[Remodelling of the spinal canal after thoracolumbar burst fractures: significance of nonoperative management].

OBJECTIVES: To determine the phenomenon of remodelling of the spinal canal after thoracolumbar burst fracture and to investigate the efficacy of nonoperative management. METHODS: A total of 31 patients with thoracolumbar burst fractures, including 7 untreated, 16 treated nonoperatively, and 8 treated operatively, were retrospectively analysed and followed up for 3 to 7 year. The initial and final degrees of neurological deficit and stenotic ratio of the spinal canal were recorded. RESULTS: The stenotic ratio significantly decreased (P < 0.01) from the discharge (range: 12.3% to 74.5%, averag

Spontaneous remodeling of the spinal canal after conservative management of thoracolumbar burst fractures.

STUDY DESIGN: Forty-two conservatively treated patients with a burst fracture of the thoracic, thoracolumbar, or lumbar spine with more than 25% stenosis of the spinal canal were reviewed more than 1 year after injury to investigate spontaneous remodeling of the spinal canal. OBJECTIVES: To investigate the natural development of the changes in the spinal canal after thoracolumbar burst fractures. SUMMARY OF THE BACKGROUND DATA: Surgical removal of bony fragments from the spinal canal may restore the shape of the spinal canal after burst fractures. However, it was reported that restoration of

Microdecompression and uninstrumented single-level fusion for spinal canal stenosis with degenerative spondylolisthesis.

STUDY DESIGN: A retrospective review was completed on 21 patients who had a "least invasive" (one or two level) microdecompression and uninstrumented single-segment lumbar fusion for spinal canal stenosis with degenerative spondylolisthesis. OBJECTIVE: To determine whether a "least invasive" approach to lumbar spinal canal stenosis and degenerative spondylolisthesis would yield acceptable results. SUMMARY OF BACKGROUND DATA: The prevailing surgical technique for symptomatic spinal canal stenosis with degenerative spondylolisthesis is a wide midline decompression and instrumented fusion. MET

[Rehabilitation following surgery of the lumbar spine: herniated disk, spinal canal stenosis, spondylodesis].

The value of the physical therapy after the operation at the lumbar spine is unquestionable. But the treatment should be chosen individually, according to the kind of operation, the patient's age, the state of the spine, but also according to the social and familiar conditions. We present our criteria of choosing the adequate treatment. In times of sharp increase of the medical costs such frequently, rather costly treatment must only be prescribed when inevitably indicated.

The Role of Electrophysiological Workup in Lumbar Spinal Canal Stenosis (LSCS): A Systematic Review.

STUDY DESIGN: Systematic literature review. OBJECTIVE: Update on diagnostic utility of electrophysiology in lumbar spinal canal stenosis (LSCS). SUMMARY OF BACKGROUND: LSCS is a highly prevalent degenerative spine condition characterized by neurogenic claudication, radicular pain, and muscle weakness. While lumbar spine MRI is the imaging modality for detecting spinal canal narrowing, it correlates poorly with clinical symptoms. Electrophysiological methods, including electromyography (EMG), nerve conduction studies (NCS), and evoked potentials (MEP and SEP), may provide complementary inform

Fluoroscopy-Guided Surgical Removal of a Microchip from the Spinal Canal of Two Cats.

Two cats were presented with acute left-sided paresis after implantation of a microchip at the referring veterinary clinic. Neurological examinations were consistent with left-sided lesions between spinal cord segments C1 and C5. Orthogonal radiographs of the cervical spine showed a microchip dorsoventrally oriented, partially embedded in the cervical vertebral canal. Fluoroscopy was used to localize and retrieve the foreign body from the cervical spinal cord in each case. Improvement in clinical condition and return to ambulation was observed in both cats within 48 hr of surgical removal of t

Introduction of a Novel Sequential Approach to the Ponte Osteotomy to Minimize Spinal Canal Exposure.

Ponte osteotomy is an increasingly popular technique for multiplanar correction of adolescent idiopathic scoliosis. Prior cadaveric studies have suggested that sequential posterior spinal releases increase spinal flexibility. Here we introduce a novel technique involving a sequential approach to the Ponte osteotomy that minimizes spinal canal exposure. One fresh-frozen adult human cadaveric thoracic spine specimen with 4 cm of ribs was divided into three sections (T1-T5, T6-T9, T10-L1) and mounted for biomechanical testing. Each segment was loaded with five Newton meters under four conditions:

Spinal Eumycetoma: A Rare Cause of Lumbar Canal Stenosis and Significance of "Dot in Circle".

Mycetoma is a chronic granulomatous disease that is more common in tropical regions with predominant involvement of foot. Spinal mycetoma presenting as lumbar canal stenosis is extremely rare. We hereby present a case of fungal eumycetoma of vertebral column in a 42-year-old male who presented with chronic progressive low back pain and features of lumbar canal stenosis without any skin swelling or discharging sinuses. The "dot-in-circle" sign, a highly specific magnetic resonance imaging (MRI) and ultrasonography (USG) sign of mycetoma has been described in the literature as a pathognomonic fe

Ewing's sarcoma in the spinal canal of T12-L3: A case report and review of the literature.

