Noninvasive monitoring technologies to identify discomfort and distressing symptoms in persons with limited communication at the end of life: a scoping review.

BACKGROUND: Discomfort and distressing symptoms are common at the end of life, while people in this stage are often no longer able to express themselves. Technologies may aid clinicians in detecting and treating these symptoms to improve end-of-life care. This review provides an overview of noninvasive monitoring technologies that may be applied to persons with limited communication at the end of life to identify discomfort. METHODS: A systematic search was performed in nine databases, and experts were consulted. Manuscripts were included if they were written in English, Dutch, German, French

Impact of the COVID-19 Pandemic on Cancer Death Locations in Japan: An Analysis of Excess Mortality Through February 2023.

BACKGROUND: The novel coronavirus disease 2019 (COVID-19) pandemic has significantly impacted end-of-life decisions for cancer patients in Japan, with disparities existing between preferred and actual care settings. Our study investigates the potential shifts in cancer death locations during the pandemic and if there were excess cancer deaths. METHODS: Utilizing national mortality data from the Ministry of Health, Labour and Welfare from January 2012 to February 2023, we identified cancer deaths using International Classification of Disease, 10th revision codes. We assessed death locations, i

Large-scale evaluation of a computer-based learning program to increase prison staff knowledge on geriatric and end-of-life care.

PURPOSE: The purpose of this study is to continue research and development of the ECAD-P learning system with an emphasis on developing a scalable unit for testing in a larger number of more diverse correctional settings. There are almost 2.3 million US persons incarcerated. Geriatric and end-of-life (EOL) care in corrections is not as equitable as care in the free world. Technological delivery of geriatric training to staff through computer-based learning (CBL) offers a novel approach to improve care and reduce disparities among those who are most vulnerable during confinement. End of life care for patients with meningioma.

Palliative care (PC) supports patient with serious illnesses and can help patients with meningioma through the phases of their clinical trajectory, from initial diagnosis through the last hours of life. The PC team implements a multimodal transdisciplinary approach to address physical, psychosocial, and spiritual suffering with patients and their families, while also fostering constructive communication with the many health care providers involved. To achieve these goals the PC core team is comprised of physicians, nurse practitioners, physician assistants, nurses, social workers, and spiritua

Coma and disorders of consciousness.

Patients in coma, vegetative state/unresponsive wakefulness syndrome, and in minimally conscious states pose medical, scientific, and ethical challenges. As patients with disorders of consciousness are by definition unable to communicate, the assessment of pain, quality of life, and end-of-life preferences in these conditions can only be approached by adopting a third-person perspective. Surveys of healthcare workers' attitudes towards pain and end of life in disorders of consciousness shed light on the background of clinical reality, where no standard medical-legal framework is widely accepte

Late stage care in Huntington's disease.

Huntington's disease (HD) is a relentlessly progressive neurodegenerative disease, which is presently incurable. Despite the hope for future therapies that exists for the families, they meantime are aware that there is little that their clinicians can do to delay either onset or progression of the disease. There are unique issues to address in a disorder where patients are aware in advance that their ability to communicate, as well as their cognitive capacity, will become impaired. Most affected individuals have experience of the disease in its late stages because of their parent's and other f

Advance care planning in the end-of-life phase: coordinating between social worlds.

Advance care planning (ACP) for people in the last phase of life, living at home with complex care needs, is increasingly seen as a valuable practice to deliver patient-centred care and to prevent unnecessary hospitalizations and delay nursing home admissions. A notable difficulty of ACP is the coordination of ACP-practices over various domains involved in patient care (e.g. home care, GP care, hospital care). We draw on ethnographic data on ACP practices in home care, GP-care and hospital care in the Netherlands. We use social worlds theory to analyse the different information requirements of

Advanced care planning--empowering patients for a peaceful death.

In the early 1900's, Americans had a life expectancy of about 50 years. Childhood mortality was very high and an adult who lived into their sixties was considered to be doing pretty well. Prior to the advent of different types of antibiotics, people would die quickly of infectious disease or accidents and medicine only really focused on caring and comfort. Since then, there has been a shift in medicines focus. New science, technology and communications have shifted the way Americans treat incurable diseases and have promoted the idea of aggressive fighting as well as to keep patients alive at

Experiences, barriers and enablers of stroke end-of-life care: a systematic review with mixed-methods synthesis.

