The sphenopterygoid canal and pterygoid foramen.

BACKGROUND: In human anatomy, there are well-known the foramina of the greater sphenoidal wing, the foramen rotundum, the foramen ovale, the foramen spinosum, as well as the inconstant sphenoidal emissary foramen of Vesalius and the foramen of Arnold. Different canals are found in several species of rodents, such as the alisphenoid and sphenopterygoid (SPC) canals. METHOD: It was re-explored an archived computed tomography angiogram of a 60 y.o. female case. RESULTS: Bilateral superior recesses of the pterygoid fossae (SRPF) and a right SPC were found. The SRPF on each side penetrated the no

Sternberg's Canal and Defect: Is the Lateral Craniopharyngeal Canal a Source of Spontaneous Cerebrospinal Fluid Leak? Anatomic and Radiological Analysis in Pediatric and Adult Populations.

BACKGROUND: The lateral craniopharyngeal or Sternberg's canal (SC) originates from superior orbital fissure (SOF) and traverses the sphenoid body into the nasopharynx. A remnant of the canal, Sternberg's defect (SD), has been debated as a source of cerebrospinal fluid (CSF) leak. The canal was described in 1888, and there is limited accurate visual illustration in the literature. OBJECTIVE: To provide a detailed anatomic and radiological illustration of the canal in pediatric and adult population including the mechanism undermining the incidence of the canal, and the possibility of the canal

Endoscope-Assisted Transmaxillosphenoidal Approach to the Sellar and Parasellar Regions: An Anatomic Study.

BACKGROUND: Anterolateral skull base surgery in the sellar and parasellar regions has always represented a technical challenge for neurosurgeons. The microscopic endoscope-assisted transmaxillosphenoidal approach (MEMSA) affords a direct surgical corridor free from critical skull base structures. Here we describe and critically evaluate the use of MEMSA to access the sellar and parasellar areas, in terms of surgical exposure and operability. METHODS: Six cadaveric heads were examined. A stepwise dissection using MEMSA was performed. Relevant anatomy and surgical technique were critically desc

Analysis of pneumatization and neurovascular structures of the sphenoid sinus using cone-beam tomography (CBT).

BACKGROUND: The sphenoid sinus is a frequent target of paranasal sinus surgery. Because of the high risk of injuring the surrounding structures (e.g. internal carotid artery, optical nerve) a preoperative imaging is absolutely necessary. PURPOSE: To analyze the possibilities of cone-beam computed tomography (CBCT), which is especially quite a new technique in ENT, in the evaluation of the sphenoid sinus, its surrounding structures, and the corresponding anatomical variations. MATERIAL AND METHODS: This was a retrospective, single-centre study of 580 patients (1160 sides = cases). The Accu-I-

A surgical technique to avoid postoperative enophthalmos in the cranioorbital approach. Technical note.

The authors describe a surgical technique to avoid postoperative enophthalmos when using the cranioorbital approach. To perform osteotomies with a less demanding technique, two separate bone flaps were created: 1) a free frontotemporal bone flap and 2) en bloc removal of the superior and lateral orbital rims: Because the latter bone flap includes both the orbital roof and the posterolateral wall of the orbit with the greater wing of the sphenoid bone, unnecessary bone defects in the lateral orbital wall are avoided. The technique has been performed in seven patients treated for medially locate…

Supraorbital, Transorbital, and Endonasal Approaches to the Supraoptic Recess: A Comparative, Qualitative, and Quantitative Anatomic Study.

BACKGROUND AND OBJECTIVES: The supraoptic recess is a bony pyramid-shape area with a base that abuts the sphenoid sinus and an apex that corresponds intracranially to the anterior clinoid process and the body of the lesser sphenoid wing. During an extended endoscopic endonasal approach, the recess needs to be identified and delineated to achieve further lateral extension over the planum sphenoidale. By drilling the supraoptic recess in a medial-to-lateral direction toward the body of the lesser sphenoid wing, the most medial portion of the lesser wing overlying the orbit could be exposed and p

Photosensitivity and double vision as initial symptoms of colon cancer.