Primary Ewing's sarcoma (ES) is rare, especially when it occurs in the spinal canal during middle or old age. The rarity of Ewing's sarcoma breakpoint region 1 fusion-negative ES has been reported in the literature. The present case report describes a 60-year-old Chinese patient who was diagnosed with ES originating from the spinal canal in 2016. The patient was hospitalized with pain resembling electric shock in the waist and buttocks, which occurred intermittently for 1 month, and incontinence for 1 week. Magnetic resonance imaging demonstrated multiple inhomogeneous, oval-shaped nodules in

Pedicle distraction increases intervertebral and spinal canal area in a cadaver and bone model.

INTRODUCTION: Lumbar spinal stenosis is degenerative narrowing of the spinal canal and/or intervertebral foramen causing compression of the spinal cord and nerve roots. Traditional decompression techniques can often cause significant trauma and vertebral instability. This paper evaluates a method of increasing pedicle length to decompress the spinal and intervertebral foramen, which could be done minimally invasive. METHODS: Three Sawbone (Sawbones Europe, Sweden) and 1 cadaveric lumbar spine underwent bilateral pedicle distraction at L4. A pedicle channel was drilled between the superior art

[Ectopic adrenal cortical adenoma in the spinal canal: A case report and a review of the literature].

Ectopic adrenal cortical neoplasms are extremely rare. The authors describe their own case of intradural, extramedullary conus medullaris adenoma that occurred in a 55-year-old woman, which was clinically accompanied by lumbar pains, left leg numbness, and left foot weakness during 10 years. The mass was histologically composed of rounded and polygonal cell fields with rounded, regular nuclei and abundant eosinophilic and clear cytoplasm. There were no necroses or mitoses. The cells were immunohistochemically positive for cytokeratin AE1/3, vimentin, inhibin-α, melan-A, and synaptophysin. An e

Pycnodysostosis with Multi-Segmental Spinal Canal Stenosis due to Ossification of the Yellow Ligament.

Pycnodysostosis is an autosomal recessive disorder characterized by osteosclerosis, small stature, acro-osteolysis of the distal phalanges, loss of the mandibular angle, separated cranial sutures with open fontanels, and frequent fractures. One identified cause of the disease is reduced activity of the cysteine protease cathepsin K. A 48-year-old woman with a history of frequent fractures presented with a severe gait disturbance. Radiography, computed tomography, magnetic resonance imaging, and gene analysis were performed. Physical examination revealed open fontanels, and radiographs showed i

Chondrogenic and fibrotic process in the ligamentum flavum of patients with lumbar spinal canal stenosis.

STUDY DESIGN: A histological, biological, and immunohisto-chemical study of human lumbar ligamentum flavum. OBJECTIVE: To analyze changes in the hypertrophied ligamentum flavum and clarify their etiology. SUMMARY OF BACKGROUND DATA: Hypertrophy of the ligamentum flavum has been considered a major contributor to the development of lumbar spinal canal stenosis (LSCS). Although previous studies have reported some factors related to ligamentum flavum hypertrophy, its etiology is still unclear. METHODS: Ligamentum flavum samples were collected from 20 patients with LSCS (LSCS group) and 10 patie

Geometrical dimensions and morphological study of the lumbar spinal canal in the normal Egyptian population.

Information regarding the precise dimensions of the lumbar vertebrae is essential for spinal surgery and instrumentation. When stenosis of the vertebral canal or the intervertebral foramen exists, the neural structures in them can be affected and cause symptoms such as low back or radicular pain. Accurate and comprehensive spinal canal measurements in the lumbar vertebrae are incomplete. The purpose of this study was to collect data on the dimensions of the lumbar spinal canal from computed tomography scans. Three hundred patients (162 men and 138 women) were studied. Computed tomography scans

Tapering of the cervical spinal canal in patients with scoliosis with and without the Chiari I malformation.

BACKGROUND: Cervical spinal canal tapering may increase CSF velocities and pressures. One report suggests that the cervical spinal canal tapers more steeply in patients with Chiari I than in healthy subjects. The goal of this study was to test the conclusion by measuring spinal canal tapering in another cohort of patients. MATERIALS AND METHODS: Consecutive patients with scoliosis and MR imaging were selected. The MR images were evaluated for tonsillar herniation and syringomyelia. On a midline T2-weighted MR image, the anteroposterior diameter of the spinal canal was measured at each cervica

Unusual case of bifocal leptomeningeal melanocytoma in the posterior fossa with seeding in the spinal canal.

A 26-year-old man presented with signs of raised intracranial pressure. CT and MRI of the head demonstrated two separate lesions in the posterior fossa. The radiological differential diagnoses included multiple meningiomas, schwannomas, neurofibromas and subependymomas. Both lesions were surgically resected. Histopathological examination revealed localisations of a leptomeningeal melanocytoma. Leptomeningeal melanocytoma is a rare tumour of the central nervous system. Generally, it has a good prognosis if radical resection can be performed. In cases of subtotal resection, adjuvant radiotherapy

Magnetic stimulation of the cauda equina in the spinal canal with a flat, large round coil.

Magnetic round coil stimulation over the spinal enlargement activates the spinal nerves at the neuro-foramina level. However, activation of the cauda equina in the spinal canal has never been described in the literature. This study, for which 40 healthy subjects were recruited, activated the cauda equina using a round 20-cm-diameter coil designated as a Magnetic Augmented Translumbosacral Stimulation (MATS) coil. Magnetic stimulation placing the edge of the coil over the L1 and L3 spinous processes elicited compound muscle action potentials (CMAPs) from the abductor hallucis muscle. The CMAPs
