A 5-year-old girl had hypotony, myopia, and anterior displacement of the lens-iris diaphragm following routine strabismus surgery. This constellation of findings suggests that surgical manipulation of the intact sclera produced a limited effusion involving the supraciliary space. Following cycloplegia, these findings resolved over several weeks, suggesting that this rare complications is self-limited and associated with a good prognosis for recovery.

Necrotizing scleritis may occur following ocular surgery, most commonly after cataract extraction. This complication developed in a 60-year-old woman following strabismus surgery for a gaze palsy and sixth-nerve paresis following a stroke. Although an autoimmune process is present in many patients with necrotizing scleritis, none was detected in this patient. Necrotizing scleritis with inflammation of the right eye developed after bilateral eye muscle surgery for thyroid ophthalmopathy. Debilitating pain, delay in onset, and involvement of the sclera distinguish this condition from anterior segment ischemia. The surgery may have acted as a nonspecific trigger in an eye at risk for scleritis. Necrotizing scleritis has occurred infrequently after other types of eye surgery but, to our knowledge, has not been previously reported as a complication of eye muscle surgery.

PURPOSE: To investigate changes in accommodation amplitude (AA) after strabismus surgery.
METHODS: The medical records of patients who underwent full-tendon tenotomy on at least one rectus muscle between January 2023 and July 2023 were reviewed retrospectively. We queried registered strabismus trials that were completed prior to January 1, 2021, from ClinicalTrials.gov. The paradigm that an "underacting" extraocular muscle is always atrophic or hypoplastic and that an overacting extraocular muscle should always be enlarged leads to inconsistencies with clinical observations. BACKGROUND: About 2% of the patients with an early-onset esotropia reveal a severe head tilt, of which about 80% tilt their head towards the side of the dominant eye. This is an abnormal posture without a profit for the patient, needing a torsional shift operation. A minority (20%) tilt to the side of the non-dominant eye, while presenting with a severe and unilaterally pronounced DVD. Once the DVD is really bilateral in terms of clinical aspects, patients have a tendency to give in preferring a consistent upgaze position.

Unusual ocular motility abnormalities have been rarely reported to result from anomalous extraocular structures. These structures, which may be either muscular or fibrotic, attach to the globe and produce a mechanical restriction resulting in incomitant strabismus. To our knowledge, we report the first patient with an anomalous extraocular muscle in whom the clinical, radiologic, surgical, and histopathologic findings are described.
