Agrypnia excitata as the main feature in anti-leucine-rich glioma-inactivated 1 encephalitis: a detailed clinical and polysomnographic semiological analysis.

BACKGROUND AND PURPOSE: The core manifestations of leucine-rich glioma-inactivated 1 (LGI1) autoantibody-mediated encephalitis are limbic encephalitis and faciobrachial dystonic seizures. Agrypnia excitata (AE) is a rare syndrome characterized by sleep-wake cycle disruption, autonomic hyperactivation and episodes of oneiric stupor. Only a few diseases are known to present with AE. An autoimmune etiology must be considered when accompanied by neuromyotonia. A case of anti-LGI1 encephalitis presenting with AE is reported. METHODS: Detailed clinical, video-polysomnographic, laboratory, radiologi

Neuronal Glutamate Transporters Control Dopaminergic Signaling and Compulsive Behaviors.

There is an ongoing debate on the contribution of the neuronal glutamate transporter EAAC1 to the onset of compulsive behaviors. Here, we used behavioral, electrophysiological, molecular, and viral approaches in male and female mice to identify the molecular and cellular mechanisms by which EAAC1 controls the execution of repeated motor behaviors. Our findings show that, in the striatum, a brain region implicated with movement execution, EAAC1 limits group I metabotropic glutamate receptor (mGluRI) activation, facilitates D1 dopamine receptor (D1R) expression, and ensures long-term synaptic pl

Calibration of the Leg Muscle Responses Elicited by Predictable Perturbations of Stance and the Effect of Vision.

Motor adaptation due to task practice implies a gradual shift from deliberate control of behavior to automatic processing, which is less resource- and effort-demanding. This is true both for deliberate aiming movements and for more stereotyped movements such as locomotion and equilibrium maintenance. Balance control under persisting critical conditions would require large conscious and motor effort in the absence of gradual modification of the behavior. We defined time-course of kinematic and muscle features of the process of adaptation to repeated, predictable perturbations of balance eliciti

Development and Implementation of a Brain Rest Protocol in Nonintellectually Challenged Youth With Head-Banging Behaviors.

INTRODUCTION: Concussions as a result of mild traumatic brain injury (MTBI) in youth are often associated with athletic injury; however, they can also occur as a result of intentional self-injury by head banging. Despite the known neurological sequelae secondary to MTBI, assessment, interventions, and consequences of head banging in the nonintellectually disabled populations have not been well studied. AIMS: The intent of this nurse-led intervention was to develop a brain rest protocol at a residential treatment program serving youth who were between 12 and 19 years old in order to improve the

Motor control, habits, complex motor stereotypies, and Tourette syndrome.

Tourette syndrome (TS) and primary complex motor stereotypies (CMS) are two relatively common, distinctly different movement disorders of childhood. Despite their frequency, the precise underlying pathophysiological mechanism(s) for tics and stereotypies remains unknown. Both are likely to involve cortical-striatal-thalamo-cortical (CSTC) pathways or their interconnecting brain regions. In recent studies, distinct, separate cortical-striatal pathways have been identified for goal-directed and habitual behavioral activity with important influences from structures, such as the hippocampus, amygd

Children with autism demonstrate circumscribed attention during passive viewing of complex social and nonsocial picture arrays.

Although circumscribed interests are a hallmark characteristic of autism spectrum disorders, providing a means for quantifying their functional impairment has proven difficult. We developed a passive viewing task to measure aspects of visual attention in children with autism spectrum disorders and typically developing controls. Task stimuli included picture arrays that were matched for social and nonsocial content. Nonsocial content was balanced to include items related to circumscribed interests (e.g., trains) as well as more commonplace items (e.g., furniture). Discrete aspects of gaze behav

Variations of stereotypies in individuals with Rett syndrome: A nationwide cross-sectional study in Taiwan.

Individuals with Rett syndrome (RTT) can have variable manifestations of stereotypies. In this nation-wide cross-sectional study, we recruited all individuals with RTT in Taiwan diagnosed as RTT by neurologists based on genetic findings and diagnostic criteria. The data were collected using questionnaire. A total 43 cases of typical RTT and 15 cases of atypical RTT, aged from 2.1 to 40.1 years, were enrolled. They included 3 (5.2%) in stage II, 42 (72.4%) in stage III, and 13 (22.4%) in stage IV. All individuals presented with at least one stereotypy. Individuals with atypical RTT had more var

The hands, and the mind, pre- and post-regression, in Rett syndrome.

Forty girls with Rett syndrome were included in a study of behaviour, with particular regard to the hands, before, during and after regression. Data was taken from examination of each girl, and in some cases from detailed developmental histories given by parents, and films taken before, during and after regression. The important findings are shown in two tables and described. Pre-regression abnormalities included hypotonia, jerky incoordination, an excess of patting or waving activity and involuntary movements which included alternate opening and closing of the fingers and twisting of wrists o

Effects of Anxiety on Spontaneous Ritualized Behavior.

