Impaired flexible decision-making in Major Depressive Disorder.

BACKGROUND: Depression is associated with dysfunctional affective states, neuropsychological impairment and altered sensitivity to reward and punishment. These impairments can influence complex decision-making in changing environments. METHODS: The contingency shifting variant Iowa Gambling Task (IGT) was used to assess flexible decision-making performance in a group of medicated unipolar Major Depressive Disorder (MDD) patients (n=19) and a group of healthy control volunteers (n=20). The task comprised the standard IGT followed by a contingency-shift phase where decks progressively changed r

Neurologic examination abnormalities in children with bipolar disorder or attention-deficit/hyperactivity disorder.

BACKGROUND: Attention-deficit/hyperactivity disorder (ADHD) and bipolar disorder (BPD) are frequently comorbid and overlapping diagnoses. To move beyond diagnosis toward unique pathophysiology, we evaluated both ADHD and BPD children for neurologic examination abnormalities (NEAs) in comparison with normal control (NC) children. METHODS: We performed the Revised Physical and Neurological Examination for Soft Signs in three groups (ADHD, BPD, NC). Then, a rater blind to diagnosis evaluated their motor performance. Results were analyzed with a multiple analysis of covariance. RESULTS: Subjects

Obsessive-compulsive disorder and global mental health.

The discipline of global mental health has emphasized the importance of the treatment gap in mental disorders, and of addressing this gap via changes in health policy, an emphasis on human rights, and innovations such as task-shifting. Although global mental health research has focused on both common mental disorders such as depression, and serious mental disorders such as schizophrenia, it has paid relatively little attention to obsessive-compulsive and related disorders (OCRDs). Nevertheless, international collaborations have recently paid a good deal of attention to the nosology and neurobi

Developmental Unity and Cultural Variation in Forms of Metarepresentational False Belief Understanding.

Around age four, children in urban, post-industrial societies begin to understand the subjectivity of beliefs and other mental states: they can be true or false, and they are aspectual, representing situations under specific descriptions. These two insights have been found to emerge in tandem, suggesting a shift toward a metarepresentational Theory of Mind. However, as evidence for this shift comes primarily from formally educated, urban, post-industrial societies, its broader generalizability remains speculative. Moreover, standard pretense-heavy false belief tasks may bias conclusions drawn

"Phase capture" in the perception of interpolated shape: cue combination and the influence function.

This study was concerned with what stimulus information observers use to judge the shape of simple objects. We used a string of four Gabor patches to define a contour. A fifth, center patch served as a test pattern. The observers' task was to judge the location of the test pattern relative to the contour. The contour was either a straight line, or an arc with positive or negative curvature (the radius of curvature was either 2 or 6 deg). We asked whether phase shifts in the inner or outer pairs of patches distributed along the contour influence the perceived shape. That is, we measured the pha

Quantifying and Visualizing Emergency Physician Workflow: Observational Time-Motion Study.

BACKGROUND: Emergency physicians face considerable workflow challenges in the emergency department (ED) due to unpredictable work environments, frequent interruptions, and mounting documentation requirements, all of which contribute to substantial computer-based work. Excessive time away from direct patient care is increasingly viewed as detrimental to the quality of care, communication, and patient safety. OBJECTIVE: This study aimed to quantify and visualize how emergency physicians allocate their time during ED shifts, particularly the time spent on the computer. METHODS: This cross-secti

Exploring the effects of ecological activities during exposure to optical prisms in healthy individuals.

Prism adaptation improves a wide range of manifestations of left spatial neglect in right-brain-damaged patients. The typical paradigm consists in repeated pointing movements to visual targets, while patients wear prism goggles that displace the visual scene rightwards. Recently, we demonstrated the efficacy of a novel adaptation procedure, involving a variety of every-day visuo-motor activities. This "ecological" procedure proved to be as effective as the repetitive pointing adaptation task in ameliorating symptoms of spatial neglect, and was better tolerated by patients. However, the absence

Hemispheric efficiency during the day: the influence of circadian preference and time-of-day on verbal and visuospatial tasks.

INTRODUCTION: Cognitive performance fluctuates daily due to the interaction between circadian and homeostatic processes, which may differently activate the two hemispheres. This study investigated the effects of time-of-day, circadian typology, and the synchrony effect on semantic categorization and visuospatial processing tasks with potential hemispheric lateralization. METHODS: A sample of 113 healthy young women (20.60 ± 2.60 years) performed two divided visual field tasks (semantic categorization and vertical landmark) in morning (08:00) and early afternoon (13:00) sessions. RESULTS: In

Executive functions in older adults with generalised anxiety disorder and healthy controls: Associations with heart rate variability, brain-derived neurotrophic factor, and physical fitness.

Executive functions (EF) decline with age and this decline in older adults with generalised anxiety disorder (GAD) may be influenced by heart rate variability (HRV), brain-derived neurotrophic factor (BDNF), and physical fitness. Understanding these relationships is important for tailored treatments in this population. In this study, 51 adults with GAD (M age = 66.46, SD = 4.08) and 51 healthy controls (M age = 67.67, SD = 4.04) were assessed on cognitive inhibition (Stroop task), shifting (Trails part 4), flexibility (Wisconsin Card Sorting Test - Perseverative errors), working memory (Digit

Prism adaptation alters spatial remapping in healthy individuals: evidence from double-step saccades.