BACKGROUND: Despite advances in stroke care, 12% of patients die within 30 days of stroke highlighting the requirement for high-quality end-of-life care (EoLC) within stroke services. EoLC aims to improve the quality-of-life of patients facing death whilst also supporting their families. EoLC after stroke can be difficult due to complex stroke-specific challenges and little clinical guidance. There is a lack of knowledge and understanding of current provision of poststroke EoLC and the barriers and enablers to quality EoLC. Therefore, the aim of this review was to identify the experiences, exp

Baruch Spinoza and the Ethics of Care: Towards A Monistic Understanding of Spiritual Care at the End of Life.

Dualism has shaped current healthcare based on the dichotomies of body-mind, science-spirituality and life-death, consolidating a biomedical paradigm that treats the patient as an object, marginalising the spiritual dimension. This article explores Baruch Spinoza's philosophy as an alternative to the ethics of nursing care, integrating the physical, emotional, and spiritual dimensions within the same ontological continuum. Through a philosophical-conceptual approach inspired by hermeneutic interpretation, key concepts from the work Ethics are presented, inviting the reader to reflect on them i

Pruritus, Fever, and Sweats at the End of Life: Nursing Assessment and Management Considerations.

Pruritus, fever, and sweats are frequently observed thermoregulatory symptoms among patients with advanced chronic illnesses, especially during the terminal phase. These symptoms can substantially affect patient comfort and may contribute to fatigue, delirium, sleep disturbances, and lead to increased caregiver burden. Nurses play a crucial role in distinguishing between reversible and irreversible causes of pruritus, fever, and sweats. They are essential in implementing evidence-based interventions focused on patient comfort that align with the goals of care for both patients and families. Th

Understanding and addressing factors affecting carers' mental health during end-of-life caregiving: synopsis of meta synthesis of literature and stakeholder collaboration.

BACKGROUND: Family carers provide crucial support to patients nearing end of life. This can affect carers' own mental health negatively. It is important to understand what factors may affect carers' mental health and convey this information to stakeholders who can effect change. We conducted reviews of the qualitative, observational and intervention carer literature and worked with carer advisors and other stakeholders to make findings useful and accessible. AIM OF THE SYNOPSIS: To provide a synopsis of (1) project methods, (2) findings and implications from the evidence syntheses, (3) outcom

Attachment at the end of life: A systematic review.

BACKGROUND: The diagnosis of an advanced life-threatening illness brings with it existential challenges that activate the attachment system and different attachment styles influence coping with advanced illness. OBJECTIVES: The objective of this work were (a) to analyze the influence of attachment styles of patients with advanced disease and their relatives on emotional distress and other psychological and existential aspects, and (b) to identify the most used assessment instruments to measure it, highlighting those with better psychometric properties in palliative care contexts. METHODS: Ar

Relevance and consequence of economic and social resources of severely ill persons and their informal caregivers at the end-of-life: a systematic review of qualitative studies.

BACKGROUND: Life-limiting illnesses often lead to complex care needs and multifaceted support requirements for both severely ill persons and their informal caregivers, particularly at the end of life. The extent to which these needs are met may be significantly influenced by available economic and social resources. However, an in-depth understanding of how such resources shape end-of-life experiences is still missing. Therefore, our systematic review of qualitative studies aimed to explore and understand the perceptions of severely ill persons at the end of life and their informal caregivers r

Understanding the Policy Landscape Surrounding Medical Assistance in Dying in Canada's Federal Prison System.

Medical assistance in dying (MAiD) has been legal in Canada since 2016, and the implementation of MAiD for people who are incarcerated has raised ethical and procedural concerns. In this article, we review the current Correctional Service Canada guideline on MAiD alongside a joint report by the Office of the Correctional Investigator (OCI) and the Canadian Human Rights Commission (CHRC) on aging and dying in prison. We echo concerns raised by the OCI and the CHRC about the limits of adequate end-of-life care currently provided to those in custody and offer our analysis of the procedural guidel

End-of-life delirium: issues regarding recognition, optimal management, and the role of sedation in the dying phase.

CONTEXT: In end-of-life care, delirium is often not recognized and poses unique management challenges, especially in the case of refractory delirium in the terminal phase. OBJECTIVES: To review delirium in the terminal phase context, specifically in relation to recognition issues; the decision-making processes and management strategies regarding its reversibility; the potential refractoriness of delirium to symptomatic treatment; and the role of sedation in refractory delirium. METHODS: We combined multidisciplinary input from delirium researchers and knowledge users at an international deli
