suffered major com

Work attitudes, employment barriers, and mental health symptoms in a sample of rural welfare recipients.

Most of the extant research on welfare reform has neglected to consider the experiences of families in rural settings. Fifty women receiving welfare for their dependent children in a rural community were interviewed about their work experiences and aspirations, barriers to employment and service use, as well as mental health and social support. The findings indicate that the majority of participants were connected to the labor force and expressed positive attitudes about work. Barriers to employment (lack of available jobs, child care) and service use (transportation, inconvenient office hours

Epidemiology of health and safety risks in agriculture and related industries. Practical applications for rural physicians.

Epidemiologic studies document that work in the agricultural sector is associated with many occupational health hazards. Exposure to organic dusts and airborne microorganisms and their toxins may lead to respiratory disorders. The burden of exposure-related chronic bronchitis, asthma, hypersensitivity pneumonitis, organic-dust toxic syndrome, and chronic airflow limitation can be diminished by appropriate preventive measures. The contribution of exposures to agricultural chemicals to cancers and neurodegenerative disorders is being investigated. Some studies document that farmers and those in

The Ngaglik study: an inquiry into birth interval dynamics and maternal and child health in rural Java.

Preliminary analysis of the Ngaglik Study has been able to confirm much of what we knew about birth interval dynamics from past retrospective studies, while also adding new insights into birth spacing and maternal and child health in rural Java. In Ngaglik, a median duration of breast-feeding of over 2 years is associated with durations of lactational amenorrhoea of over a year on average, despite the very early introduction of other foods in the infant diet. Postpartum abstinence appears to play a role in birth spacing for many women, though as previous research has shown, it does not extend

The relationship of personal financial status to the utilization of community mental health centers in rural Appalachia.

As a follow-up to an aggregate study of the relationship of economic factors to the utilization of mental health facilities, this study, using individual data, attempted to determine the associative strengths of income factors--welfare and unemployment--for utilization of community mental health centers (CMHCs) in rural Appalachia. On the basis of the earlier study and of previous studies of social class and utilization of CMHCs it was hypothesized that for this sample of 151 clients of eight CMHCs in Ohio and West Virginia, unemployment and welfare factors would be most strongly associated wi

Agents affecting health of mother and child in a rural area of Kenya. III. The epidemiology of measles.

Between April 1974 and March 1976 surveillance of measles has been conducted by 12 fieldworkers making fortnightly home visits among a total population of 24,000 living in nearly 4000 households scattered in variable density throughout an area of 87 sq km in the northern division of the Machakos district, Kenya. The diagnosis of measles was verified by one of the project's physicians according to standardized procedures and was, whenever possible, supported by virus isolation and serum antibody level determination. Because not all reported patients were actually seen during the period of clini

Rural Cardiovascular Health in Missouri: Needs, Care Gaps, and Resources for a More Connected System.

Missouri's rural health needs are inseparable from cardiovascular health. Nearly one-quarter of Missourians live in nonmetro areas, where rural hospitals, clinics, community health centers, emergency medical services agencies, public health departments, and community organizations anchor care. Yet heart disease is the leading cause of death in Missouri; the 20 counties with the highest heart disease death rates are rural; and recent data show higher rural heart disease and stroke mortality than urban Missouri. Cardiovascular risk is shaped by clinical care, primary care access, transportation,…

Beyond the Textbook: An HBM and SCT Analysis of Adolescent Reproductive Health Education in Rural Bangladesh.

The effective delivery of adolescent and reproductive health (ARH) education in rural Bangladeshi schools is influenced by factors extending beyond curriculum content, including teacher preparedness, classroom interactions, gender norms, and the broader social environment. This qualitative study examined how teachers and students experienced the delivery and discussion of ARH topics in rural secondary schools, drawing on the Health Belief Model (HBM) and Social Cognitive Theory (SCT) as interpretive frameworks. Data were collected through 18 in-depth interviews with teachers and nine focus gro

Structural and Intermediate Social Determinants of Health Linked to Adolescent Pregnancy in the Peruvian Rural Context.

BACKGROUND: Adolescent pregnancy is a public health problem in Latin America, with a higher prevalence in rural areas. This population faces barriers to accessing education and healthcare services, increasing the risk of maternal and neonatal complications. METHODS: This study employed a cross-sectional analytical design and included 72 adolescents aged 12 to 17 years in Lonya Grande. The associations between structural and intermediate social determinants of health and the prevalence of adolescent pregnancy were assessed. RESULTS: Among the structural determinants, extreme poverty (PR: 2.48

Creating a Feasible, Safe and Sustainable Penicillin Oral Challenge Program in a Rural Health Service.

PROBLEM: Programs that identify people with a low-risk allergy who can be safely delabeled by a direct oral challenge have emerged but are mostly reported in tertiary centres. There is limited information in the rural setting where resources are more constrained. SETTING: A medium-sized rural health service in Victoria (Australia). KEY MEASURES FOR IMPROVEMENT: The outcome measure was the number of patients with a low-risk penicillin allergy who completed a penicillin oral challenge. STRATEGIES FOR CHANGE: The penicillin oral challenge program developed considered four critical workflows: c…

Implementation of Screening, Brief Intervention, and Referral to Treatment in a Rural Outpatient Behavioral Health Clinic.

Substance use disorders are associated with significant disability and mortality in the United States. Treatment for substance use disorder is effective; however, screening is not a common practice in medical or psychiatric outpatient clinics. At a rural ambulatory behavioral health clinic, patients were not being screened for alcohol and drug misuse despite recommendations by the U.S. Preventive Services Task Force and the National Institute of Drug Abuse. This quality improvement project aimed to provide screening, brief intervention, and referral to treatment training for providers for new

Digital Health Innovation by Design: A Logic Model Scaffold for Rural, Regional, and Remote Settings.

Digital health innovations (DHIs) have the potential to transform access, continuity, and quality of healthcare in rural, regional, and remote (RRR) settings, yet they often fall short in practice. Barriers extend beyond infrastructure and technology to include workforce challenges and the complex realities of delivering care across diverse geographic, cultural, and social contexts. Effective DHIs must therefore be designed with local needs and systemic constraints in mind. Conventional logic models can align inputs and activities with intended outcomes, but their linear and static assumptions

Mental health and stigma in persons affected by Hansen's disease and their families in rural Sitapur, India.

Psychological problems are common among persons affected by Hansen's disease, a chronic but treatable disease that remains endemic in India despite available and effective treatment. Stigmatisation can also lead to delayed care-seeking, which can impede eradication efforts and negatively impact the physical and psycho-social wellbeing of the affected person and their family. This study aimed to examine how stigma in society affect the mental health of persons with Hansen's disease and their families in endemic villages of Sitapur, India. Semi-structured interviews were conducted to explore how
