The right thalamic ventral posterolateral nucleus seems to be determinant for macrosomatognosia: a case report.

BACKGROUND: Macrosomatognosiais the illusory sensation of a substantially enlarged body part. This disorder of the body schema, also called "Alice in wonderland syndrome" is still poorly understood and requires careful documentation and analysis of cases. The patient presented here is unique owing to his unusual macrosomatognosia phenomenology, but also given the unreported localization of his most significant lesion in the right thalamus that allowed consistent anatomo-clinical analysis. CASE PRESENTATION: This 45-years old man presented mainly with long-lasting and quasi-delusional macrosom

Tremor habituation to deep brain stimulation: Underlying mechanisms and solutions.

DBS of the ventral intermediate nucleus is an extremely effective treatment for essential tremor, although a waning benefit is observed after a variable time in a variable proportion of patients (ranging from 0% to 73%), a concept historically defined as "tolerance." Tolerance is currently an established concept in the medical community, although there is debate on its real existence. In fact, very few publications have actually addressed the problem, thus making tolerance a typical example of science based on "eminence rather than evidence." The underpinnings of the phenomena associated with

Auditory thalamic reticular nucleus of the rat: anatomical nodes for modulation of auditory and cross-modal sensory processing in the loop connectivity between the cortex and thalamus.

The auditory sector of the thalamic reticular nucleus (TRN) plays a pivotal role in gain and/or gate control of auditory input relayed from the thalamus to cortex. The TRN is also likely involved in cross-modal sensory processing for attentional gating function. In the present study, we anatomically examined how cortical and thalamic afferents intersect in the auditory TRN with regard to these two functional pathways. Iontophoretic injections of biocytin into subregions of the auditory TRN, which were made with the guidance of electrophysiological recording of auditory response, resulted in re

Visual discrimination in the pigeon (Columba livia): effects of selective lesions of the nucleus rotundus.

The nucleus rotundus is a large thalamic nucleus in birds and plays a critical role in many visual discrimination tasks. In order to test the hypothesis that there are functionally distinct subdivisions in the nucleus rotundus, effects of selective lesions of the nucleus were studied in pigeons. The birds were trained to discriminate between different types of stationary objects and between different directions of moving objects. Multiple regression analyses revealed that lesions in the anterior, but not posterior, division caused deficits in discrimination of small stationary stimuli. Lesions

Thalamic neuron theory: theoretical basis for the role played by the central nervous system (CNS) in the causes and cures of all diseases.

The Thalamic Neuron Theory (TNT) postulates that the central nervous system (CNS) is involved in all disease processes, as the CNS not only processes incoming physical and chemical information from the periphery, it also sends out physiological commands to the periphery in order to maintain homeostasis for the entire body. Inherent in its capacity to learn and adapt (i.e. to habituate) is the CNS' ability to learn to be sick (pathological habituation) by looking in certain deranged central neural circuitries, leading to chronic disease states. These pathologically habituated states can be reve

Stereotactic thalamotomy for dystonic patients.

A total of 54 patients (22 females and 32 males) with dystonia underwent thalamotomy. These patients consisted of 25 dystonia musculorum deformans (DMD) and 29 secondary dystonia cases. All of the patients were analyzed prospectively. Preoperative and postoperative symptoms were compared and grade. The target areas of thalamotomy were the ventral intermediate (Vim) and posterior ventral oral (Vop) nuclei. 59% of the patients showed more than 25% improvement, 23% were slightly improved cases (less than 25%), and 16% showed no change or their symptoms had worsened.

Control of tremor and involuntary movement disorders by chronic stereotactic stimulation of the ventral intermediate thalamic nucleus.

The authors report on the long-term results of chronic stereotactic stimulation of the ventralis intermedius thalamic nucleus performed in 14 cases of disabling and intractable tremor. There were 10 patients with parkinsonian tremor and four with essential tremor. Three of the 10 parkinsonian patients had previously undergone contralateral thalamotomy. Tremor was assessed by clinical evaluation, surface electromyography, accelerometer, and videotape recordings before and after stimulation. The deep-brain electrode was implanted in the ventralis intermedius nucleus according to stereotactic pro

Extensive dual innervation and mutual inhibition by forelimb and hindlimb inputs to ventroposterolateral nucleus projection neurons in the rat.

