Confusional arousals during non-rapid eye movement sleep: evidence from intracerebral recordings.

STUDY OBJECTIVES: Confusional arousals (CA) are characterized by the association of behavioral awakening with persistent slow-wave electroencephalographic (EEG) activity during non-rapid eye movement (NREM) sleep-suggesting that sensorimotor areas are "awake" while non-sensorimotor areas are still "asleep." In the present work, we aimed to study the precise temporo-spatial dynamics of EEG changes in cortical areas during CA using intracerebral recordings. METHODS: Nineteen episodes of CA were selected in five drug-resistant epileptic patients suffering incidentally from arousal disorders. Spe

Twenty-four-hour disruption of the sleep-wake cycle and sleep-onset REM-like episodes in a rat model of African trypanosomiasis.

STUDY OBJECTIVES: Patients with human African trypanosomiasis (sleeping sickness) due to the inoculation of Trypanosoma brucei gambiense or rhodesiense show a major disruption of the 24-hour sleep-wake distribution, accompanied by the occurrence of sleep-onset rapid-eye-movement (REM) sleep episodes, proportional to the severity of the illness. Although animal models of human African trypanosomiasis have been developed to understand the pathogenic mechanisms leading to immune alterations, the development of an animal model featuring the alterations of endogenous biologic rhythms remains a nece

Sleep disorders in the elderly and their management.

Sleep aging is a complex physiological process driven by changes in circadian regulation systems (phase advance, decreased melatonin secretion) while homeostatic drive remains largely preserved. In healthy older adults, sleep is characterized by sleep fragmentation, a decrease in deep slow-wave sleep, and reduced sleep efficiency, though adaptation mechanisms often prevent significant complaints. Nevertheless, approximately 50% of older adults report sleep disturbances, frequently linked to geriatric comorbidities (neurocognitive disorders, heart failure, chronic pain), polypharmacy, or change

The sleep of non-depressed patients with panic disorder: a comparison with normal controls.

All-night sleep EEG recordings were performed in non-depressed patients with panic disorder, agoraphobia, and a group of age- and sex-matched normal controls. Patients were selected according to DSM-IV and all subjects were studied under drug-free conditions. In addition to sleep continuity disturbances, patients with panic disorder have a reduced percentage of slow wave sleep, mainly due to diminished amounts of stage 4. REM sleep characteristics are identical in the two groups. When depressive co-morbidity and non-specific causes of insomnia are excluded, the sleep EEG of panic patients seem

Sleep phenotype in the Townes mouse model of sickle cell disease.

PURPOSE: Patients with sickle cell disease (SCD) regularly experience abnormal sleep, characterized by frequent arousals and reduced total sleep time. However, obstructive sleep apnea syndrome (OSAS) is a common comorbidity of SCD, making it unclear whether the disease per se is impacting sleep, or sleep disruption is secondary to the presence of OSAS. Thus, we assessed sleep, independent of OSAS, using a mouse model of SCD. METHODS: Sleep was compared between 10-to-12-week-old Townes knockout-transgenic mice with the sickle cell phenotype SS (n = 6) and Townes mice with sickle cell trait AS

Dex/CRH-test response and sleep in depressed patients and healthy controls with and without vulnerability for affective disorders.

Sleep electroencephalographic (EEG) abnormalities and increased hypothalamo-pituitary-adrenal (HPA) axis activity are the most prominent neurobiological findings in depression and were suggested as potential biomarker for depression. In particular, increased rapid eye movement sleep (REM) density, deficit in slow wave sleep and excessive stress hormone response are associated with an unfavorable long-term outcome of depression. Recent studies indicate that the sleep and endocrine parameters are related to each other. This study investigated the association of sleep structure including a quanti

Chronic fenfluramine administration: some cerebral effects.

Human cerebral function was monitored electrophysiologically during sleep over a period of months before, during, and after the intake of fenfluramine, 40-120 mg/day. Effects included dose-related reduction of paradoxical sleep, increase of intra-sleep restlessness, and changes in E.E.G. slow-wave sleep. It is hypothesized that weight loss may be associated with increase of the last. Grinding of teeth (bruxism) also was noted.Long-term studies make it possible to demonstrate changing central effects with time, including tolerance phenomena. Obstructive Sleep Apnea and Alzheimer's Disease Pathology: Is Sleep Architecture the Missing Key?

Impairments of the sleep architecture due to disrupted sleep in individuals with obstructive sleep apnea (OSA) may result in reduced slow wave sleep (SWS), intermittent hypoxemia, and excessive day time sleepiness- all factors that have been shown to impact Alzheimer's disease (AD) risk. In this commentary, we comment on the work by Cavuoto and colleagues in which they examine the associations between nocturnal hypoxemia or sleep disruptions (during SWS) and amyloid-β burden in individuals with OSA. We review the findings in the context of other similar studies and highlight the strengths and

Reflection of tolerance to alcohol in the structure of the sleep wakefulness cycle.

The purpose of the present work was to study the effects of tolerance to ethanol on the sleep-wakefulness cycle (SWC) structure. Experiments were carried out in chronic conditions on the adult cats (n=5). The following methods were used: the stereotaxic - for implanting electrodes; polysomnographic - for EEG registration of the SWC structure. Alcoholisation (0.2-2.5 g/kg 25% ethanol solution) was conducted by intraperitoneal injections that lasted for two weeks. The obtained results were processed statistically and significance of the changes was determined by the Student t-test. Low single do

The relevance of pharmacological studies to sleep research in psychiatry.

The involvement of central serotonergic mechanisms in the organisation of human sleep has been confirmed in several single and multiple-dose clinico-pharmacological studies with the specific serotonin-S2-antagonist ritanserin. The pronounced thymosthenic effect observed with this compound in patients suffering from dysthymia, generalized anxiety disorder and negative symptoms of schizophrenia can possibly be attributed to a restoration of the energetic function during the night as a consequence of a dramatic increase in slow wave sleep during treatment with ritanserin.

