Giant cell arteritis and scleritis: A rare association.

BACKGROUND: Giant cell arteritis (GCA) is a vasculitis of the large and medium-sized arteries in the elderly whose ischemic complications adversely affect the eye. The irreversible loss of visual acuity is most often related to acute anterior ischemic optic neuropathy. Very few cases of scleritis have been described in the literature. CASE PRESENTATION: The patient presented an obvious case of giant cell arteritis, initially revealed by an ophthalmologic involvement in the form of posterior scleritis, an ear, nose, and throat (ENT) involvement with vestibular and neurological involvement with…

Nodular syphilitic scleritis: a rare and forgotten entity.

Ocular manifestations of syphilis are usually seen in the secondary or tertiary stages of the disease, which is a nonspecific inflammatory response. We report a case of unilateral nodular scleritis in a patient with late latent syphilis, which resolved with intravenous crystalline penicillin for 2 weeks, topical fluorometholone, and tobramycin eye drops for 3 weeks.

Current Approach for the Diagnosis and Management of Noninfective Scleritis.

Scleritis is a rare, vision-threatening inflammation of the sclera that is often associated with life-threatening systemic illnesses. Rheumatoid arthritis remains the most common associated systemic rheumatic disease and the commonest systemic association of scleritis. Granulomatosis with polyangiitis is the most common cause of vasculitis-associated scleritis. The etiopathogenesis of scleritis remains unclear, but can be immune complex-mediated or due to a local delayed hypersensitivity reaction. Scleritis can involve either the anterior or posterior sclera, and has a wide spectrum of clinica…

An OCT Study of Anterior Nodular Episcleritis and Scleritis.

Anterior scleritis and episcleritis are a well-known presentation in tuberculosis. The case of a female patient with presumed tuberculous anterior scleritis and episcleritis is discussed in this article. Anterior segment OCT was efficient in diagnosis and evaluation of the therapeutic outcome. Antituberculosis chemotherapy was sufficient to achieve clinical remission.

Scleritis Associated with SAPHO Syndrome: A Case Report.

A 31-year-old woman developed bilateral painful red eyes. A slit-lamp examination revealed anterior diffuse scleritis. She had been diagnosed with palmoplantar pustulosis 2 years before. Further evaluation revealed hyperostosis of the sacroiliac joint and inflammation of the bilateral sternoclavicular joints and right sternocostal joint. Ultimately, she was diagnosed with SAPHO syndrome by rheumatologists after excluding other causative diseases. Scleritis associated with SAPHO syndrome is relatively uncommon. An identification of any systemic symptoms and early consultation with rheumatologis…

Nodular Scleritis in Association with Panuveitis in Behçet's Disease.

This case report involves a 32-year-old man with Behçet's disease who had simultaneous bilateral anterior uveitis, unilateral nodular scleritis, and occlusive vasculitis with retinal hemorrhages. Although scleritis is not a classical feature of Behçet's disease, a diagnosis of Behçet's disease should be considered in patients with scleritis.

Epidemiology and visual outcomes in patients with infectious scleritis.

PURPOSE: To describe the epidemiology of patients with infectious scleritis and identify factors associated with poor visual prognosis. METHODS: Retrospective review of inciting factors, causative organisms, and visual outcomes of patients with infectious scleritis. RESULTS: Fifty-five patients (56 eyes) with confirmed infectious scleritis were included. The median time from inciting event to scleritis symptoms was 1.9 months. Eyes with a history of pterygium surgery had a longer time from surgery to development of scleritis (median 49 months, range 0-183) compared to those with a history of…

Can lenalidomide play a role in the management of scleritis?

Lenalidomide is an immunomodulatory agent that was approved for the treatment of a monoclonal bone marrow disorders, myelodysplastic syndrome del(5q)(MDS del(5q)), in 2005; the drug was subsequently also approved for the treatment of refractory multiple myeloma, a bone marrow malignancy of the B-lymphocyte lineage. The purpose of this study is to report a case of MDS del(5q) in a female patient, which was most likely secondary to the immunosuppressive drugs that the patient was taking for scleritis. After lenalidomide treatment, the patient's haematological symptoms rapidly resolved and she be…

Bilateral scleritis and sclerokeratitis associated with IgA nephropathy.

