Towards automated ingestion detection: swallow sounds.

Obesity is a worldwide epidemic and is a cause of many major chronic diseases. In most cases, obesity is a result of an imbalance between food intake and calories burned. Steps toward automated ingestion detection are being made. In order to automate the process of capturing ingestion, a method for detecting, analyzing, and recording sounds related to ingestion is being developed. In this paper, preliminary swallow sound analysis is presented and compared with various other noises captured from a throat mounted microphone. Initial frequency analysis indicates a stronger presence at high freque

Effect of wearing a palatal plate on swallowing function.

PURPOSE: The purpose of this study was to determine the effect of wearing a palatal plate on swallowing function. METHODS: The subject group consisted of 10 healthy, fully dentate males. Two experimental palatal plates (EPP) were used in this study: one was 1.4mm thick (EPP1), and the other was 2.8mm thick (EPP2). Tongue movements and swallowing sounds were simultaneously recorded. The position of the surface of the tongue was recorded by using ultrasound diagnostic equipment in the sagittal plane. Swallowing tests were performed under three conditions: without EPP (WOE), with EPP1 (WP1), and

An unusual case of a swallowed thermometer perforated in the mediastinum.

A 39-year-old woman, who was addicted to drugs, was admitted because of referred chest pain and dysphagia after deliberate ingestion of a thermometer in a suicide attempt 6 hours earlier. Rigid esophagoscopy was unrevealing. On awakening, the patient confessed that the thermometer had been swallowed more than a month be operated on and get narcotic pain medications. Her history disclosed multiple admissions and laparotomies after suicide attempts with swallowed thermometers in the last 3 years.

Cortical recovery of swallowing function in wound botulism.

BACKGROUND: Botulism is a rare disease caused by intoxication leading to muscle weakness and rapidly progressive dysphagia. With adequate therapy signs of recovery can be observed within several days. In the last few years, brain imaging studies carried out in healthy subjects showed activation of the sensorimotor cortex and the insula during volitional swallowing. However, little is known about cortical changes and compensation mechanisms accompanying swallowing pathology. METHODS: In this study, we applied whole-head magnetoencephalography (MEG) in order to study changes in cortical activat

Swallowing disorders in trauma patients: impact of tracheostomy.

Tracheostomy is associated with increased aspiration rates, and swallowing disorders have not been well-studied in trauma patients with tracheostomy. Swallowing evaluations were conducted in 224 patients (102 trauma and 122 nontrauma patients). Half of the patients in each group had tracheostomies. Bedside swallow studies were conducted in 40 patients, videofluoroscopy swallow studies in 100 patients, and both studies in 84 patients. chi2, Fisher's exact test, Cramer's V, and descriptive statistics were used for data analysis. Aspiration occurred in 35 per cent (36 of 102) of trauma patients w

Hoarseness and misdirected swallowing in patients with hiatal hernia.

The purpose of this study was to elucidate whether misdirected swallowing is an extra-laryngeal cause of hoarseness and investigate whether the prevalence of misdirected swallowing and hoarseness in patients with hiatal hernias differ from those with and without pathological gastroesophageal reflux (GER). One hundred and ninety eight patients with hiatal hernias diagnosed via esophageal manometry and pH-reflux test and 262 subjects in the general population who did not have a hiatal hernia at endoscopy, filled in a questionnaire about symptoms on hoarseness, misdirected swallowing, and heartbu

Affective modulation of swallowing rates: unpleasantness or arousal?

OBJECTIVE: It has been observed that spontaneous swallowing rates are modulated by the affective state. Affect-induced swallowing may be an important biobehavioral factor in gastrointestinal disorders. However, previous studies failed to distinguish between pleasantness and arousal effects and did not analyze gender effects. METHOD: We analyzed swallowing rates in 42 participants (22 women) who viewed 3 blocks of 12 affective pictures preselected for inducing positive, negative, and neutral affect. Swallowing rate was measured by brief and rapid resistance increases in forced oscillation meas

Videofluoroscopy and swallowing studies for neurologic disease: a primer.

Many patients with neurologic impairment due to stroke, multiple sclerosis, trauma, bulbar palsy, and other disorders have difficulty swallowing. Videofluoroscopy can provide important information on patterns of impairment of the swallowing mechanism, allowing important changes in patient treatment. The detailed videofluoroscopic evaluation required to provide this information is now seldom taught and is practiced by relatively few radiologists. The aim of this article is to (a) describe the indications for videofluoroscopic swallowing studies in the evaluation of patients with neurologic cond

Hyoid-bolus transit latencies in normal swallow.

Pharyngeal swallow delay is frequently found in dysphagic patients and is thought to be a factor in a range of swallowing problems, including aspiration. Implicit in notions of swallow "delay" is a temporal interval between two events that is longer than normal. However, there appears to be little agreement about which referent events should be considered in determining delay. A number of pharyngeal bolus transit points and various pharyngeal gestures have been used in delays determined from fluoroscopic evidence, and other referents have been used in electromyographic and manometric studies o

Clinical and cognitive predictors of swallowing recovery in stroke.

This retrospective study determined whether specific neurological features were associated with initial and final swallowing outcomes in acute stroke patients. A chart review of 65 acute stroke patients suggested that certain clinical and neurocognitive behaviors were associated with swallowing outcomes. Hemispatial neglect was significantly associated with initial nonoral dietary intake, whereas aphasia was not associated with swallowing outcome. Results from the initial clinical swallowing evaluations suggested that the presence of at least four of six clinical features (cough after swallow,

Outcome measurement in dysphagia: not so hard to swallow.

This article reports on the use of a new tool from the Australian Therapy Outcome Measures (AusTOMs) set-the Swallowing scale. The scale is one of six, designed to measure outcomes of clients attending speech pathology practices in Australia. The tool was used for six months in clinical practices across 14 healthcare sites in Victoria, Australia (including six acute hospitals, six rehabilitation services, one specialist pediatric hospital, and one specialist cancer institute). This article provides preliminary descriptive data and analyses of outcomes from swallowing therapy, along with discus

Assessment of infant oral sensorimotor and swallowing function.

The development of feeding and swallowing is the result of a complex interface between the developing nervous system, various physiological systems, and the environment. The purpose of this article is to review the neurobiology, development, and assessment of feeding and swallowing during early infancy. In recent years, there have been exciting advances in our understanding of the physiology and neurological control of feeding and swallowing. These advances may prove useful in furthering our understanding of the pathophysiology of dysphagia in infancy. Progress in developing standardized, reli

Digital cineradiographic swallow study: our experience.

PURPOSE: Swallowing disorders can be secondary to different types of diseases in which, at least initially, patients succeed in establishing voluntary or involuntary compensatory mechanisms that enable them to maintain a sufficient nutritional state. When the compensatory mechanisms become insufficient massive food aspiration into the airways can occur and suffocation may prove to be the main pathology. It has been calculated that in the USA about 8,000-10,000 people die each year due to suffocation. The dynamic radiological examination of swallowing is considered important not only for diagno

Swallowing dysfunction related to snoring: a videoradiographic study.

Biopsy studies of the soft palatal and oropharyngeal tissues in habitual snorers and patients suffering from obstructive sleep apnoea have shown signs of neurogenic lesions. These lesions might affect the pharyngeal swallowing function, which is dependent on adequate sensitivity. The objective of the present study was to test the hypothesis that snoring is associated with aberrant pharyngeal swallowing function. Forty-one consecutive patients without dysphagia, seeking medical attention because of heavy snoring and various degrees of daytime sleepiness, were prospectively selected. Fifteen non
