BACKGROUND: Operation choice is a complex decision in the surgical management of proximal humerus fractures. Recently, there has been an increase in the use of total shoulder arthroplasty (TSA) for complex fracture patterns.
HYPOTHESIS: Patients with proximal humerus fractures who receive TSA are more likely to have higher hospital charges and a prolonged length of stay relative to patients receiving hemiarthroplasty (HA), open reduction with internal fixation (ORIF) or closed reduction with internal fixation (CRIF).
MATERIALS AND METHODS: A statewide electronic database was used to identify

INTRODUCTION: Shoulder dislocations can cause acute and chronic instabilities that need to be addressed in order to restore joint functioning. The transfer of the coracoid process has become a feasible surgical procedure in patients with shoulder instability. Several concomitant injuries after recurrent dislocations have been described.
CASE PRESENTATION: A 32-year-old German man presented to our department with a history of recurrent shoulder dislocations. He was diagnosed with an avulsion fracture of the coracoid process and dislocation of an osseous piece with attachment to the conjoined t

BACKGROUND: Significant controversy surrounds optimal treatment of displaced 4-part proximal humeral fractures. Reverse total shoulder arthroplasty (RTSA) has recently been proposed as an alternative to hemiarthroplasty (HA) and open reduction-internal fixation (ORIF). Several authors have questioned the additional implant cost for RTSA. The purpose of this study was to compare outcomes and cost of RTSA, HA, and ORIF.
MATERIALS AND METHODS: We prospectively evaluated patients who underwent RTSA for displaced 3- and 4-part proximal humeral fractures and then retrospectively developed age- and

BACKGROUND AND AIM: One of the major problems of all ceramic restorations is their probable fracture against the occlusal force. The aim of the present in vitro study is to compare the effect of two marginal designs (shoulder and chamfer) on the fracture resistance of all ceramic restorations, INCERAM.
MATERIALS AND METHODS: On an extracted first maxillary premolar 50 in. chamfer margin (0.7 mm) was prepared. Ten impressions were taken using a polyvinylsiloxane. The impressions were poured with epoxy resin to fabricate dies. The same tooth was retrieved and 50 in. chamfer was converted into 9

BACKGROUND: Treatment of complex proximal humeral fractures remains controversial. In cases where adequate open reduction and internal fixation cannot be achieved, hemiarthroplasty has been the traditional treatment; however, clinical results have been mixed. Reverse shoulder arthroplasty (RSA) has been suggested as an alternative, and this study aimed to compare the functional results of RSA versus hemiarthroplasty in patients with acute proximal humeral fracture.
METHODS: Ten patients with acute proximal humerus fractures unsuitable for internal fixation (mean age 77) who underwent RSA for

BACKGROUND: Periprosthetic humeral fractures (PPHFs) after reverse total shoulder arthroplasty (RTSA) are challenging complications, particularly in elderly patients with osteoporotic bone and multiple comorbidities. Although surgical treatment is commonly recommended for displaced fractures, conservative management may be preferable in high-risk patients. This study reports 6 cases of PPHF after RTSA successfully treated with conservative management incorporating adjunctive teriparatide therapy.
METHODS: Six patients (mean age, 77.2 years; range, 70-87 years) who developed PPHF after RTSA we

PURPOSE: This study aims to determine the stress shielding and other radiological outcomes of patients who underwent an uncemented reverse shoulder arthroplasty (RSA) with an anatomic proximal coated stem for complex proximal humeral fractures (PHF) with 3 or 4 parts at 2 years postoperatively.
METHODS: 37 shoulders underwent an uncemented RSA for PHF from November 2015 to February 2019 and were followed up for 2 years. A radiographic assessment of stress shielding, filling ratio, stem stability (radiolucent lines/subsidence/alignment), tuberosity healing and notch was performed.
RESULTS: Th

PURPOSE: Reverse shoulder arthroplasty (RSA) in fractures is especially indicated in patients of advanced age, although the influence of age on functional outcomes, health-related quality of life (HRQoL) and complication rate is unknown. We hypothesized that complication rate would decrease in the geriatric population because of their reduced activity, improving the perceived HRQoL.
METHODS: Retrospective study of 42 patients with proximal humeral fractures treated with RSA, divided into two groups: <80 years (<80y)(n = 16) and ≥80 years (≥80y)(n = 26). The assessment tools used were radiolog

Floating shoulder associated with multiple rib fractures with cervical dorsal spine fractures and lung injury has not been reported in the literature. Patient underwent single stage elective clavicle and scapula fracture stabilization. Good functional outcome at 6 months postoperative with near normal range of movements. Surgery allows early mobilization and better functional outcome. A CT scan of thorax and spine is highly recommended in such high velocity shoulder injuries to detect associated rib, spinal fractures and lung injury.

