The role of the central nervous system in hypertension.

Normally, the kidney plays the dominant role in setting long-term arterial pressure, and the nervous system acts primarily as a short-term r … earchers recently characterized abnormalities in the brain that seem to predispose animal models to sympathetic nervous system overactivity and hypertension. Together, the current data strongly suggest that the brain, via the sympathetic nervous system, directly contributes to some forms of hypertension and indirectly contributes to all of them. This review is not intended …

Disorders of blood pressure regulation-role of catecholamine biosynthesis, release, and metabolism.

Catecholamines (epinephrine and norepinephrine) are synthesised and produced by the adrenal medulla and postganglionic nerve fibres of the sympathetic nervous system. It is known that essential hypertension has a significant neurogenic component, with the rise in blood pressure mediated at least in part by overactivity of the sympathetic nervous system. Moreover, novel therapeutic strategies aimed at reducing sympathetic activity show promise in the treatment of hypertension. This article reviews recent advances within this rapidly changing field, particularly focusing on the role of genetic p…

Intensity of motion sickness symptoms as a function of apparent autonomic balance.

Twenty volunteer male college students were exposed to motion picture films which reliably elicit symptoms of motion sickness. Those Ss with relatively higher apparent sympathetic nervous system dominance showed significantly smaller autonomic reactions to the film. It was concluded that increased sympathetic tone tended to the film. It was concluded that increased sympathetic tone tended to reduce autonomic reactions to motion sickness stimuli. It was suggested that the sympathetic nervous system symptoms that usually occur in motion sickness are actually defensive reactions rather than sympt…

New insights into sympathetic regulation of glucose and fat metabolism.

The autonomic nervous system modulates glucose and fat metabolism through both direct neural effects and hormonal effects. This review prese … nt of feeding. The effects of leptin on glucose and fat metabolism could be in part mediated by the sympathetic nervous system. Studies using mice with a genetic disruption of serotonin 5-HT2c receptor indicate that central neural mechanisms in the regulation of sympathetic outflow and satiety could be dissociated. Abnormalities of sympathetic effects, including disturbance …

Autonomic and endocrine factors in the regulation of energy balance.

The regulation of energy reserves is modified by both the autonomic nervous system and the hormonal milieu. The activity of the two limbs of the autonomic nervous system shows a reciprocal response to stimulation or damage in either the ventromedial or the lateral hypothalamus. Ventromedial hypothalamic lesions decrease the activity of the sympathetic nervous system and increase the activity of the vagus nerve. Lateral hypothalamic lesions, on the other hand, increase the activity of the sympathetic nervous system. Central neurotransmitters involved in energy balance include the monoamines, am…

[Sleep Disorders and Autonomic Nervous System Dysfunction].

Parasympathetic nervous system activity dominates during non-rapid eye movement (REM) sleep, whereas the sympathetic nervous system is dominant during REM sleep. The balance between the sympathetic and parasympathetic nervous systems reduces the heart rate and blood pressure during non-REM sleep and increases these parameters during REM sleep and wakefulness. Heart rate variability analysis, a non-invasive method of autonomic nervous system analysis, may be useful in screening for obstructive sleep apnea and also to predict orthostatic hypotension secondary to autonomic disturbances in patient…

Sympathetic control of the circulation in hypertension: lessons from autonomic disorders.

PURPOSE OF THE REVIEW: Inappropriate sympathetic overactivity is consistently observed in patients with essential hypertension. The present review summarizes the recent advances that have been made in our understanding of the role of the sympathetic nervous system in hypertension. RECENT FINDINGS: Studies in patients with autonomic disorders underscore the role of the sympathetic nervous system in the long-term maintenance of hypertension. Abnormalities in the afferent limb of the sympathetic nervous system, in the regulation of central neurons where sympathetic outflow originates, and in the…

Studies of neurotransmitter release in the pathogenesis of hypertension.