A 75-year-old man suffering from rheumatoid arthritis, myxoedema and type II diabetes mellitus, presented with occasional double vision and photosensitivity. The patient underwent an MRI of the brain showing a tumour located in the right of the sphenoid bone. A subsequent diagnostic CT scan of the thorax, abdomen and pelvis revealed a left-sided colon tumour, which biopsy proved as being an adenocarcinoma.

Diffuse symmetric meningioma en plaque mimicking metabolic disease of bone.

A 50-year-old African American woman with right proptosis and decreased vision demonstrated symmetric, bilateral thickening of the sphenoid wings, calvarium, and skull base on computed tomography. Histopathologic evaluation of the hyperostotic lesion was consistent with meningotheliomatous meningioma. The patient underwent uncomplicated right frontal orbital craniectomy with postoperative radiation. The presentation of en plaque meningioma, when diffuse and symmetric, can mimic a metabolic disease of bone.

Endoscopic Endonasal Transpterygoid Approach and the Need for Myringotomy.

OBJECTIVE: The expanded endonasal transpterygoid approach (EETA) is used to access the middle and posterior fossa through the pterygoid process. Traditionally, the eustachian tube (ET) was resected during EETA, which often required subsequent myringotomy for inner ear drainage. Anterolateral transposition of the ET was proposed to decrease potential morbidity associated with resection. However, a comparison of resection versus transposition regarding the need for subsequent myringotomy has not been reported. METHODS: This is a retrospective cohort study of patients who underwent an EETA. Pati

The management of non-functioning pituitary adenomas.

Non-functioning pituitary adenomas most commonly present secondary to mass effect and are classified according to their size and immunohistochemical staining. Local intrasellar mass effect may cause varying degrees of hypopituitarism. With extrasellar growth, neurological signs and symptoms develop. Appropriate therapy for these tumors requires close interaction across multiple disciplines. Trans-sphenoidal surgery offers safe and effective treatment in the overwhelming majority of patients with relatively low risk of new neurological and endocrinologic deficits. The multidisciplinary manageme

[Sphenoidal mucocele. A case report].

Mucocele of the sphenoidal sinus is a rare benign neoplasm. These tumors are probably under-diagnosed as they may be asymptomatic or cause non-specific symptoms. The findings of computed tomography and magnetic resonance imaging increase the precision of the pretherapeutic evaluation. Treatment is surgical. A case of sphenoidal sinus mucocele in a 41-year-old man is reported. Sudden dyplopia was the first symptom. The characteristics of sphenoidal mucocele are reviewed, with special attention to its clinical features and radiodiagnosis.

Congenital trans-sellar trans-sphenoidal encephalocele: a systematic review of diagnosis, treatment, and prognosis.

PURPOSE: Clinical presentations encompass respiratory, feeding issues, nasopharyngeal mass, meningitis, CSF leakage, craniofacial anomalies, and endocrine problems. Surgery is the primary treatment, transitioning from frontal craniotomy to endoscopic methods, offering improved outcomes. Yet, more studies are needed. A comprehensive review on trans-sellar trans-sphenoidal encephalocele (TSTSE) is missing. Our study aims to fill this gap, offering a comprehensive perspective for physicians. METHODS: This review adhered to the PRISMA guideline. Eligible studies focused on human subjects, specifi

Refinement of the extradural anterior clinoidectomy: surgical anatomy of the orbitotemporal periosteal fold.

OBJECTIVE: Extradural removal of the anterior clinoid process is technically challenging because of the limited exposure. In our study of the extradural anterior clinoidectomy, we describe anatomic details and landmarks to facilitate sectioning of the orbitotemporal periosteal fold and elevation of the temporal fossa dura from the superior orbital fissure. We assess the morbidity associated with these procedures as well as compare the indications, advantages, and disadvantages of intra-versus extradural clinoidectomy. METHODS: Of five formalin-fixed cadaveric heads, four were used for cadaver

[Frontosphenotemporal fractures : a separate anatomical and clinical entity (author's transl)].

The author describes twenty cases of a type of external craniofacial fracture centred on the orbit, characterized by the frequency of ophthalmological complications, and mobilizing a segment of the frontosphenotemporal vault, and segments of the middle and anterior stages of the base of the skull. It is suggested that these be called frontosphenotemporal fractures.