Environmental uncertainty and uncontrollability cause psycho-physiological distress to organisms, often impeding normal functioning. A common response involves ritualization, that is, the limitation of behavioral expressions to predictable stereotypic and repetitive motor patterns. In humans, such behaviors are also symptomatic of psychopathologies like obsessive-compulsive disorder (OCD) and autism spectrum disorders (ASDs). Although these reactions might be mediated by different neural pathways, they serve to regain a sense of control over an uncertain situation by engaging in behavioral pat

Inchworming: a novel motor stereotypy in the BTBR T+ Itpr3tf/J mouse model of autism.

Autism Spectrum Disorder (ASD) is a behaviorally defined neurodevelopmental disorder characterized by decreased reciprocal social interaction, abnormal communication, and repetitive behaviors with restricted interest. As diagnosis is based on clinical criteria, any potentially relevant rodent models of this heterogeneous disorder should ideally recapitulate these diverse behavioral traits. The BTBR T+ Itpr3tf/J (BTBR) mouse is an established animal model of ASD, displaying repetitive behaviors such as increased grooming, as well as cognitive inflexibility. With respect to social interaction an

Exome sequencing analysis in a pair of monozygotic twins re-evaluates the genetics behind their intellectual disability and reveals a CHD2 mutation.

BACKGROUND: Neurodevelopmental disorders include a broad spectrum of conditions, which are characterized by delayed motor and/or cognitive milestones and by a variable range of intellectual disability with or without an autistic behavior. Several genetic factors have been implicated in intellectual disability onset and exome sequencing studies have recently identified new inherited or de novo mutations in patients with neurodevelopmental disorders. CASE: We report the case of two monozygotic twins who came for the first time to our attention at the age of 20months for a global neurodevelopmen

Repetitive behaviors in chronic schizophrenia: using the Japanese version of the Elgin Behavior Rating Scale (JEBRS).

The main aim of this study was to test the reliability and value of the Japanese Elgin Behavior Rating Scale (JEBRS) with 92 schizophrenia patients. Cronbach's alpha for the JEBRS was 0.61. All inter-rater reliability coefficients were in the satisfactory range. The JEBRS had a high stability over time. Almost all of the correlations among the nine item scores and total score in the JEBRS were significant. The total score for nine repetitive behaviors in the JEBRS was positively correlated with positive and negative symptoms. This study showed that the JEBRS is a useful and reliable scale.

Description of Motor Stereotypies in Adolescents and Adults With Autistic Spectrum Disorder.

BACKGROUND: Motor stereotypies (MS) are highly prevalent in children with autism spectrum disorders (ASD) and, although they tend to decrease with age, may persist into adulthood. The primary objective of this study was to describe the frequency, severity, number, and types of MS in adolescents and adults with ASD, to retrospectively evaluate their evolution over time, as well as to examine their relationship with sociodemographic and clinical variables. METHODS: A sample of 90 adolescents and adults with ASD were included in a cross-sectional and retrospective study. Rojahn's Stereotypic Beh

Health-related physical fitness management for a child with Tourette syndrome.

Tourette syndrome (TS) is a neurobiological disorder characterized by tics, which are repetitive, stereotypical, involuntary movements and vocalizations. Although the causes of TS are not completely understood, previous studies indicated that many children with TS not only experience comorbid conditions such as a lack of concentration, hyperactivity, impulsive behavior, and obsessive-compulsive symptoms, but also demonstrate poorer motor skills than their peers with typical development. There is limited information on health-related fitness exercises and/or physical therapy for children with T

[Clinical study of four cases of Rett Syndrome].

Rett Syndrome was first described in 1966 by Andreas Rett. The disorder is characterized by a progressive loss of cognitive and motor skills as well as development o stereotyped hand movements, occurring after an apparently normal development. Authors present three typical cases, and, another one atypical, being all of them female. This study takes into account ten different areas about chronology, age and reasons in the first consultation, some milestones of psychomotor development, the diagnostic criteria--according to Rett Syndrome Diagnostic Criteria Work Group (R.S.D.C.W.G.), they consist

Rett syndrome.

PURPOSE OF REVIEW: Nearly 70 reports on Rett syndrome were published in 2004. We have selected 51 articles, including clinical reports, on pathophysiology, genotype-phenotype correlation, and clinical and basic molecular biology studies. These articles explain how mutation of the gene (MECP2) for methyl-CpG-binding protein 2 causes the particular disorders of Rett syndrome, and also induces other neurodevelopmental disorders, clarifying the situation for future studies. RECENT FINDINGS: The role of X-chromosome inactivation has been clarified in animal experiments. New isoforms of MeCP2 have

Beyond the Stethoscope: Robotic Empathy in Pediatric Cardiology. The Emerging Role of Social Robots. A Pilot Study.

Children diagnosed with autism spectrum disorder (ASD) often present with restricted interests, stereotyped behaviors, and differences in social communication that can increase distress during clinical encounters. These characteristics may exacerbate anxiety and oppositional behaviours in medical settings. Conversely, the predictable and repetitive way social robots operate appears to align with the cognitive style exhibited by many people with autism and may offer opportunities to reduce situational stress. This pilot study investigated the potential of a humanoid social robot (NAO) to improv
