Megacolon in patients with chronic spinal cord injury.

PURPOSE: To investigate the clinical and functional correlates of megacolon in individuals with chronic spinal cord injury (SCI). PATIENTS AND METHODS: This is a cross-sectional study of 128 patients consecutively admitted to a SCI in-patient service in a US Veterans Administration Medical Centre (mean age 57+/-15 years, mean years since injury 20+/-13, 97% male) who underwent plain abdominal radiography for study purposes. Participants were characterised by radiological findings. 'Megacolon' was defined as colonic dilatation of >6 cms in one or more colonic segment(s). Clinical, functional,

Symptomatic gallstones in patients with spinal cord injury.

Patients with spinal cord injury (SCI) have an increased prevalence of cholelithiasis. The goal of this study was to clarify the presentation and management of symptomatic gallstone disease in patients with SCI. We performed a retrospective study of presentation of gallstone complications in patients with SCI who underwent cholecystectomy for complications of gallstone disease. The West Roxbury Veterans Administration Medical Center SCI registry (605 patients) was searched for patients who had undergone cholecystectomy more than 1 year after SCI (35 patients). Gallbladder disease profiles for

Spasticity in rats with sacral spinal cord injury.

We have investigated sacral spinal cord lesions in rats with the goal of developing a rat model of muscular spasticity that is minimally disruptive, not interfering with bladder, bowel, or hindlimb locomotor function. Spinal transections were made at the S2 sacral level and, thus, only affected the tail musculature. After spinal transection, the muscles of the tail were inactive for 2 weeks. Following this initial period, hypertonia, hyperreflexia, and clonus developed in the tail, and grew more pronounced with time. These changes were assessed in the awake rat, since the tail is readily acces

[Traumatic ascending ischemic injury of the spinal cord].

We report five cases that the paraplegic level showed ascending from the segment of spinal injury upward to the higher level. In T10-11 fracture-dislocation, the paraplegic level ascended gradually to C2-3 in case 1 within 10 days and death due to breathing paralysis. In case 2, the paraplegic level ascended to cervical cord with loss of strength in both arms within 14 days. The remaining 3 cases were Ti2(in 2 cases) and L3(1 case) fracture. Their paraplegic level ascended to T8(2 cases) and T9(1 case). There were lower motor neuron paralysis in all cases. Observation on spinal cord specimen o

Bladder neck dysynergia in spinal cord injury.

Urodynamic evaluations were performed in 43 male patients with spinal cord injuries, before any therapeutic decisions, and a minimum of 5 mo following the injury. Results were subdivided according to level of injury. Mean detrusor contraction pressures, incidence of detrusor-sphincter dysynergia (DSD), and incidence of detrusor-bladder neck dysynergia (DBND) were calculated. The incidence pattern of DBND was found to follow closely the incidence pattern of DSD, with the highest incidence among the upper thoracic injuries, considerably more than among the cervical injuries. A significantly high…

Omental transposition in chronic spinal cord injury.

The results of omental transposition in chronic spinal cord injury have been reported in 160 patients operated upon in the United States, Great Britain, China, Japan, India and Mexico, with detailed outcomes reported in few studies. Recovery of function to a greater degree than expected by natural history has been reported. In this series, 15 patients with chronic traumatic spinal cord injury (> 1.5 years from injury) underwent transposition of pedicled omentum to the area of the spinal cord injury. Of the first series of four patients who were operated upon in 1988, one died, one was lost to

Pyrexia in hospitalised spinal cord injury patients.

To discover the frequency of pyrexia in spinal cord injury (SCI) patients in the Hospital for Medicine of the Locomotor System (HMLS/SARAH), a review study was undertaken. For a period of 3 months, all new episodes of fever were recorded. During the study 221.3 patients/month were observed. The age ranged from 14 to 60 years and time of lesion from 1 to 6,264 days: 85 patients were paraplegia or paraparetic, and 44 tetraplegic or tetraparetic. At the beginning of the study 10.8% of the patients had fever. During follow up, we observed a rate of 33.9 new incidences per 100 patients/month. In 77

Spinal cord injury due to suicide attempts.

During 1965 to 1987 45 patients were admitted to the Rehabilitation Hospital in Hornbaek, Denmark because of spinal cord injury (SCI) due to suicide attempts. The proportion of traumatic SCI due to this cause increased significantly during the period: 1965-74: 3.5%, 1975-84: 8%, 1985-87: 13%. The median age at injury was 31 years, and the female to male ratio 1.6. In 38 instances (84%), SCI was caused by jumps from buildings. The neurological level of the lesion was: 7 cervical, 8 thoracic, 22 lumbar; the remaining 8 had minor spinal cord/cauda equina lesions. At the time of the SCI 62% were u

Bladder training in patients with spinal cord injury.

