Rheumatoid Arthritis and Tenosynovitis at the Metatarsophalangeal Joints: An Anatomic and MRI Study of the Forefoot Tendon Sheaths.

Background Although tenosynovitis in the hands is associated with rheumatoid arthritis (RA), it is unknown whether tenosynovitis of the forefoot is associated with RA. Purpose To determine the anatomy of tendon sheaths of the forefoot and the relationship between MRI-detected tenosynovitis at metatarsophalangeal (MTP) joints and RA. Materials and Methods Fourteen forefeet of donated bodies were examined at flexor tendons and extensor tendons for the presence and course of tendon sheaths. In the prospective study between June 2013 and March 2016, newly presenting patients with RA, patients with…

Tuberculous Tenosynovitis of the Wrist: A Report of Two Cases with Special Reference to Magnetic Resonance Findings.

BACKGROUND: Tuberculous tenosynovitis of the wrist is rare among cases of musculoskeletal tuberculosis. It is a slow, progressive disease with silent symptoms, leading to a late diagnosis. Also, it can be misdiagnosed as inflammatory arthritis, pyogenic infections, and inflammatory tenosynovitis. Thus, patients present with destructive changes of the bone and joint, as well as abscesses, often receive incorrect or unnecessary medical and surgical treatment. DISCUSSION: Well-documented anamneses along with physical examinations and radiologic imaging methods, such as computed tomography (CT) a…

Dynamic Functional Assessment of Hand Motion Using an Animation Glove: The Effect of Stenosing Tenosynovitis.

BACKGROUND: The aim of the present study is to determine whether an animation glove can be utilized to provide a reliable and reproducible assessment of dynamic hand function and whether this assessment is altered in the setting of hand pathology. METHODS: Ten subjects without known hand pathology and 11 subjects with known stenosing tenosynovitis were assessed on tasks involving hand function at varied speeds, including forceful and gradual making of a fist and the quick and slow grip of a baseball using an animation glove to record range of motion and measures of velocity (CyberGlove II). …

Current limitations in control of viral arthritis and tenosynovitis caused by avian reoviruses in commercial poultry.

Avian reoviruses are the causative agent of viral arthritis/tenosynovitis in chickens and turkeys. Clinical signs of disease include swelling of the hock joints accompanied by lesions in the gastrocnemius and digital flexor tendons causing lameness in addition to hydropericardium. The economic impact is significant as it results in poor weight gain, increased feed conversion ratios and condemnations at the processing plant. Vaccination with both live attenuated and inactivated oil emulsion vaccines have been used successfully for decades to control the disease. Current commercial vaccine strai…

MRI assessment of tenosynovitis in children with juvenile idiopathic arthritis: inter- and intra-observer variability.

BACKGROUND: There is sparse knowledge about grading tenosynovitis using MRI. OBJECTIVE: The purpose of this study was to assess the reliability of a tenosynovitis MRI scoring system in juvenile idiopathic arthritis. MATERIALS AND METHODS: Children with juvenile idiopathic arthritis and wrist involvement were enrolled in two paediatric centres, from October 2006 to January 2010. The extensor (compartments II, IV and VI) and flexor tendons were assessed for the presence of tenosynovitis on T1-weighted postcontrast fat-saturated MR images and were scored from 0 (normal) to 2 (moderate to severe…

Acute gonococcal flexor tenosynovitis in an adolescent male with pharyngitis. A case report and literature review.

A 15-year-old boy had acute gonococcal flexor tenosynovitis of the middle finger associated with symptomatic gonococcal pharyngitis. The patient had a history of blunt trauma to the hand. The need for a careful history in any sexually active adolescent is emphasized. Acute flexor tenosynovitis may be the only manifestation of disseminated gonococcal infection.

Ulcerative dermatitis, tendinitis, tenosynovitis, and desmitis caused by chronic infection with Serratia odorifera in a racehorse.

We investigated a case of severe Serratia odorifera infection in a racehorse. The horse had developed progressive lameness associated with swelling of the left tarsal region, which progressed to ulcerative dermatitis, tendinitis, tenosynovitis, and desmitis. The plantar side of the superficial digital flexor (SDF) tendon was visible through the skin ulcer, and coalescing nodules were present in the dermis. The horse was euthanized and an autopsy was performed. Grossly, in addition to the changes described above, severe edema of the soft tissues was associated with the ulcer. Microscopically, n…

Mycobacterium haemophilum tenosynovitis of the thumb in an immunocompetent adult: a case report highlighting a diagnostic challenge.

Objective: We report a rare case of Mycobacterium haemophilum flexor tenosynovitis in an immunocompetent adult and discuss the diagnostic challenges posed by this organism under standard culture conditions. Patient and Methods: A 72-year-old Japanese man presented with chronic pain and swelling of the left thumb following a rose thorn injury and subsequent exposure to aquarium water. Magnetic resonance imaging revealed an effusion in the flexor tendon sheath. The patient underwent synovectomy and debridement. Acid-fast bacilli staining of intraoperative specimens was negative, and liquid mycob…

Coexistent gouty and infectious tenosynovitis in the hand: a case report and narrative review of comparable cases.

Although less commonly in the hand, gouty tenosynovitis may present with symptoms resembling infection. Only a few case reports document such presentations, and reports of coexisting infection and gouty tenosynovitis are even more uncommon. A 32-year-old male with polyarticular tophaceous gout, noncompliant with medications, presented with a one-day history of right index finger swelling and redness. Investigations were suggestive of infectiousious tophus. Despite broad-spectrum antibiotics and rheumatologic interventions (colchicine, allopurinol, and corticosteroids), his condition deteriorat…

A Comparison of Parenteral and Per-Oral Antibiotic Usage in Pyogenic Flexor Tenosynovitis: A Retrospective Study.

Introduction Pyogenic flexor tenosynovitis (PFT) is a common hand infection that can cause significant morbidity. Although treatment involves surgical debridement and inpatient intravenous (IV) antibiotics, there is a paucity of literature guiding antibiotic use. This study aims to determine if the use of postoperative outpatient oral antibiotics leads to poor outcomes compared to IV antibiotics given in an institutional setting. Methods A retrospective review of 110 patients treated post-operatively with either outpatient oral or inpatient IV antibiotics at our institution from 2016-2019 was …

Contribution of pathogenic T helper 1 and 17 cells to bursitis and tenosynovitis in polymyalgia rheumatica.

BACKGROUND: Although polymyalgia rheumatica (PMR) is a very common rheumatic inflammatory disease, current insight into the pathobiology of PMR is limited and largely based on studies in blood. We investigated T helper 1 (TH1) and T helper 17 (TH17) cell responses in blood, synovial fluid and bursa tissue of patients with PMR. MATERIALS AND METHODS: Blood samples were collected from 18 patients with new-onset PMR and 32 healthy controls. Synovial fluid was aspirated from the inflamed shoulder bursae or biceps tendon sheath of 13 patients. Ultrasound-guided biopsies of the subacromial-subdelto

Snapping wrist due to bony prominence and tenosynovitis of the first extensor compartment: A case report.

BACKGROUND: A snapping wrist is a rare symptom that results from the sudden impingement of one anatomic structure against another, subsequently causing a sudden movement only during wrist movement. CASE SUMMARY: A 30-year-old woman with a history of right wrist contusion reported right wrist snapping after overuse. The snapping became symptomatic after moving heavy objects. The pain persisted even when she received 1 mo of conservative treatment. Physical examination showed painful wrist snapping during wrist radioulnar motion and thumb abduction-adduction. Radiography demonstrated bone overg