The visual system is able to represent and integrate large amounts of information as we move our gaze across a scene. This process, called spatial remapping, enables the construction of a stable representation of our visual environment despite constantly changing retinal images. Converging evidence implicates the parietal lobes in this process, with the right hemisphere having a dominant role. Indeed, lesions to the right parietal lobe (e.g., leading to hemispatial neglect) frequently result in deficits in spatial remapping. Research has demonstrated that recalibrating visual, proprioceptive a

Independent control of force and timing symmetry in dynamic standing balance: implications for rehabilitation of hemiparetic stroke patients.

The extent to which force and timing are independently controlled has aroused research interest. The present study aims to examine the relationship between force and timing symmetrical performances during repetitive body weight shifting. Eight hemiparetic stroke patients and eight age- and gender-matched controls were required to repetitively shift weight from one leg to the other while standing with each foot on a separate force plate. Instructions emphasised symmetry of weight transfer in each direction. No knowledge of result feedback was given. The ratio of the vertical ground reaction for

Frontal impairment in subcortical ischemic vascular dementia in comparison to Alzheimer's disease.

We compared the performance of patients with Alzheimer's disease to that of patients with subcortical vascular dementia (s-IVD) in a set of tasks assessing categorization abilities, sustained and selective attention, and set-shifting and set-maintaining skills. Only the measures of naming and categorization abilities on the Test of Classification and Recall of Pictures (TCRP) proved useful in differentiating AD from s-IVD patients. s-IVD patients showed worse performance than AD on the TCRP categorization measures, while both AD and s-IVD patients were equally impaired in other tasks assessing

DeID-Aligner: Fine-Grained Brain-Vision Semantic Alignment for Cross-Subject Visual Decoding from Limited fMRI Data.

Recent advances in fMRI-based semantic decoding and visual reconstruction have achieved remarkable progress. However, severe inter-subject variability and the scarcity of paired fMRI-image data remain major obstacles to cross-subject generalization. To overcome these challenges, we propose DeID-Aligner, a brain functional alignment framework that enables multi-task visual decoding across subjects under limited fMRI data. The method first constructs a region-of-interest (ROI)-wise fMRI encoder that captures regional functional representations. A dynamic Patch-to-ROI fusion mechanism with Progre

Differential effects of sertraline and cognitive behavioural therapy on behavioural inhibition in patients with obsessive compulsive disorder.

Patients with obsessive compulsive disorder (OCD) randomised to sertraline, manualised cognitive behavioural therapy (CBT), or combination (sertraline + CBT), underwent cognitive assessment. Cognitive testing was conducted at baseline and at week 16. The stop signal reaction time task (SSRT) was used to evaluate motor impulsivity and attentional flexibility was evaluated using the intra/extra-dimensional set shifting task. Paired-samples t -tests or nonparametric variants were used to compare baseline and posttreatment scores within each treatment group. Forty-five patients were tested at base

Compulsivity and Inhibitory Control Deficits in Abstinent Individuals With Heroin Addiction and Their Biological Siblings Compared With Unrelated Healthy Control Participants.

BACKGROUND: Compulsivity represents the performance of persistent and repetitive acts despite negative consequences and is considered one of the critical mechanisms for drug addiction. Although compulsivity-related neurocognitive impairments have been linked to addiction, it remains unclear whether these deficits might have predated drug abuse as potential familial susceptibilities. METHODS: A large sample of 213 adult participants were recruited, including 70 abstinent individuals addicted to heroin (HAs), 69 unaffected biological siblings of the HAs (siblings), and 74 unrelated healthy cont

Assessment of Stress Effects on Cognitive Flexibility using an Operant Strategy Shifting Paradigm.

Stress affects cognitive function. Whether stress enhances or impairs cognitive function depends on several factors, including the 1) type, intensity, and duration of the stressor; 2) type of cognitive function under study; and 3) timing of the stressor in relation to learning or executing the cognitive task. Furthermore, sex differences among the effects of stress on cognitive function have been widely documented. Described here is an adaptation of an automated operant strategy shifting paradigm to assess how variations in stress affect cognitive flexibility in male and female Sprague Dawley

The involvement of the orbitofrontal cortex in learning under changing task contingencies.

Previous investigations examining the rat prefrontal cortex subregions in attentional-set shifting have commonly employed two-choice discriminations. To better understand how varying levels of difficulty influence the contribution of the prefrontal cortex to learning, the present studies examined the effects of orbitofrontal cortex inactivation in a two- or four-choice odor reversal learning test. Long-Evans rats were trained to dig in cups that contained distinct odors. In the two-choice odor discrimination, one odor cup was always associated with a cereal reinforcement in acquisition while t

Flexibility in young people with autism spectrum disorders on a card sort task.

Adults with autism spectrum disorders (ASD) have shown deficits in switching between rules governing their behaviour, as have high-functioning children with ASD. However, there are few studies of flexibility in lower-functioning children with ASD. The current study investigated this phenomenon with a group of low-functioning children with ASD compared to a mental-age-matched comparison group. The ASD group learned an initial discrimination task as quickly as the matched comparison group, but when the rule governing the discrimination was shifted, the comparison group learned the task with fewe

Go/No-Go task engagement enhances population representation of target stimuli in primary auditory cortex.

Primary sensory cortices are classically considered to extract and represent stimulus features, while association and higher-order areas are thought to carry information about stimulus meaning. Here we show that this information can in fact be found in the neuronal population code of the primary auditory cortex (A1). A1 activity was recorded in awake ferrets while they either passively listened or actively discriminated stimuli in a range of Go/No-Go paradigms, with different sounds and reinforcements. Population-level dimensionality reduction techniques reveal that task engagement induces a s