Studies using a sensitive radioenzymatic assay for plasma noradrenaline suggest there is a selective overactivity of the sympathetic nervous system in essential hypertension. Methodology which allows the study of local sympathetic turnover in CNS nuclei and peripheral blood vessels is described. This approach has been used to study the non-innervated sympathetic turnover phaeochromocytoma. It is suggested that studies of local regulatory mechanism in neurotransmitter release are required to give a greater understanding of the central and peripheral role of the sympathetic nervous system in the…

Stellate Ganglia and Cardiac Sympathetic Overactivation in Heart Failure.

Heart failure (HF) is a major public health problem worldwide, especially coronary heart disease (myocardial infarction)-induced HF with red … egulatory circuitry at multiple levels, including afferent, central, and efferent components of the sympathetic nervous system. Much evidence, from other investigators and us, has confirmed the afferent and central neural mechanisms causing sympathoexcitation in HF. The stellate ganglion is a peripheral sympathetic ganglion formed by the fusion of the 7th cervical and 1st t …

Autonomic control of bone formation: its clinical relevance.

Bone remodeling is thought to be regulated by many factors including nutritional status, humoral factors, and biomechanical stress. However, the involvement of the autonomic nervous system, mainly the sympathetic nervous system (SNS), in the modulation of bone remodeling is beginning to receive more attention. Neural innervation of bone has been demonstrated. Both experimental and clinical evidence has indicated the involvement of autonomic nervous system regulation in bone metabolism. The sympathetic neural pathway is so far the only identified link between the potent leptin-dependent central…

Sympathetic neural mechanisms in human hypertension.

This review discusses the role of the sympathetic nervous system in the pathogenesis and maintenance of human hypertension. Three points are emphasized: first, there are mechanisms by which the sympathetic nervous system can contribute to the long-term regulation of vascular resistance and arterial pressure in addition to the moment-to-moment regulation of arterial pressure; second, the microneurographic method for direct intraneural recording of sympathetic nerve activity in humans has provided mounting evidence for increased sympathetic neural activity in human essential and renovascular hyp…

The sympathetic innervation of the heart: Important new insights.

Autonomic control of the heart has a significant influence over development of life threatening arrhythmias that can lead to sudden cardiac death. Sympathetic activity is known to be upregulated during these conditions and hence the sympathetic nerves present a target for treatment. However, a better understanding of the anatomy and physiology of cardiac sympathetic nerves is required for the progression of clinical interventions. This review explores the organization of the cardiac sympathetic nerves, from the preganglionic origin to the postganglionic innervations, and provides an overview o

Neuroimmunomodulation of adrenoblockers during liver cirrhosis: modulation of hepatic stellate cell activity.

The sympathetic nervous system and the immune system are responsible for producing neurotransmitters and cytokines that interact by binding to receptors; due to this, there is communication between these systems. Liver immune cells and nerve fibres are systematically distributed in the liver, and the partial overlap of both patterns may favour interactions between certain elements. Dendritic cells are attached to fibroblasts, and nerve fibres are connected via the dendritic cell-fibroblast complex. Receptors for most neuroactive substances, such as catecholamines, have been discovered on dendr…

Paroxysmal Sympathetic Hyperactivity.

Paroxysmal sympathetic hyperactivity (PSH) is a relatively common, but often unrecognized, complication of acute diffuse or multifocal brain diseases, most frequently encountered in young comatose patients with severe traumatic brain injury. It is presumed to be caused by loss of cortical inhibitory modulation of diencephalic and brain stem centers and possible additional maladaptive changes in the spinal cord that combine to produce exaggerated sympathetic responses to stimulation. The syndrome consists of repeated sudden episodes of tachycardia, tachypnea, hypertension, sweating, and sometim

Autonomic nervous system dysregulation in human hypertension.

An increased sympathetic drive combined with decreased parasympathetic inhibition is found in patients with borderline hypertension, who characteristically have rapid heart rates, high cardiac output and relatively normal vascular resistance (hyperkinetic state). In established hypertension, cardiac output is normal, vascular resistance is elevated and signs of increased sympathetic drive are absent. Apparently hemodynamics and sympathetic drive change during hypertension. The mechanism of the hemodynamic transition in the course of hypertension is well understood. Cardiac output returns from
