Atypical cervical spondylotic myelopathy mimicking intramedullary tumor.

STUDY DESIGN: Case report and a review of the literature. OBJECTIVE: We report the case of a young man with a short course of progressive cervical myelopathy (CM). Cervical magnetic resonance imaging (MRI) revealed a stenosis of the cervical spinal canal at C4-C6 and an atypically enlarged intramedullary high intensity extending from C1-T1 (T2-weighted) with contrast enhancement at C4-C5 (T1-weighted). Neurologic and radiologic diagnosis therefore favored a tumor of the spinal cord. SUMMARY OF BACKGROUND DATA: CM is a clinical diagnosis of mostly degenerative origin in older patients that fe

Recurrence of solitary extramedullary plasmacytoma affecting both optic nerves: a case report.

PURPOSE: To present a case of atypically located plasmacytoma recurrence with bilateral involvement of the optic nerves. OBSERVATIONS: A 73-year-old man with diabetes, essential hypertension, and primary hypercholesterolemia presented to our clinic with subjective visual loss in his left eye. The patient had a previous history of solitary extramedullary plasmacytoma in the spinal cord, which was treated with resection surgery at the thoracic T6-T12 levels and curative radiotherapy two years prior. The ophthalmological examination indicated best-corrected visual acuity of 20/50 (0.4 logMAR) in

Solitary Cervical Plasmacytoma Presenting as Acute Quadriparesis Following a Recent Cerebral Infarct: A Case Report.

Solitary plasmacytoma of bone (SPB) is a rare plasma-cell neoplasm that can cause acute neurological compromise, especially when involving the cervical spine. We report a 59-year-old male with a recent right hemispheric hemorrhagic infarct who developed rapidly progressive quadriparesis and urinary retention 15 days later. Neurological examination showed spastic quadriparesis with a C5 sensory level. MRI and CT revealed a pathological C4-C5 wedge compression fracture with cord compression. Emergency anterior C4-C5 corpectomy with fusion was performed. Intraoperative tissue culture grew Staphyl

Role of Immunohistochemistry in the Identification of Supratentorial C11ORF95-RELA Fused Ependymoma in Routine Neuropathology.

Ependymomas (EPs) are tumors of the brain and spinal cord constituting ∼10% of the childhood central nervous system neoplasms and about 30% in children aged <3 years. Their anatomic distribution varies according to the age, with those arising in the supratentorial (ST) compartment, spinal cord being more common in older children and adults, and those at the infratentorial location are more common and occurring more frequently in infants and children. Recently, molecular classification of EP subgroups has been proposed and a supratentorial ependymoma subgroup characterized by RELA-fusion genes

The evolution of spinal cord surgery: history, people, instruments, and results.

INTRODUCTION: Spinal cord surgery has and always will be a challenging operation with satisfying results, but also with potentially devastating results. Over the last century, there has been an evolution in the way we perceive and conduct spinal cord surgery. The phenomenal evolution in technology from the very first x-ray pictures helps to localize the spinal pathology through the use of high-resolution MRI and ultrasonography that allows for high precision surgery with relatively minimal exposure. METHODS: The advancements in the surgical technique and the utilization of neuromonitoring all

Surgery for spina bifida occulta: spinal lipoma and tethered spinal cord.

The technical evolution of the surgery for spina bifida occulta (SBO) over the course of a half-century was reviewed with special foci placed on the spinal lipoma and tethered spinal cord. Looking back through history, SBO had been included in spina bifida (SB). Since the first surgery for spinal lipoma in the mid-nineteenth century, SBO has come to be recognized as an independent pathology in the early twentieth century. A half-century ago, the only option available for SB diagnosis was the plain X-ray, and pioneers of the time persevered in the field of surgery. The classification of spinal

Craniocervical junction intradural extramedullary meningioma with cord compression.

Craniocervical spine meningiomas are rare. They often present with non-specific motor or sensory symptoms. Presenting symptoms can include gait ataxia, radiculopathy, myelopathy, back pain and sensory deficits. Spinal meningiomas are slow-growing tumours, with an insidious onset. Due to the critical location of craniocervical meningiomas, severe symptoms such as respiratory distress and quadriparesis are possible. We describe the clinical presentation of a craniocervical junction meningioma, its relevant neuroimaging findings, diagnostic challenges and management. A woman in her 30s presented

How I do it: resection of spinal cord cavernous malformation.

