Autonomic hyperreflexia with spinal cord injury.

Autonomic hyperreflexia occurs in up to 85 percent of individuals with spinal cord injuries above the major splanchnic sympathetic outflow. In such cases, paroxysmal reflex sympathetic activity develops in response to noxious stimuli below the level of the neurologic lesion. The clinical features of autonomic hyperreflexia are due largely to reflex sympathetic adrenergic and cholinergic discharges with dysfunctional supraspinal regulatory control. Cephalgia, diaphoresis, flushing, tachycardia or bradycardia, and paroxysmal hypertension are most commonly observed. Although a variety of stimuli …

Spinal cord injury: preventing secondary injury.

Spinal cord injury is devastating to the victim, as well as being costly in terms of medical expenses, lost wages, and lost independence. The initial damage to the spinal cord results from several mechanisms of injury--flexion, extension, compression, penetration, rotation, and the disease process. When the spinal cord is injured and there is necrosis of the nervous tissue, no regeneration of that tissue occurs. Unlike in the peripheral nervous system, where regeneration is possible, the spinal cord is part of the central nervous system, as is the brain. The spinal cord extends from the base o…

Medical complications of spinal cord disease.

Spinal cord injury increases the risk of many life-threatening medical problems, including respiratory failure, pulmonary embolism, and renal failure. Respiratory failure results from paralysis of muscles of inspiration (which impairs oxygen transport to alveoli) and of expiration (which impairs cough and predisposes to pneumonia and atelectasis). Respiratory failure in patients with spinal cord injury can be prevented by proper positioning of the patient, training of ventilatory muscles, pulmonary toilet, and aggressive use of antibiotics and bronchodilators. When respiratory failure occurs, …

Nutrition in acute spinal cord injury.

Nutrition in acute spinal cord injury is complicated. Not every aspect of nutrition as it relates to the acutely injured spinal cord patient is known. The stress response to injury, fever, infection, sepsis, and surgery alter nutritional needs, as does the spinal cord injury itself. The sequelae of spinal cord injury, including denervation atrophy and paralysis, glucose intolerance, skin and wound breakdown, poikilothermy, anemia, respiratory paralysis, pneumonia, paralytic ileus, gastrointestinal ulcers and hemorrhage, neurogenic bowel and bladder, and depression, all affect the nutritional n…

Spinal cord involvement in encephalocraniocutaneous lipomatosis.

Encephalocraniocutaneous lipomatosis is a rare hamartomatosis involving the craniofacial region and the central nervous system. The most prominent clinical features are large areas of scalp alopecia, soft subcutaneous craniofacial masses, lipomas, connective tissue nevi of the eyelids and surrounding areas, pterygium-like choriostoma of the ocular conjunctiva, mental retardation, motor deficit, and seizures. Of the eight patients reported previously, three had spinal cord evaluations and two had evidence of lipomatosis. We report the third patient with this association, review the literature o…

Spinal cord stimulation revisited: psychological effects.

97 patients undergoing spinal cord stimulation (SCS) were studied for a median of 24 days after initiation of treatment. SCS was associated with significant improvement in memory, visuo-spatial integration, activation, cognitive efficiency, attention and overall psychological status. Significant emotional improvement in self-image, anxiety and depression was also confirmed following SCS. Diagnosis did not differentially affect psychological outcome except for relief of anxiety, which was less pronounced in the case of torticollis and multiple sclerosis. The role of spinal cord modulation of be…

Spinal cord lesions in heat stroke.

A clinical and pathological report of a patient who died 15 days after suffering a classic heat stroke is presented. The clinical picture was of a flaccid quadriplegia with anhidrosis and sphincter disturbance. The most severe pathological lesions were to be seen in the anterior and intermedio-lateral horns of the spinal cord. Hyperthermia was the only physiopathological mechanism demonstrated in the patient. It is suggested that the motor neurons and vegetative neurons of the spinal cord are specially sensitive to hyperthermia.

II. Coping with spinal cord injury.

Paraplegia creates social, sexual and emotional problems, and the impact of these was investigated in an interview study of 22 male and female paraplegics. Of particular concern was the way in which individuals were coping with the difficulties confronting them. People used a number of strategies for dealing with other people and their attitudes, and for readjusting sexually. In dealing with their own personal feelings about their disability, ways of coping were varied and included suppression, denial and repression, resignation and acceptance, positive thinking and independence and assertiven

Osteomyelitis in spinal cord injured people.

Prospective evaluation of 285 spinal cord injured patients at the Houston VAMC over a consecutive 44-month period identified 60 cases of osteomyelitis: 41 cases of infected bone beneath pressure sores, 8 bone infections around the site of insertion of metal fixation devices, 5 cases of vertebral osteomyelitis, and 6 other causes and types of bone infection. As reported before, significant problems with the lack of specificity of roentgenograms, bone scans, and gallium scans were encountered; over half the patients without osteomyelitis on bone biopsy (with multiple biopsy specimens) had abnorm…

[Progressive course of spinal cord tumors].

The main clinical varieties of spinal cord and equine tail tumors are reviewed. Of 221 cases, the progressive course of the disease was recorded in 76,9%, slow-progressive course was observed in 68,1% and rapid-progressive in 8,8% of cases. It is stressed that in the presence of the progressive course of the disease there may occur different manifestations of the tumorous process of the spinal cord, the examination of which makes it possible to improve the diagnosis, particularly in the early stage of the spinal oncological process.

Spinal cord compression in mycosis fungoides.

Symptomatic involvement of the central nervous system (CNS) with mycosis fungoides is rare, and usually characterized by involvement of meninges. We describe a patient with long-standing mycosis fungoides who developed acute spinal cord compression. Since tumor-related spinal cord compression requires early intervention for a successful outcome, it should be recognized as an additional form of CNS mycosis fungoides.