The stimulation of brachial plexus and sciatic nerve resulted in a precisely timed, synchronous volley of inputs to ventroposterolateral (VPL) neurons from either forelimb or hindlimb. Such stimulation activated sensory fibers of all modalities and was therefore modality-nonspecific. Extracellular recordings of modality-nonspecific single-unit evoked responses from VPL showed that 13% of VPL projection neurons responded to both forelimb and hindlimb inputs. We also demonstrated mutually inhibitory interactions between inputs from forelimb and hindlimb in 45% of VPL units. Unlike the somatotopi

Saccadic eye movements following kainic acid lesions of the pulvinar in monkeys.

Behavioral and anatomical experiments have suggested that the pulvinar might play a role in the generation of saccadic eye movements to visual targets. To test this idea, we trained monkeys to make visually-guided saccades by requiring them to detect the dimming of a small target. We used three different saccade paradigms. On single-step trials, saccades were made from a central fixation point (FP) to a target at 12, 24 or 36 degrees to the left or right. On overlap trials, the FP remained lit during presentation of a target at 12 or 24 degrees. On double-step trials, the target stepped first

Further physiological observations on the ventralis intermedius neurons in the human thalamus.

1. During the course of stereotaxic thalamotomy for 56 cases with tremor mainly due to Parkinson's disease and essential tremor, extracellular recordings were made from the thalamic ventralis intermedius (Vim) nucleus under local anesthesia. These procedures have been justified as an essential technique to achieve the best therapeutic results by a selective coagulation. These physiological observations provide important information about the functional organization of the ventrolateral thalamic mass in humans. 2. Using Leksell's stereotaxic apparatus, a pair of semimicroelectrodes was introduc

Topographical projections from the posterior thalamic regions to the striatum in the cat, with reference to possible tecto-thalamo-striatal connections.

Projections from the posterior thalamic regions to the striatum were studied in the cat by the anterograde tracing method after injecting wheat germ agglutinin-horseradish peroxidase conjugate (WGA-HRP) into the caudalmost regions of the lateroposterior thalamic nucleus (caudal LP), suprageniculate nucleus (Sg) and magnocellular division of the medial geniculate nucleus (MGm). The results were further confirmed by the retrograde tracing method after injecting WGA-HRP into the regions of the caudate nucleus (Cd) and putamen (Put) where afferent fibers from the caudal LP, Sg and MGm were distrib

[On the central-nervous localization of the anosmia-ageusia-syndrome (author's transl)].

After head-injury about 50% of our own patients show combined lesion of the senses of smell and taste. The discussion of the localization of the responsible defect is not closed. The results of a chemosensory test battery indicate that the thalamic nucleus ventroposteromedialis belongs to the damaged area. This nucleus is in contact with the gustatory tract, with mechanoreceptive impulses out of the upper airways, and with olfactory sensations.

Localized effects of naloxone on local cerebral glucose utilization in rat cerebral nuclei with Met-enkephalinergic neurons.

The alterations of local glucose utilization in 101 cerebral nuclei following the subcutaneous administration of the specific opioid antagonist naloxone were measured using the quantitative autoradiographic 14C-deoxy-D-glucose technique in conscious rats. Met5-enkephalin-like immunoreactivity (MELI) and ME receptor binding (MERB) levels in 82 cerebral nuclei were assayed quantitatively by microdensitometry of fluorescence micrographs and autoradiographs of the brain slices. Naloxone administration significantly reduced glucose utilization rate in 18 lower brainstem nuclei including the n. trac

Thalamic afferents to the visual cortex in congenitally anophthalmic mice.

Retrograde cell labelling with horseradish peroxidase (HRP) has been used to study the thalamic afferents to the primary visual cortex (area 17) in mutant H1-ZRDCT-An ('eyeless') mice, in which the eyes are missing throughout development. Injections of HRP that were localised to a subregion of area 17, resulted in the labelling of a group of neurons in the ipsilateral dorsal lateral geniculate nucleus, showing the existence of a point-to-point connectivity. Many labelled cells were also found in other posterior thalamic nuclei especially in the lateral posterior nucleus which in normal animals…

Object Naming After Thalamic Damage: Evidence From a Large-Scale, Chronic-Phase Study of Left Hemisphere Stroke Survivors.

Dynamics of directional coupling underlying spike-wave discharges.

PURPOSE: Spike and wave discharges (SWDs), generated within cortico-thalamo-cortical networks, are the electroencephalographic biomarker of absence epilepsy. The current work aims to identify mechanisms of SWD initiation, maintenance and termination by the analyses of dynamics and directionality of mutual interactions between the neocortex and various functionally different thalamic nuclei. METHODS: Local-field potential recordings of 16 male Wistar Albino Glaxo from Rijswijk (WAG/Rij) rats, equipped with electrodes targeting layer 4-6 of the somatosensory cortex, rostral and caudal reticular…