BACKGROUND: Spinal cord cavernous malformations (CMs) account for 5 to 12% of all spinal vascular malformations. Surgical removal is indicated in symptomatic patients, especially if the CM comes close to the dorsal or lateral surface of the spinal cord. Spinal cord CMs pose critical challenges for the potential of severe disabling complications. METHOD: We described the step-by-step surgical principles of spinal cord CM microsurgical resection. The main surgical steps are also illustrated in an accompanying operative video. CONCLUSION: Pre-surgical planning of the basic steps and good contin

Unusual presentation of a giant thoracic spinal cord lipoma.

INTRODUCTION: Non-dysraphic intradural spinal cord lipomas are rare, and true intramedullary cervical-thoracic lipomas are extremely rare. Spinal lipomas usually present with chronic, progressive myelopathic features. Unlike dysraphic lipomas, which are usually located in the lumbo-sacral region, non-dysraphic lipomas are usually located in the cervical or thoracic spine. CASE PRESENTATION: We present an unusual case of a 21-year-old female who presented with four months of severe back pain, progressive spasticity, and weakness in the lower limbs. Magnetic resonance imaging (MRI) revealed a T

Teaching neuroimages: neuromyelitis optica misdiagnosed as spinal cord tumor.

A 17-year-old girl presented with acute-onset cervical pain, followed by left arm weakness and gait disturbances. Spinal cord astrocytoma was diagnosed by MRI performed at an outpatient facility (figure, A and B). The patient was admitted to the neurosurgery department to undergo spinal cord biopsy. A second neurologic evaluation indicated neuromyelitis optica (NMO) as the most likely diagnosis, which was confirmed by NMO-immunoglobulin G seropositivity. The patient was treated with rescue plasmapheresis with substantial clinical and radiologic (figure, C) improvement.

[Pre-sacral epidermoid cyst communicating with spinal CSF cavity (author's transl)].

A 6-year-old girl was transfered to our hospital because of continuing pleocytosis, and detection of the squamous cells and cholesterin crystals in the CSF. Neurological examination on admission revealed no remarkable disorders, except for weakness of anal sphincter. X-ray examination of the spine showed a deformity of the sacral vertebra. Myelography revealed a pre-sacral cystic tumor, which was connected with spinal CSF cavity at cul-de-sac of the spinal canal by a narrow fistula. The tumor was totally removed and no meningitic sign appeared in postoperative course. Histological examination

Pediatric Spinal Solitary Fibrous Tumor: A Systematic Review of a Rare Condition.

Background: Spinal solitary fibrous tumors (SFTs) are a rare oncological entity, almost anecdotal in the pediatric population. They have a high relapse rate and represent an ongoing oncological challenge. Methods: In this article, we conducted a systematic review starting from a case report to highlight the current state of the art in managing these tumors. Results: Spinal solitary fibrous tumors (SFTs) are rare, slow-growing neoplasms that can be either intra- or extramedullary. Only a limited number of studies focus on primary pediatric spinal cord localization. Five pediatric cases of spina

Incidental Primary Intradural Carcinoid Tumor in a Patient with Lumbar Radiculopathy.

BACKGROUND: Carcinoid tumors are neuroendocrine neoplasms derived from enterochromaffin cells. Central nervous system involvement is rare. The reported cases include metastases to brain or spinal cord, and primary skeletal or extradural disease resulting in compressive myeloradiculopathy. There are 2 previously reported cases of primary intradural extramedullary carcinoid tumor, presenting with compressive symptoms. We report the first case of primary incidental intradural extramedullary carcinoid tumor. CASE DESCRIPTION: A 68-year-old man was complaining of a 2-month history of left-sided ba

35 year-old man with falcine tumor.

A 35-year-old man presented with one month history of vomitus, dizziness and headache. CT and MR imaging revealed a 3.5 x 3.2 cm solitary extra-axial midline mass arising from the frontal falx cerebri; radiological findings were diagnostic of meningioma of the falx. At surgery, the tumour appeared as an extra-axial lesion and was removed via a left midline frontal craniotomy. Macroscopically, the surgical specimen was whitish, soft, well circumscribed and measured 1.6 cm in diameter; microscopic features showed a neoplasm with high cellularity, presence of mitotic figures, without necrosis or

Clear cell meningioma of the filum terminale in a 44-year-old woman: case report.

Clear cell meningioma (CCM) is a rare subtype of meningioma, especially unusual as a neoplasm of the filum terminale. Clear cell meningioma seems to have a more aggressive nature and a higher risk of recurrence than WHO grade I meningiomas. A 44-year-old woman presented with lower back pain radiating to the left leg and mild weakness in the left leg. Magnetic resonance imaging (MRI) showed a well-demarcated, intradural lesion filling the spinal canal at the L3-S1 levels and compressing the cauda equina. The patient underwent laminectomy from L3 to S1. During the operation, the filum terminale
