[Rupture of the tendon of the m. flexor pollicis longus--a rare complication after surgery on the scaphoid bone].

Tendon ruptures in the hand without previous direct trauma are rare. Two cases are reported on with attrition of the flexor pollicis longus tendon. The first tendon rupture was along a dislocated cortico-cancellous bone graft following a Matti-Russe procedure; the second was by attrition over the screw of an AO-plate following internal fixation in scaphoid nonunion. Immediate tendon repair by interposition of a short tendon graft or by transposition of the superficialis flexor tendon of the ring finger is the treatment of choice. Further tendon ruptures can arise if surgical intervention is de

Arthroscopic Treatment of Scaphoid Nonunion with Nonvascularized Autogenous Bone Grafting: A Prospective Study of 39 Patients.

PURPOSE: To evaluate the clinical and radiological outcomes of all-arthroscopic curettage and nonvascularized autogenous bone grafting for the treatment of established scaphoid nonunion. METHODS: Since 2023, 39 consecutive patients (mean age 29.3 years) with scaphoid nonunion were prospectively enrolled. Nonunion was classified according to the Schernberg system: zone 1 in 3 cases (8%), zone 2 in 7 (18%), zone 3 in 24 (62%), and zone 4 in 5 (13%). No humpback deformity was observed. Smoking status was documented: 26 nonsmokers (67%) and 13 smokers (33%). Surgery was performed at a mean of 13.

Vascularized versus non-vascularized bone grafting for scaphoid nonunion without avascular necrosis: a randomized clinical trial.

In this clinical trial, patients were randomized to receive a pedicled vascularized bone graft, based on the 1,2-intercompartmental supraretinacular artery, or a non-vascularized iliac crest graft. Fixation was done with K-wires. Union and time to union were assessed using CT scans at regular intervals. Twenty-three patients received a vascularized graft, and 22 received a non-vascularized graft. Thirty-eight patients were available for union assessment and 23 for clinical measurements. There were no significant differences in union incidence, time to union, incidence of complications, patient

[First results with a resorbable MgYREZr compression screw in unstable scaphoid fractures show extensive bone cysts].

Osteosynthesis with headless compression screws is an established treatment option for unstable scaphoid fractures. Common implants are made of titanium alloy or steel and usually remain in place. Due to implant density and ferromagnetic properties, artefacts are common in postoperative imaging procedures, e.g. MRI. Now resorbable implants made of magnesium alloy (MgYREZr) have become available. They have biomechanical properties equivalent to human bone and may be used as an alternative to the nonresorbable screw systems.5 patients with acute scaphoid fractures were treated with a double-thre

Diagnosis of occult scaphoid fractures: a randomized, controlled trial comparing bone scans to radiographs for diagnosis.

BACKGROUND: Many patients with suspected scaphoid fractures but negative radiographs are immobilized for ≥ 2 weeks and are eventually found to have no fracture. Bone scans are reportedly 99% sensitive for these injuries if done ≥ 72 hours postinjury. OBJECTIVE: The purpose of this study was to determine if early bone scans would allow for shorter cast immobilization periods in patients with suspected scaphoid fractures. METHODS: Twenty-seven patients with clinically suspected scaphoid fractures and negative radiographs were randomized to early diagnosis (bone scan within 3-5 days; n  =  12)

The outcomes and complications of 1,2-intercompartmental supraretinacular artery pedicled vascularized bone grafting of scaphoid nonunions.

PURPOSE: Over the past decade vascularized bone grafts that use a 1,2-intercompartmental supraretinacular artery (1,2-ICSRA) pedicle have gained popularity in the treatment of scaphoid nonunions. The purpose of this study was to evaluate critically the outcome, complications, and failures of 1,2-ICSRA-based vascularized bone grafting at our institution to understand better the appropriate indications, methods, and possible contraindications. METHODS: From January 1994 through July 2003, 50 scaphoid nonunions in 49 patients were treated with 1,2-ICSRA-based vascularized bone grafts. A retrospe

Free vascularized bone graft from the lateral epicondylar region of the humerus for treatment of scaphoid nonunion.

BACKGROUND: Free vascularized bone grafting (FVBG) has become one of the essential methods for treating scaphoid nonunion complicated by avascular necrosis. However, commonly used bone graft, including the medial femoral condyle bone graft and iliac crest bone graft, still present challenges such as a high rate of donor site complications and variations of vascular pedicle. In this study, we have introduced a novel bone graft-the lateral humeral condyle bone graft with the posterior branch of the radial collateral artery (PBRCA) as the vascular pedicle-as an alternative option, with the aim of

A Scoring System to Demonstrate the Risk for Bone Injury in Patients with Clinically Suspected or Occult Scaphoid Fracture.

BACKGROUND: It is important to diagnose a scaphoid fracture accurately and start the correct treatment in the shortest time possible. However, the fracture of bone may not be visible on x-ray. In such cases, patients are clinically diagnosed with suspected or occult scaphoid fractures. The aim of this study was to define a scoring system based on physical examination to demonstrate the risk for bone injury in patients with clinically suspected and occult scaphoid fractures with negative radiographs and anatomical snuff box tenderness and to decrease the costs and workforce loss due to unnecess

The factors affecting outcome after non-vascular bone grafting and internal fixation for nonunion of the scaphoid.

This study identified variables which influence the outcome of surgical management on 126 ununited scaphoid fractures managed by internal fixation and non-vascular bone grafting. The site of fracture was defined by a new method: the ratio of the length of the proximal fragment to the sum of the lengths of both fragments, calculated using specific views in the plain radiographs. Bone healing occurred in 71% (89) of cases. Only the site of nonunion (p = 1 x 10(-6)) and the delay to surgery (p = 0.001) remained significant on multivariate analysis. The effect of surgical delay on the probability