PURPOSE: The purpose of this study is to report a case of bilateral nodular scleritis in a patient with final diagnosis of IgA nephropathy. METHODS: This is an observational case report. RESULTS: A male patient, 42 years old, presented with a bilateral nodular scleritis and OD sclerokeratitis. He had a previous history of acute otitis media and developed posterior renal failure and arterial hypertension. Clinical and systemic findings suggest Wegener's granulomatosis. A kidney biopsy was performed, and immunoflourescence findings demonstrated granular deposits of IgA in a mesangial pattern c…

Ocular hypertension and glaucoma incidence in patients with scleritis.

BACKGROUND: Scleritis belongs to the group of chronic inflammatory eye diseases that may cause ocular hypertension (OHT) and secondary glaucoma (SG). Aim of this study was to identify the incidence and risk factors for OHT and SG with regard to the scleritis form. METHODS: Single-center retrospective analysis of all consecutive patients with scleritis. RESULTS: A total of 271 scleritis patients (161 women, mean age 51.0 ± 16.9 years) were enrolled. The median follow-up was 17.0 ± 21.4 months (range 6-116). Of these patients, 56 (21 %) showed an intraocular pressure (IOP) increase with an ope…

Successful treatment of severe nodular scleritis with adalimumab.

Ocular features may occur in longstanding rheumatoid arthritis. Among them, scleritis represents the most frequent manifestation of ophthalmologic rheumatoid disease. There are three types of anterior scleritis: diffuse, nodular, and necrotizing with and without inflammation (scleromalacia perforans). Nodular scleritis is the second cause of anterior scleritis representing 20% of all types of scleritis. The conventional treatment of nodular scleritis consists in topical steroids and DMARDS. In severe cases, the therapy with immunosuppressive agents to avoid complications is necessary. We descr…

The role of Scheimpflug imaging in the management of posterior scleritis.

Posterior scleritis is a rare but sight-threatening ocular inflammatory disorder that can present in a number of different ways and sometimes silently. Diagnosis can be difficult, and usually is clinical, confirmed by B-mode ultrasonography. Patients can be at risk of angle-closure glaucoma in this condition, and traditional methods of angle assessment can be subjective and difficult to perform in the inflamed eye. We present a case of posterior scleritis where management was aided by the use of a Scheimpflug imaging device.

An immunohistochemical investigation of canine idiopathic granulomatous scleritis.

The clinical, histopathologic and immunohistochemical findings in three dogs with granulomatous scleritis are reported. The lesions of granulomatous scleritis were characterized by vasculitis, collagenolysis, granulomatous inflammation and perivascular lymphoplasmacytic aggregation. There was evidence of vascular immune complex deposition, and the inflammatory aggregates contained T lymphocytes, IgG plasma cells and macrophages expressing class II molecules of the major histocompatibility complex (MHC). There was no evidence for an infectious etiology in any case, and one of the dogs subsequen…

Posterior scleritis in a 7-month-old infant.

This article describes a 7-month-old infant with posterior scleritis, diagnosed on the basis of B-scan ultrasonography and computed tomography. The patient was initially diagnosed with preseptal cellulitis and endophthalmitis. Posterior scleritis should be considered in the differential diagnosis of acute orbital inflammation in children younger than 1 year.

Bisphosphonate-associated scleritis: a case report and review.

An 86-year-old female was treated for osteoporosis with alendronate, an aminobisphosphonate. Six weeks after alendronate therapy began, scleritis developed in the patient's right eye. The alendronate was discontinued, and, with the administration of prednisone, the symptoms of scleritis fully resolved. Rechallenge resulted in recurrence of the symptoms. Bisphosphonates are commonly used in the management of osteoporosis and are generally safe agents. Rarely, they have been linked with ocular inflammation. This case report reviews the literature on the ocular effects of bisphosphonates and disc…