Immediately following severe injury to the spinal cord or conus medullaris, there is a stage of flaccid paralysis of the bladder. The smooth muscle of the detrusor and rectum is affected. Drainage of the bladder is very important in the early care of such patients. From August 1989 to August 1990, 55 spinal cord injury patients were studied on admission to our department. The current bladder training method used for these patients is presented. A patient was deemed to have been successfully bladder trained when catheter-free, continent, and able to consistently maintain a residual volume of 10

Spinal cord injury: a ten-year report.

In the past 10 years, the RSCICDV has had a unique opportunity to serve and expand the bounds of knowledge regarding this most devastating injury. The RSCICDV has collaborated with other model SCI systems in research regarding the incidence of respiratory complications, the value of removing bullet fragments lodged within the spinal canal, the survival/cause of death following spinal cord injury, the cost of spinal cord injury care, and the recovery of motor strength after quadriplegia. Key on-site research efforts have focused on preventing deep vein thrombosis and in documenting the course o

Anemia in acute phase of spinal cord injury.

Anemia is a frequent complication during the acute phase of spinal cord injury (SCI), even in the absence of detectable blood loss. Since an improved understanding of the etiology of anemia in this population is a prerequisite to its ultimate prevention, a prospective study was conducted on 28 persons with SCI whose lesions were between C3 and C7. The injuries were either neurologically complete, incomplete with sensory sparing only, or incomplete with nonfunctional motor preservation. Laboratory profiles were obtained during the first few weeks postinjury. No persons had below-normal plasma v

Colostomy as treatment for complications of spinal cord injury.

We reviewed our experience with colostomy performed in 20 spinal cord injury (SCI) patients to determine the effectiveness and safety of colostomy when it is performed for a late complication of SCI. Objective evaluation of gastrointestinal function, ie, colonic transit time and anorectal manometry, was performed in six patients with chronic gastrointestinal complaints to identify the site and severity of bowel dysfunction. Twelve patients had colostomy performed for chronic gastrointestinal problems, seven as an adjunct in the treatment of perineal pressure ulcers, and one for rectal cancer.

Bladder hypocompliance in the spinal cord injury population.

To determine the incidence and effects of low bladder compliance on the upper urinary tracts in spinal cord injury patients, we evaluated the last 254 patients during a 3-year period who underwent a complete urodynamic study, along with an excretory urogram and/or renal ultrasound. Compliance was determined at 100 cc volume. A compliance number of 20 or less was considered low. Of the 254 patients 43 (17 per cent) had low compliant bladders by our definition. Hydronephrosis was present in 56 of the 84 renal units (64 per cent) and reflux was present in 39 (46 per cent). These results are in sh

Anemia in patients with traumatic spinal cord injury.

Anemia is often a complication following spinal cord injury which interferes with the patient's rehabilitation. Hematological profiles of 65 male patients with traumatic spinal cord injury revealed a 52.3% incidence of mild anemia. The age, duration or level of injury had no correlation with the incidence and type of anemia. The anemia was normocytic-normochromic in 32%, normocytic mild hypochromic in 56%, and microcytic hypochromic in 12% of the patients. Factors in pathogenesis included decubitus ulcers, urinary tract infections, acute and chronic blood loss, and folic acid deficiency due to

Fever and infection in spinal cord injury patients.

Fever, infection, or both occurred at some time during hospitalization in 67% of patients on referral spinal cord injury service. A single cause of fever was generally found, with urinary tract and soft-tissue infections predominating; these responded well to appropriate management. In contrast, spinal cord injury patients on other services seen in consultation by the infectious disease service had far more complicated and advanced disease. Multiple infections were diagnosed in the majority of these patients, with urinary tract and decubitus ulcers predominating. Complicating osteomyelitis was

Evaluation of the urethra in males with spinal cord injury.

Of 358 antegrade voiding urethrograms obtained in 200 males with spinal cord injury, the urethra was satisfactorily shown by 154 (77%), permitting evaluation of alterations related to cord damage, surgery, or iatrogenic trauma. In 105 cases the neurogenic urethra was normal, demonstrated only minor distension, or had a patulous appearance, in contrast to urinary tract changes such as a markedly neurogenic bladder, vesicoureteral reflux, and bladder calculi. In 49 patients, abnormalities included (a) changes related to surgical alleviation of sphincter dyssynergia; (b) iatrogenic changes such a

The pituitary-gonadal axis in spinal cord injury.

Our study documented that serum levels of luteinizing hormone, follicle-stimulating hormone, testosterone, and estradiol in 14 of 15 males with spinal cord injury (SCI) were within the normal range of those of age-matched controls. One patient had gynecomastia with compensated gonadal insufficiency of undetermined etiology. Five males with SCI showed a normal rise in serum testosterone levels following stimulation with human chorionic gonadotropin. Testicular biopsy in one of two males with SCI showed spermatogenic arrest. One of the biopsies showed immunoglobulin G immunofluorescence around t
