STUDY OBJECTIVES: Excessive daytime sleepiness (EDS) and high daytime REM sleep pressure are important sleep features of myotonic dystrophy (DM1). Small and uncontrolled studies have focused on EDS phenotype; none have focused on nocturnal REM sleep characteristics in DM1. Our objectives were to compare polysomnographic and multiple sleep latency test (MSLT) parameters, and both tonic and phasic components of REM sleep between DM1 and controls.
DESIGN AND PATIENTS: Forty consecutive DM1 patients and 40 sex- and age-matched controls were included. All subjects underwent overnight polysomnograp

STUDY OBJECTIVES: Sleep problems are a common phenomenon in most neurological and psychiatric diseases. In Parkinson disease (PD), for instance, sleep problems may be the most common and burdensome non-motor symptoms in addition to the well-described classical motor symptoms. Since sleep disturbances generally become apparent in the disease before motor symptoms emerge, they may represent early diagnostic tools and a means to investigate early mechanisms in PD onset. The sleep disturbance, REM sleep behavior disorder (RBD), precedes PD in one-third of patients. We therefore investigated sleep

Rapid eye movement sleep behavior disorder (RBD) has rarely been associated with a psychiatric condition. We report a series of cases of RBD presenting as psychiatric disorders. These patients were assessed at a specialist sleep disorders center and investigated using polysomnography and, where appropriate, magnetic resonance imaging of the brain and neuropsychological tests. These cases of RBD highlight the varying presentations and causes of RBD that may involve psychiatrists, sleep specialists, and primary care physicians. These include idiopathic RBD presenting as depression, antidepressan

STUDY OBJECTIVES: There is uncertainty concerning the relative contribution of arousal, chemoreceptor stimulation, and their potentially interactive effects, to the acute cardiovascular changes observed during sleep in patients with sleep-disordered breathing. The purpose of this study was to compare cardiovascular responses (heart rate, skin blood flow, and pulse transit time, a non-invasive measure of arterial wall stiffness) to auditory induced arousal from stage 2 sleep under conditions of normoxia and overnight mild hypoxia.
DESIGN: Randomised crossover.

Adverse childhood experiences (ACEs) are increasingly being recognized as an important risk factor for attention-deficit/hyperactivity disorder (ADHD). However, their relationship has not been adequately explored in low- and middle-income countries, including India. This study aims to explore the association between ACEs and ADHD symptoms among young adults in Delhi-NCR, India. This cross-sectional study included 1646 young adults aged 18-25 years. Sociodemographic data were collected using a pretested and modified interview schedule. ACEs were recorded using the ACE-International Questionnair

The influence of the cholinergic agonist RS 86 on electroencephalographic (EEG) sleep was investigated in 21 healthy members of families identified as being at high risk for psychiatric disorders and in 17 healthy control subjects without any personal or family history of a psychiatric illness. In comparison to the placebo night, the administration of RS 86 led to a shortening of rapid eye movement (REM) latency in both groups. This effect, however, was much more pronounced in the high-risk group, whereas in the control subjects the arousal system and the slow-wave sleep during the first nonRE

AIM: The aim of this study is to determine whether there are any differences in the dream content in different sleep disorders and to describe their characteristics.
PATIENTS AND METHODS: We studied four sleep disorders: sleep apnoea and hypopnoea syndrome (SAHS), primary insomnia (PI), idiopathic REM sleep behaviour disorder (IRBD) and narcolepsy type I. Each patient was asked to keep a dream diary for two weeks. The content of the diaries was transcribed and analysed for length, mental content, complexity and threat. The results were compared to establish differences.
RESULTS: Eighty-nine

PURPOSE: Previous studies show a high prevalence of obstructive sleep apnea (OSA) patients with a higher non-rapid eye movement (NREM) apnea-hypopnea index (AHI) (NREM-AHI) than rapid eye movement (REM) AHI (REM-AHI). However, the clinical significance of this phenomenon in patients with OSA is unknown. This study aimed to investigate whether there were significant differences in clinical and polysomnographic features between the NREM-AHI > REM-AHI group and the REM-AHI > NREM-AHI group and to determine whether NREM-AHI > REM-AHI or REM-AHI > NREM-AHI is a specific clinical entity.