Prevalence and patterns of video-EEG abnormalities in children with autism spectrum disorder: Age- and sex-specific findings from a large-scale cross-sectional study.

Children with autism spectrum disorder (ASD) commonly exhibit electroencephalographic (EEG) abnormalities, but age- and sex-related patterns of video-EEG (v-EEG) findings remain incompletely characterized. This retrospective cross-sectional study investigated v-EEG abnormalities in 761 children with ASD aged 1-17 years. All participants underwent at least 4 h of un-sedated v-EEG monitoring. The use of prolonged, un-sedated monitoring enhanced the sensitivity for detecting sleep-activated abnormalities and provided a more representative sample of physiological brain activity. Results revealed t

The relationship between sleep and epilepsy.

The occurrence of seizures in the sleep state is observed in nearly one third of patients. This is caused by an intimate relationship between the physiological state of sleep and the pathological process underlying epileptic seizures. Both sleep and sleep deprivation influence the frequency of epileptiform discharges on electroencephalograms as well as the occurrence of clinical seizures, typically during nonrapid eye movement sleep. The relationship of epileptiform activity to nonrapid eye movement sleep is vividly shown in the syndrome of continuous spikes in slow-wave sleep and the Landau-K

Vascular stiffness determined from a nocturnal digital pulse wave signal: association with sleep, sleep-disordered breathing, and hypertension.

OBJECTIVES: Reflection of the finger pulse wave form is a valid measure of arterial stiffness, which may be continuously assessed during sleep. We investigated the relationships between sleep, sleep-disordered breathing, hypertension, and pulse propagation time (PPT) in patients with suspected sleep apnea. METHODS: The digital photoplethysmographic signal derived from finger pulse oximetry was recorded during overnight sleep studies in 440 patients (64% men, age 55 ± 12 years, BMI 30 ± 6 kg/m, apnea-hypopnea index 19 ± 19 n/h). Seasonal affective disorder. A description of the syndrome and preliminary findings with light therapy.

Seasonal affective disorder (SAD) is a syndrome characterized by recurrent depressions that occur annually at the same time each year. We describe 29 patients with SAD; most of them had a bipolar affective disorder, especially bipolar II, and their depressions were generally characterized by hypersomnia, overeating, and carbohydrate craving and seemed to respond to changes in climate and latitude. Sleep recordings in nine depressed patients confirmed the presence of hypersomnia and showed increased sleep latency and reduced slow-wave (delta) sleep. Preliminary studies in 11 patients suggest th

[Physiology of normal sleep].

Normal sleep of a young adult is composed of around 4 to 6 cycles of 90 minutes each. Two states of sleep have been established, REM sleep and Non REM sleep. REM sleep is mainly characterized by a complete atonia which creates a physiological paralysis. Slow wave sleep is characterized by an increase of more than 20% of EEG delta waves. Sleep could be represented by a two process model. The first one, the homeostatic component, represents the propensity to sleep, the second one, the circadian component, symbolizes 24th light-dark cycle of the rotation of the earth. The homeostatic component is

Recovery after prolonged sleep deprivation: residual effects of slow-release caffeine on recovery sleep, sleepiness and cognitive functions.

A long work schedule often results in sleep deprivation, sleepiness, impaired performance and fatigue. We investigated the residual effects of slow-release caffeine (SRC) on sleep, sleepiness and cognitive performance during a 42-hour recovery period following a 64-hour continuous wakefulness period in 16 healthy males, according to a double-blind, randomised, placebo-controlled, crossover study. Three hundred milligrams of SRC or placebo was given twice a day at 21:00 and 9:00 during the first 48 h of wakefulness. Recovery sleep was analysed with electroencephalography (EEG) and wrist actigra

Brain single photon emission computed tomography imaging in Landau-Kleffner syndrome.

Five right-handed children with Landau-Kleffner syndrome (LKS) who had disease onset between the ages of 3 and 9 years were studied with EEG and single-photon emission computed tomography (SPECT) before and, in four cases, after 6 months of corticosteroid treatment. EEG findings included both focal and generalized spikes as well as spike-wave discharges with bilateral temporal predominance. These increased markedly during sleep in 1 child, and continuous spike-and-wave complexes appeared during slow-wave sleep in another patient. Neuropsychological testing demonstrated verbal auditory agnosia.

Polysomnography findings in spinocerebellar ataxia type 6.

Spinocerebellar ataxia type 6 (SCA6) is usually described as a pure ataxia syndrome. However, SCA6 patients may have sleep complaints. In this paper, sleep disorders were investigated in patients with SCA6. Twelve SCA6 patients and 12 subjects matched by gender, age and body mass index (control group) underwent polysomnography and clinical investigation for sleep disorders. SCA6 had a higher frequency of snoring (P = 0.01), a higher index of awakening due to respiratory events (P = 0.003) and central apnea events during sleep (P = 0.024), a longer sleep Stage N1 (P = 0.02) and a lower sleep St

Homeostasis and narcolepsy.

Narcolepsy is characterized by irresistible daytime sleep episodes and cataplectic attacks. Because of the finding of an ultradian rhythmicity of slow-wave sleep in narcolepsy, an alteration of nonrapid eye movement sleep homeostatic regulation has been hypothesized to explain the impairment of the sleep-wakefulness cycle. This hypothesis was tested by two different methods: 1) a sleep-deprivation method (16 or 24 hours) increasing the prior sleep wakefulness and 2) a bed-rest method shortening the prior sleep wakefulness. In both studies normal subjects, sex- and age-matched to narcoleptic su
