Transverse maxillary hypoplasia, in adolescents and adults, is frequently seen as an acquired deformity and in congenital deformities patients and can be corrected by means of surgically assisted rapid maxillary expansion. Traditionally, the distractors for expansion are tooth-borne devices, i.e. hyrax appliances, which may have some serious disadvantages such as tooth tipping, cortical fenestration, skeletal relapse and loss of anchorage. In contrast, with bone-borne distractors most of the maxillary expansion is orthopedic and at a more mechanically desired level with less dental side effect

PURPOSE: Stabilization and replacement of ribs is still a challenge, because most available systems for intramedullary and extramedullary fixation are less than perfect. We present our experience with a modified device, which compensates for several disadvantages in other methods.
DESCRIPTION: Originating from the Strasbourg Thoracic Osteosyntheses System (STRATOS [MedXpert GmbH, Heitersheim, Germany]), the multidirectional thoracic wall stabilization system uses tripodal clips with sharp clasping legs. They can be placed without dissecting the ribs, and bridge fractures or defects with titan

Adolescent tibial tubercle avulsion fracture (TTAF) is a rare knee injury resulting from the combination of anatomical fragility and mechanical stress. This narrative review provides an updated synthesis of current evidence on the epidemiology, imaging-based classification, management strategies, and outcomes of TTAF. In this update, we highlight the role of advanced imaging modalities, such as computed tomography (CT) and magnetic resonance imaging (MRI), in improving visualization of fracture morphology and associated soft tissue injuries; review emerging evidence supporting early range of m

We encountered some complications during the graft passage and fixation using a TransFix implant. Despite the large number of cases and publications related to the ligament surgery, few papers have reported intra-operative complications. We report our experience of such complications and prevention methods. From June 2005 to October 2007, anterior cruciate ligament reconstructions were performed in 128 patients using the TransFix devices. From the surgical records, the opinions of the assistant during the graft passage were divided into three subsets: tight, modest, and loose. The results were

The aim of this study was the analysis of 12 cases showing a L5-S1 spondylolisthesis of a shift superior to 50%. The surgery performed on those 12 children and teenagers was Roy Camille's technical procedure that is reduction and fixation with posterior plates and postero-lateral grafting The criteria of the study and technique used are first recalled. The result's analysis show the quality of the reduction and the restoration of the lumbo-pelvic statics. The postero-lateral arthrodesis appears to the authors as sufficient to guarantee the result's stability. In cases of associated radicular i

OBJECTIVE: To evaluate risk factors for infection in severe open tibial shaft fractures.
METHODS: A secondary analysis of a multicenter prospective study investigated internal versus external fixation of severe open tibia fractures at 20 US Level I trauma centers. Adult patients, aged <65 years, with a Gustilo-Anderson Type IIIB or severe IIIA metaphyseal or diaphyseal tibia fracture were included. All fractures underwent definitive fixation with either a modern ring external fixator, intramedullary device, and/or plate. Fourteen variables previously identified as risk factors for infection w

BACKGROUND: Sigmoid volvulus (SV) is the third leading cause of large bowel obstruction after colonic neoplasms and diverticular disease. SV has traditionally been managed via endoscopic detorsion and decompression followed by non-emergent surgical resection owing to the high risk of recurrence. Most cases of SV occur in elderly debilitated patients with multiple comorbidities and may not be candidates for surgical resection. Endoscopic sigmoidopexy has been described as an alternative to surgical resection. Here, we describe our experience with endoscopic sigmoidopexy using T-fasteners.

A number of approaches have been attempted to address the treatment of acute odontoid fractures, spanning from conservative treatment with an external orthotic device to posterior C1-C2 arthrodesis or anterior screw fixation. The current report describes stabilisation of such fractures with a C1 supralaminar hook and a C2 pars interarticularis screw construct. Two patients with odontoid fracture were treated by this technique. Two-year follow-up of both patients revealed complete union without complications. The authors stress that this technique avoids the hazards of C1 lateral mass screws.

Fracture of the lateral portion of the distal end of the tibia in adults is known as the fracture of Tillaux (Tillaux, 1872). The fracture line extends vertically from the distal articular surface of the tibia upwards to the lateral cortex. The fragment is roughly quadrangular in shape. The analogous epiphyseal injury is less common and two such cases are presented here, both were fixed internally. There now appears to be a consensus of opinion that for fractures of the lateral portion of the distal tibial epiphysis open reduction should be performed, and that closed reduction should be reserv

The effect of external skeletal fixation on blood supply and healing of the tibia was investigated in the New Zealand white rabbit. Bilateral osteotomies were made in ten rabbits. One osteotomy was stabilized with a small unilateral external fixator and the other with a conventional long-leg cast. The animals were killed at various intervals from one to ten weeks. Microsphere infusions indicated a greater blood supply in the cast-treated osteotomies after five weeks. Histologic examination showed that external fixation alters the proportions of periosteal and medullary ossification in the heal

Brachymetatarsia is not an uncommon congenital foot disorder that is due to a premature closure of the ephiphyseal plate. The literature has numerous surgical procedures for the correction of this problem. In recent years, the callus distraction technique described by ilizarov has been utilized for this problem. This is a retrospective study of the use of the callus distraction technique for the correction of congenitally short metatarsal. This series included six feet in six female patients, all with a short fourth metatarsal. Preoperative complaints were metatarsalgia and/or cosmesis. A nonc

BACKGROUND: Clavicle fractures are common injuries. As displaced or multifragmentary midshaft patterns carry a substantial risk of malunion and nonunion with nonoperative treatment, they have contributed to the increasing use of surgical fixation. Modern operative techniques include plate fixation, intramedullary devices, and hybrid strategies to address comminution and improve fragment stability. Supplementary interfragmentary screws and cerclage constructs have been proposed to enhance fragment control and optimize load transfer in multifragmentary fracture patterns. This study aimed to comp

UNLABELLED: Management of severely comminuted, complete articular tibial pilon fractures (AO/OTA 43-C) remains a challenge, with few treatment options providing good clinical outcomes. Open reduction and internal fixation of the tibial plafond, tibiotalar arthrodesis, and salvage hindfoot reconstruction procedures are all associated with surgical complications and functional limitations. In this report, we present a case of a complex pilon fracture in a patient with multiple medical comorbidities and socioeconomic disadvantages that was successfully and acutely treated with a retrograde tibiot

Background and Clinical Significance: Non-prosthetic peri-implant fractures (NPPIFs) are rare injuries occurring around internal fixation devices, and are distinct from periprosthetic fractures. While most studies focus on the femur, humeral NPPIFs remain poorly documented. This case illustrates a complex humeral NPPIF and highlights key surgical considerations. Case Presentation: A 62-year-old woman presented with a spiral humeral shaft fracture (AO 12B2) after a fall. Following closed reduction and antegrade intramedullary nailing, an intraoperative peri-implant fracture occurred at the dist