PURPOSE: Reduced rapid-eye movement (REM) sleep has been linked to increased mortality in the general population. We investigated whether diminished REM sleep is associated with higher mortality in adults with coronary artery disease (CAD) and obstructive sleep apnea (OSA).
METHODS: This secondary analysis of the RICCADSA trial included 356 revascularized CAD patients with OSA (apnea-hypopnea index [AHI] ≥ 15 events/h) and total sleep time (TST) ≥ 240 min on baseline polysomnography. Reduced REM sleep was defined as the lowest quartile of REM percentage. Cox proportional hazards models assess

BACKGROUND: The minimum narcolepsy criteria "mean sleep latency (MSL) ≤8 min and ≥2 sleep onset rapid eye movement (REM) periods (SOREMPs) on polysomnography (PSG) and the multiple sleep latency test (MSLT)," according to The International Classification of Sleep Disorders, Third Edition (ICSD-3), are not specific to narcolepsy. Recently, the characteristic sleep stage sequences preceding SOREMPs in narcolepsy have received attention, but their diagnostic utility remains unclear.
METHODS: We retrospectively reviewed PSG/MSLT records and chart data for 102 Japanese patients with hypersomnia an

BACKGROUND: Accumulating studies have linked obstructive sleep apnea (OSA) to the occurrence of atrial fibrillation (AF). However, data on the relationship between sleep architecture parameters in OSA patients and the occurrence of AF are limited.
OBJECTIVE: This exploratory study examined the association between 14 objectively collected sleep architecture parameters and the incidence of AF in OSA patients.
METHODS: This study is a population-based prospective cohort study that included 1810 participants from the Sleep Heart Health Study (SHHS) who underwent in-home polysomnography monitorin

BACKGROUND: Supine sleeping position and obesity are well-known risk factors for obstructive sleep apnea (OSA) and modulate the risk for OSA-related daytime symptoms. Although respiratory event durations are associated with OSA-related severe health consequences, it is unclear how sleeping position, obesity, and daytime sleepiness are associated with respiratory event durations during REM and NREM sleep. We hypothesize that irrespective of the apnea-hypopnea index (AHI), respiratory event durations differ significantly between various OSA subgroups during REM and NREM sleep.

In human African trypanosomiasis (sleeping sickness), sleep and wake episodes are sporadically distributed throughout the day and the night. To determine whether these sleep disturbances affect the 24-h hormone profiles and the normal relationships between hormone pulsatility and sleep stages, polygraphic sleep recordings and concomitant hormone profiles were obtained in 6 African patients with sleeping sickness and in 5 healthy African subjects selected from Abidjan on the Ivory Coast. Polysomnographic recordings were continuous, and blood was taken every 10 min throughout the 24-h period. Pl

While VPS13C is a recessively inherited Parkinson's disease (PD) gene, its potential dominant effects in idiopathic Rapid-eye movement (REM) sleep behavior disorder (iRBD) remain unexplored. The relation between its monogenic form and the onset of PD suggested that subtype specificity may need to be considered. We examined the presence of likely pathogenic VPS13C variants in 150 iRBD and 180 α-synucleinopathy patients (iRBD-first and movement disorder-first). VPS13C variants were significantly enriched in iRBD patients, and ten iRBD risk variants have been identified. iRBD risk VPS13C variant

Changes of neurotransmitter levels and autonomic nervous system parameters during sleep, such as variations in blood pressure, heart and respiratory rate are well known. REM and NREM sleep stage differences are created by neurotransmitter changes mainly generated in the brainstem, but several distinct subcortical areas, such as the amygdala, hippocampus, basal ganglia or anterior insula are involved. The neurotransmitter changes differ in sleep stages and define not only the stage of sleep, but also the autonomic alterations. During NREM sleep, there is a parasympathetic dominance, while durin

BACKGROUND AND OBJECTIVES: Clinical trials in REM sleep behavior disorder (RBD) to delay or prevent the development of Parkinson disease (PD), dementia with Lewy bodies (DLBs), and multiple system atrophy (MSA) will soon begin. The Prodromal Synucleinopathy Rating Scale (PSRS) was developed to capture the breadth and severity of clinical burden of prodromal disease. We analyzed the clinicometric properties and reliability of the PSRS in the North American Prodromal Synucleinopathy (NAPS) cohort.
METHODS: PSRS ratings were examined for visits conducted from August 2022-June 2025 on participant

Identifying the properties of the rapid eye movement (REM) sleep circuitry and its relation to diseases has been challenging due to the neuronal heterogeneity of the brainstem. Here, we show in mice that neurons in the pontine sublaterodorsal tegmentum (SubLDT) that express corticotropin-releasing hormone-binding protein (Crhbp+ neurons) and project to the medulla promote REM sleep. Within the medullary area receiving projections from Crhbp+ neurons, neurons expressing nitric oxide synthase 1 (Nos1+ neurons) project to the SubLDT and promote REM sleep, suggesting a positively interacting loop

The cerebral cortex is spontaneously active during sleep, yet it is unclear how this global cortical activity is spatiotemporally organized, and whether such activity not only reflects sleep states but also contributes to sleep state switching. Here we report that cortex-wide calcium imaging in mice revealed distinct sleep stage-dependent spatiotemporal patterns of global cortical activity, and modulation of such patterns could regulate sleep state switching. In particular, elevated activation in the occipital cortical regions (including the retrosplenial cortex and visual areas) became domina
