Predicting short-term adverse outcomes in the geriatric population presenting with syncope: a comparison of existing syncope rules and beyond.

OBJECTIVES: Syncope at age 65+ is associated with increased mortality, irrespective of cause. Syncope rules were designed to aid in risk-stratification but were only validated in the general adult population. Our objective was to determine if they can be applied to a geriatric population in predicting short-term adverse outcomes. METHODS: In this single-center retrospective study, we evaluated 350 patients aged 65+ presenting with syncope. Exclusion criteria included confirmed non-syncope, active medical condition, drug or alcohol-related syncope. Patients were stratified into high or low ris…

Psilocybin-induced neurocardiogenic syncope: a case report.

BACKGROUND: Psilocybin is among the serotonergic psychedelics closest to potential FDA approval, with growing evidence of therapeutic benefit across psychiatric conditions. As clinical trials expand, systematic characterization of adverse events (AEs) remains essential. While transient hypertensive responses are well documented, hypotensive events such as neurocardiogenic syncope (NCS) are rarely reported. CASE PRESENTATION: We describe a healthy 35‑year‑old male enrolled in an open‑label study investigating psilocybin‑evoked changes in brain function during transcranial magnetic stimulation …

Procedural case: Ultrasound Case: Syncope in PE.

AUDIENCE: This certifying exam practice ultrasound case is intended for emergency physicians (EP) in training. INTRODUCTION: Point-of-care … Examination (OSCE) focused on point-of-care ultrasound (POCUS) in the evaluation of a patient with syncope and suspected pulmonary embolism (PE).3 This format aligns with the American Board of Emergency Medicine's (ABEM) newly implemented certification examination, emphasizing real-time clinical reasoning and ultrasound skills in a simulated encounter.4 …

Signature ECG part 5: Exercise induced syncope.

14-year-old boy presented with aborted sudden cardiac arrest during exertion. His brother died suddenly at 16 years of age during an emotional situation. On stress testing, patient developed classical bidirectional PVCs typical of CPVT (Catecholaminergic polymorphic ventricular tachycardia). The genetic testing showed pathogenic heterozygous variant in CASQ2 gene. Patient was started on propranolol and flecainide and advised to avoid strenuous physical activity.

Bladder Paraganglioma Presenting as Post-Micturition Syncope.

UNLABELLED: A 22-year-old woman presented with a 12-year history of intensifying paroxysms of anxiety, palpitations and recurrent syncope following micturition. The patient was referred to endocrinology upon discovery of hypertension. An extended family history revealed metastatic phaeochromocytoma and paraganglioma in two grand-uncles. Clinical examination revealed hypertension, with a mean 24-hour ambulatory blood pressure of 150/100 mmHg. Supine plasma normetanephrines were markedly elevated with a raised 3-methoxytyramine, while plasma metanephrines were normal. Computed tomography identif…

Micturition syncope secondary to urinary bladder paraganglioma.

Paraganglioma of the bladder is a rare tumour accounting for less than 0.06% of all urinary bladder tumours and has varied presentations. It may present with clinical symptoms of phaeochromocytoma, may be non-functioning and asymptomatic or may present with haematuria. Hence, paragangliomas are occasionally misdiagnosed, and this results in unanticipated intraoperative hypertensive crisis. We present the case of a 44-year-old woman with urinary bladder paraganglioma who presented with young onset hypertension, recurrent micturition syncope with prior history of coronary artery disease and stro…

"Complex" Vasovagal Syncope: A Zebra Among Horses.

Background: Vasovagal syncope (VVS) occurs due to cerebral hypoperfusion from a fall in blood pressure, with accompanying bradycardia in most cases. Seizure and/or asystole may accompany VVS, though their prediction within the VVS cohort remains elusive. Objective: To further characterize VVS and to find predictive features of "complex" VVS (defined as VVS associated with seizures and/or asystole). Methods: We reviewed medical records of all patients who were referred for orthostatic intolerance and had a definite VVS during the head-up tilt table testing (HUTT). The following variables were r…

Elusive cardiac dysrhythmia in high-risk syncope.

Emergency department presentations of syncope can vary from benign to life-threatening etiologies. Older patients are at increased risk of cardiac causes of syncope. Ventricular standstill is a rare phenomenon that can manifest as syncope and must be correctly identified and promptly treated to prevent sudden cardiac arrest. We report the case of a 70-year old man with dizziness and convulsive syncope whose initial ECG showed a right bundle branch block, but then developed ventricular standstill and intermittent high-grade AV block while still in the ED. He was transferred to the ICU and under…

Syncope due to unexpected paroxysmal sinus arrest.

We present two cases with unexpected long sinus arrest resulting in syncope. There were no signs or symptoms of either sick sinus syndrome or increased vagal tone before the occurrence of pauses in these patients. The fact that these patients remained asymptomatic for quite long shows striking resemblance to paroxysmal sub-AV nodal block. We would therefore prefer to classify these cases as paroxysmal sinus arrest as a possible manifestation of the sick sinus syndrome.

Syncope and bundle branch block : Diagnostic approach.

Syncope and bundle branch block are reviewed, addressing their specific clinical characteristics, natural history, initial diagnostic approach and the role and limitations of different diagnostic tests with a special focus on electrophysiological studies and implantable loop recorders. A critical review of the different published strategies to be followed in these patients is made and finally an algorithm of how to manage these patients is proposed.

Syncope Episodes and Blood Flow Restriction Training.

The combination of low-load resistance training [or more recently, neuromuscular electrical stimulation (NMES)] with a moderate local blood flow restriction (BFR) is becoming a widespread training and rehabilitation method. Scientific data indicate the overall safety of BFR, at least in healthy young people. However, it has been associated with side effects, usually minor, and further research is warranted regarding the safety and efficacy of this technique, especially in clinical populations. We found 3 syncope/presyncopal episodes among 21 healthy people (9 men), all occurring in men and dur…

Bradycardia, Syncope, and Left Ventricular Noncompaction Cardiomyopathy.

A 55-year-old man with syncopal episodes was found to have sinus bradycardia at a rate of 37 beats/min, an episode of nonsustained ventricular tachycardia, and left ventricular noncompaction cardiomyopathy. After placement of a dual-chamber cardioverter defibrillator 4 years ago, he has had no further syncopal episodes, and there have been no defibrillator shocks.

Cardioinhibitory syncope due to bloodphobia associated asystole.

Neurocardiogenic syncope is not an uncommon occurrence in the general population and affects people across a wide range of age groups. Several sub-specialties are involved in the management of this phenomenon and quite often a multidisciplinary approach is needed for arriving at the final diagnosis and deciding on the optimal treatment for this condition. Often, a thorough history and physical examination will aid in narrowing the differential of syncope and, in the right setting, both passive and provocative testing can be complementary. Neurocardiogenic syncope with a malignant course is a s…

Emotion-triggered cardiac asystole-inducing neurocardiogenic syncope.

The pathophysiology of neurocardiogenic syncope (NCS) is multifactorial. Recurrent syncopal episodes can result in injury and can provoke substantial anxiety among patients. Although an abundance of descriptions of various forms of syncope have been reported in the literature, few articles to date address a documented case due to emotional stress or sound. This is a report of a 31-year-old woman who fainted after being startled by someone sneezing. Review of the episode on her event recorder revealed a transient cardiac asystole of 10 seconds. We discuss the incidence of NCS and the proposed m…
