This is a case report of a 31-year-old woman going through cancer staging after being diagnosed with breast cancer. During sentinel node dissection, a remarkable dark lymph node was found. Metastatic malignant melanoma was suspected, but with careful histochemical examination the lymph node was confirmed to only contain tattoo pigment. The patient had rather large tattoos on her arms, which was suspected to be the source of the ink in her lymph nodes. Tattoo pigment can complicate cancer staging, and it is important to know this rare effect on lymph nodes when dealing with cancer staging.

Tattoo reactions have become more common as tattoos gain visibility and popularity worldwide. Traditional Chinese medical theory holds that fire acupuncture can relieve neuropathic pain, and in many Asian countries, acupuncture has been used as one of the methods to relieve herpes zoster nerve pain. Sometimes, the patterns left on the skin by the needles may be very confusing to the practitioner who sees the patient. Recently, we saw a patient with puzzling tattoo-like dermatitis on his skin, and upon further questioning, he had recently undergone a fire-needle treatment and was left with this pattern.

BACKGROUND: Worldwide 10-20% of the population is tattooed. However, tattoo complications can occur, such as allergic tattoo reactions, infections, and manifestations of autoimmune dermatoses.
