[Management of congenital scalp defects in infants].

OBJECTIVE: To investigate the management of congenital scalp defects in infants. METHODS: From 1996 to 2008, 6 infants with congenital scalp defects were treated with dressing change, flap transposition, or tissue expansion combined with skull defect reconstruction. RESULTS: Parietal scalp defects in 6 cases were healed after treatment. 5 cases were followed up for 3 months to one year. 2 cases had scalp alopecia in some areas. The scar was inconspicuous in the other 3 cases. CONCLUSIONS: Dressing change is suitable for small scalp defect, while flap transposition should be used for medium …

Arteriovenous malformation of the scalp with cerebral steal.

BACKGROUND: Arteriovenous malformation of the scalp resulting from blunt trauma to the head is an unusual occurrence and is rarely associated with neurological sequelae. This case details an arteriovenous malformation with a cerebral steal phenomenon. METHODS: A 21-year-old man presented with an enlarging, pulsatile mass in the right facial/temporal area without known cause. CT and angiography were consistent with a large arteriovenous malformation involving the external carotid system, causing a secondary cerebral steal that resulted in neurological symptoms including seizure. Surgical resec…

Options for the management of forehead and scalp defects.

Forehead and scalp reconstruction comprises a diverse and complex set of defects. Repair must be performed with minimal disturbance to surrounding structures, such as the eyelid, eyebrow, and hairline. Care must be taken to maintain symmetry between sides. This article addresses the options for the management of forehead and scalp defects, including secondary intention healing, skin grafting, local flaps, free flaps, tissue expansion, and negative pressure treatment. When possible, special consideration is given to addressing the advantages and disadvantages of each repair option, while provid…

A case of steatocystoma multiplex limited to the scalp.

We report here on a 63-year-old woman who had several small, yellowish papules on the scalp for the previous 2 years. There was no family history of similar lesions. Yellowish, creamy material was expressed from a papule during punch biopsy. Histologic examination from the lesion revealed the typical features of steatocystoma multiplex. We report here on this rare variant of steatocystoma multiplex that was limited to the scalp.

Angiosarcoma of the scalp mimicking a sebaceous cyst.

A 58-year-old Caucasian woman presented with a cystic lump behind the right ear that was clinically diagnosed as an infected sebaceous cyst. The lesion was treated with incision and drainage followed by antibiotics for 3 months. Because there was no resolution, a biopsy was performed that revealed a high grade angiosarcoma. She expired 2 months later. Cutaneous angiosarcoma is an extremely aggressive tumor. Therefore early diagnosis and management is crucial in providing better patient care.

Scalp metastases of a renal cell carcinoma.

An 80-year-old man presented with a localized tumor of the right occipital scalp. The tumor was a 1-cm, bright red-purple, ulcerated, and crusted exophytic nodule on a smooth base (Figure 1). The lesion had grown asymptomatically over 18 months except for profuse bleeding induced by minimal trauma. It was extirpated with the clinical diagnosis of pyogenic granuloma vs. renal metastasis to the scalp. The patient's medical history included a transurethral prostatic resection 3 years earlier and, 1 year later, a right nephrectomy for a 2-kg kidney tumor verbally reported as "benign." The patient …

Seborrheic dermatitis of the scalp: etiology and treatment.

Seborrheic dermatitis is a common chronic infection of the lipid-rich areas of skin. While seborrheic dermatitis has been a recognized clinical entity for decades, its etiology is far from clear. Early investigators of the development of seborrheic dermatitis focused on the role of the Malassezia (previously Pityrosporum) yeasts. These yeasts are also normal skin commensals, thus their importance as pathogens in this disorder came to be doubted. However, it was subsequently found that treatment of seborrheic dermatitis with an antifungal agent not only resulted in clinical improvement but also

Ciclopirox gel for seborrheic dermatitis of the scalp.

BACKGROUND: Seborrheic dermatitis is a common inflammatory skin disorder that usually occurs in patients with pre-existing seborrhea. The etiology of seborrheic dermatitis is uncertain. Typically, sites dense with sebaceous glands support growth of the lipophilic yeast Malassezia furfur. Ciclopirox (Loprox) gel is a hydroxypyridone, broad-spectrum antifungal agent proven effective against the yeast M. furfur. OBJECTIVE: A multicenter, randomized, double-blind, vehicle controlled study of 178 subjects evaluated the efficacy of ciclopirox gel in treating seborrheic dermatitis of the scalp. MET…

Potential of tomotherapy for total scalp treatment.

PURPOSE: Total scalp radiotherapy is required in a variety of clinical situations. We compared conventional lateral photon-electron (LPE) technique with tomotherapy (intensity-modulated radiotherapy [IMRT]). METHODS AND MATERIALS: A patient with Merkel cell carcinoma was treated at our institution using conventional treatment techniques. Treatment plans were conducted using conventional three-dimensional treatment planning and IMRT. The clinical target volume included the entire scalp tissue volumes to the surface of underlying cranial bone, as well as superficial and deep neck nodes in the b…

Subclinical speckled perifollicular melanosis of the scalp.

Based on the clinical presentation of some skin pigmentation disorders it is thought that a bicompartmental functional system exists in the epidermal melanocyte population. It corresponds to the perifollicular and interfollicular compartments, respectively. The present study was undertaken looking for the presence of such a system on scalp unaffected by pigmentary disorders. The scalps of 100 men with incipient to severe androgenic alopecia were examined using a videocamera equipped with an internal ultraviolet light-emitting unit. The face, trunk and limbs were similarly examined in 45 of the…

Dysplastic nevi of the scalp and forehead in children.

To determine if there is a significant difference in the relative frequency and degree of atypia of sporadic dysplastic nevi from the scalp, face, and neck area in children as compared with nevi from the rest of the body, we reviewed 99 consecutive biopsy specimens of melanocytic nevi from the scalp, face, and neck areas in children less than 18 years of age and compared them with 95 consecutive cases of nevi from other areas of the body in children of the same age. Large numbers of the nevi biopsied from the scalp (13 of 31; 41.93%) and forehead (2 of 10; 20%) were dysplastic. The number of d…

The "teepee" sign in inflammatory scalp disease.

Psoriasis and other diffuse inflammatory diseases of the scalp can cause a unique physical findings that we call the "teepee sign" because the hair forms pyramidal tufts at regular intervals that resemble an American Indian encampment. A pathological mechanism for this finding is proposed, with the description of two cases in Air Force basic trainees, one of a severe seborrheic dermatitis and the other psoriasis. The significance with regard to physical qualification for entry into military service is discussed.
