[A new catheter for hemodynamic studies].

It is reported on a new catheter technique for the determination of the cardiac output by means of the method of thermodilution. It consists of a thin sensorcatheter (3 Ch), which contains a microthermistor as temperature sensor and additionally two electrodes. The insertion of the catheter is carried out via puncture of the subclavian vein by means of a special introducer set, the outer sheath of which can used for injecting the cold solution. The positioning of the sensor catheter into the pulmonary artery may be done by intracardiac ECG monitoring with the help of the two electrodes. By mea

Changes in spleen size in patients with acute ischemic stroke: a pilot observational study.

BACKGROUND: In animal models, the spleen contracts after acute ischemic stroke, followed by release of inflammatory cells leading to secondary brain injury. AIMS: We aim to characterize splenic responses in patients with acute ischemic stroke. METHODS: In this prospective observational study, we measured daily spleen sizes with abdominal ultrasound in 30 patients with suspected acute ischemic stroke. Splenic ultrasounds were also performed in 20 healthy individuals. RESULTS: A generalized estimating equation, longitudinal regression model for adjusted spleen measurements showed the differen

Structural cerebellar reserve positively influences outcome after severe stroke.

The concept of brain reserve capacity positively influencing the process of recovery after stroke has been continuously developed in recent years. Global measures of brain health have been linked with a favourable outcome. Numerous studies have evidenced that the cerebellum is involved in recovery after stroke. However, it remains an open question whether characteristics of cerebellar anatomy, quantified directly after stroke, might have an impact on subsequent outcome after stroke. Thirty-nine first-ever ischaemic non-cerebellar stroke patients underwent MRI brain imaging early after stroke a

Quantitative analysis of the effect of institutional case volume on complications after surgical clipping of unruptured aneurysms.

OBJECTIVE The mechanism by which greater institutional case volume translates into improved outcomes after surgical clipping of unruptured intracranial aneurysms (UIAs) is not well established. The authors thus aimed to assess the effect of case volume on the rate of various types of complications after clipping of UIAs. METHODS Using information on the outcomes of inpatient admissions for surgical clipping of UIAs collected within a national database, the relationship of institutional case volume to the incidence of different types of complications after clipping was investigated. Dementia screening in elderly high-risk patients following heart failure decompensation may predict unfavorable long-term clinical outcomes.

BACKGROUND: Recently heart failure (HF) has been found to be a new dementia risk factor; nevertheless, their relations in patients following HF decompensation remain unknown. We sought to investigate whether a screening diagnosis for dementia (SDD) in this high-risk population may predict unfavorable long-term clinical outcomes. METHODS: One hundred forty-two patients following HF decompensation requiring hospitalization were enrolled. Within a median time of 55 months all patients were screened for dementia with ALFI-MMSE scale whereas their compliance was assessed with the Morisky Medicatio

Hippocampal size and dementia in stroke patients: the Sydney stroke study.

BACKGROUND: Hippocampal atrophy is an early feature of Alzheimer's disease (AD) but it has also been reported in vascular dementia (VaD). It is uncertain whether hippocampal size can help differentiate the two disorders. METHODS: We assessed 90 stroke/TIA patients 3-6 months after the event, and 75 control subjects, with neuropsychological tests, medical and psychiatric examination and brain MRI scans. A diagnosis of VaD, vascular mild cognitive impairment (VaMCI) or no cognitive impairment (NCI) was reached by consensus on agreed criteria. T1-weighted MRI was used to obtain total intracrania

Stroke after transcatheter aortic valve implantation: incidence, temporal trends and predictors.

BACKGROUND: Stroke remains one of the most serious complications of transcatheter aortic valve implantation (TAVI). This study evaluated temporal trends in 30-day stroke after TAVI, explored potential contributors to these trends and examined associations with mortality. METHODS: All patients undergoing TAVI in Sweden between 2008 and 2023 were identified from the SWEDEHEART registry. The primary endpoint was 30-day ischaemic or haemorrhagic stroke. Temporal trends were analysed using Bayesian binomial regression. To investigate potential contributors to the observed trends, sequential models

[Postoperative cardiopulmonary function studies in children and adolescents with pulmonary valvular stenosis (author's transl)].

Cardiopulmonary function studies at rest and during submaximal and maximal exercise were performed in 20 children and adolescents who had undergone valvotomy of a pulmonary valvular stenosis. Maximal oxygen uptake of the patients was 90%, stroke volume 95% of expected values. There was a strong correlation between stroke volume and maximal oxygen uptake. Mean right ventricular systolic pressure at rest was 39 mm Hg and increased to 60 mm Hg at a submaximal load corresponding to 1 1/2 Watt/kg body weight. There was no correlation between right ventricular systolic pressure on the one hand and m…

Evidence-based guideline: The role of diffusion and perfusion MRI for the diagnosis of acute ischemic stroke [RETIRED]: report of the Therapeutics and Technology Assessment Subcommittee of the American Academy of Neurology.

OBJECTIVE: To assess the evidence for the use of diffusion-weighted imaging (DWI) and perfusion-weighted imaging (PWI) in the diagnosis of patients with acute ischemic stroke. METHODS: We systematically analyzed the literature from 1966 to January 2008 to address the diagnostic and prognostic value of DWI and PWI. RESULTS AND RECOMMENDATIONS: DWI is established as useful and should be considered more useful than noncontrast CT for the diagnosis of acute ischemic stroke within 12 hours of symptom onset. DWI should be performed for the most accurate diagnosis of acute ischemic stroke (Level A)

Motor recovery as assessed with isometric finger movements and perfusion magnetic resonance imaging after acute ischemic stroke.

OBJECTIVE: Recovery from hemiparetic stroke is variable. An important goal for clinicians and clinical researchers is to identify predictors of recovery. The initial phase after acute ischemic stroke is considered to be of major importance for neurological outcome. The authors sought to determine in patients with acute ischemic stroke whether early motor recovery, as measured by repetitive isometric index-thumb oppositions, is correlated with ischemic lesion volume. METHODS: Thirty-six acute hemiparetic stroke patients with residual hand function were investigated. The European Stroke Scale (