BACKGROUND: Proximal humerus fractures are a common challenge in patients with initial treatment ranging from nonoperative management, to open reduction and internal fixation, to arthroplasty. Initial treatment failure is not uncommon, and revision to reverse total shoulder arthroplasty (rTSA) has emerged as a viable salvage option, though long-term outcomes in these patients are variable and remain underreported. This study aimed to evaluate long-term outcomes and failure rates of rTSA after failed initial proximal humerus management (nonoperative and operative). We hypothesized that rTSA for

An 85-year-old male underwent open reduction and internal fixation (ORIF) for a displaced acromion stress fracture that occurred two years prior. The complete fracture occurred two months after an ipsilateral reverse total shoulder arthroplasty (RTSA). Four weeks after his RTSA, the pain was felt at the posterior-superior shoulder with activities of his daily living as a rancher, reflecting non-compliant use. A stress fracture was suspected but not detected radiographically. Four weeks later, higher pain occurred after he lifted a hay bale, and a displaced basilar acromion fracture was detecte

OBJECTIVES: The humerus fracture is one of the most commonly occurring fractures. In this research, we attempted to evaluate and compare the extent of malrotation and biomechanical environment after surgical treatment of humeral shaft fractures.
METHODS: A finite element (FE) model of the shoulder was built based on Computed Tomography (CT) data of a patient with a humeral shaft fracture. The muscle group around the shoulder joint was simulated by spring elements. The changes of shoulder stresses under rotation were analyzed. The biomechanics of the normal shoulder and postoperative malrotati

OBJECTIVE: We sought to determine if white-light three-dimensional (3D) body scanning can identify clinically relevant shoulder girdle deformity after displaced diaphyseal clavicle fracture (DCF).
METHODS: Adult patients with DCF (OTA/AO 15A) were prospectively enrolled. Four subcutaneous osseous landmarks were used to measure shoulder girdle morphology of the injured and uninjured shoulder. Measurements were made both manually with a tape measure and digitally with a white-light 3D scanner. Bilateral radiographs were obtained, and clavicle length was recorded. Quick-Disabilities of the Arm,

BACKGROUND: The purpose of this investigation is to identify the in-hospital and 30-day postoperative complications for reverse total shoulder arthroplasty (RTSA) performed because of proximal humerus fracture (PHFx) vs. cuff tear arthropathy (CTA), and determine whether acute fracture is associated with differences in complications after RTSA.
METHODS: The National Surgical Quality Improvement Program database was queried for RTSA performed for PHFx and CTA. This database contains surgical outcomes within 30 days after the index procedure. Patients underwent a 1:1 propensity matched based on

Hemiarthroplasty implantation for proximal humerus fracture is challenging. In case of young demanding patients with functional rotator cuff, humeral head replacement is an option if open reduction internal fixation is not a viable solution. The criteria for success in terms of clinical outcomes and longevity are anatomic healing of the tuberosities and stem positioning (restoring correct height, version, and axis). The purpose of this article is to describe a technique including a new stem guide and a tuberosity anchor "lasso" system for the implantation of hemiarthroplasty in complex proxima

BACKGROUND: Concerns about test administration, reliability estimations, content and internal structure (dimensionality) of available shoulder measures for people with proximal humeral facture led to the development of a new clinician-observed outcome measure: the Shoulder Function Index (SFInX). The SFInX measures shoulder function by judgement of actual ability to perform daily tasks in which the shoulder is involved. Patients and health professionals had input into the instrument development, and Rasch analysis was used to create a unidimensional, interval-level scale. This study comprehens
