Acute and Chronic Management in an Atypical Case of Ethylmalonic Encephalopathy.

Ethylmalonic encephalopathy (EE) is caused by mutations in the ETHE1 gene. ETHE1 is vital for the catabolism of hydrogen sulfide (H2S). Patients with pathogenic mutations in ETHE1 have markedly increased thiosulfate, which is a reliable index of H2S levels. Accumulation of H2S is thought to cause the characteristic metabolic derangement found in EE. Recently introduced treatment strategies in EE, such as combined use of metronidazole (MNZ) and N-acetylcysteine (NAC), are aimed at lowering chronic H2S load. Experience with treatment strategies directed against acute episodes of metabolic decomp

Effect of procaine hydrochloride on cyanide intoxication and its effect on neuronal calcium in mice.

Cyanide intoxication in mice can be antagonized by the local anesthetic, procaine HCI, either alone or in combination with sodium nitrite and/or sodium thiosulfate. Antitoxic potency, derived from LD50 values, was compared in groups of mice pretreated with sodium nitrite (43.9 mg/kg i.p.), sodium thiosulfate (1 g/kg i.p.), and procaine HCI (37.0 mg/kg i.p.) either alone or in various combinations. The results of present study indicated that procaine HCI provides a significant protection against the lethal effects of potassium cyanide. Dose-response studies revealed procaine hydrochloride pretr

[Biooxidation of gold-bearing sulfide ore and subsequent biological treatment of cyanidation residues].

The percolation biooxidation parameters of ore from the Bakyrchik deposit were studied. An investigation of the technological parameters (such as the concentration of leaching agents, irrigation intensity, and pauses at various stages of the leaching) revealed the optimal mode for precious metal extraction. The stages of the ore processing were biooxidation, gold extraction by cyanidation or thiosulfate leaching, and biological destruction of cyanide. The gold and silver recovery rates by cyanidation were 64.0 and 57.3%, respectively. The gold and silver recovery rates by thiosulfate leaching

Fluorometric determination of cyanide in biological fluids and pyridoxal.

Investigations of the physiological disposition of cyanide during thiosulfate therapy has necessitated the development of a convenient and sensitive method for cyanide analysis. A fluorometric method involving the catalytic conversion of pyridoxal to 4-pyridoxylactone was adapted for use with biological fluids by employing microdiffusion analysis. The presence of the cyanide antagonist, sodium thiosulfate, interferes with the formation of the fluorophore. In order to circumvent this interference, the pH of the diffusion media was altered to selectively diffuse cyanide. After testing various ac

Inhibition of rat liver rhodanese by di-, tricarboxylic, and alpha-keto acids.

Rat liver rhodanese [EC 2.8.1.1] purified by ammonium sulfate fractionation, CM-cellulose and Sephadex G-200 chromatography yielded two active fractions (I & II). Their molecular weights were estimated to be 1.75 X 10(4) (I) and 1.26 X 10(4) (II) by the gel filtration method. Kinetic studies revealed that Fraction I rat liver rhodanese catalyzes thiocyanate formation from thiosulfate and cyanide by a double displacement mechanism. Carboxylic acids such as DL-isocitric, citric malic, pyruvic, and oxaloacetic acid were competitive inhibitors with respect to thiosulfate, whereas fumaric, succinic

The uptake and excretion of partially oxidized sulfur expands the repertoire of energy resources metabolized by hydrothermal vent symbioses.

Symbiotic associations between animals and chemoautotrophic bacteria crowd around hydrothermal vents. In these associations, symbiotic bacteria use chemical reductants from venting fluid for the energy to support autotrophy, providing primary nutrition for the host. At vents along the Eastern Lau Spreading Center, the partially oxidized sulfur compounds (POSCs) thiosulfate and polysulfide have been detected in and around animal communities but away from venting fluid. The use of POSCs for autotrophy, as an alternative to the chemical substrates in venting fluid, could mitigate competition in t

[Oxidation of inorganic sulfur compounds by obligatory organotrophic bacteria].

New data obtained by the author and other researchers on two different groups of obligately heterotrophic bacteria capable of inorganic sulfur oxidation are reviewed. Among culturable marine and (halo)alkaliphilic heterotrophs oxidizing sulfur compounds (thiosulfate and, much less actively, elemental sulfur and sulfide) incompletely to tetrathionate, representatives of the gammaproteobacteria, especially from the Halomonas group, dominate. Some of denitrifying species from this group are able to carry out anaerobic oxidation of thiosulfate and sulfide using nitrogen oxides as electron acceptor

Insights into the Versatile Sulfur Metabolism of Sulfurovum sp. MH2-6 Isolated from Deep-Sea Hydrothermal Vent Environments.

In deep-sea hydrothermal ecosystems, inorganic sulfur compounds serve as key energy sources for microbes through oxidation, reduction, or disproportionation reactions. However, to date, the bacteria that disproportionate sulfur remain poorly understood. Here, we characterized the physiological and metabolic characteristics of Sulfurovum sp. MH2-6, which was isolated from hydrothermal sediments of the South Mid-Atlantic Ridge. Based on the results of 16S rRNA gene sequence, average nucleotide identity, and DNA-DNA hybridization value, strain MH2-6 belonged to the same species as Sulfurovum mang

Effects of various known and potential cyanide antagonists and a glutathione depletor on acute toxicity of cyanide in mice.

To compare the protective potencies of a large number of known and potential cyanide antagonists in one stock of mice, groups (N = 10) of male CF-1 Swiss-Webster mice were given a single maximal or near-maximal intraperitoneal injection of each substance. Ethyl maleate, a glutathione (GSH) depletor and potential enhancer of cyanide toxicity, was given to other groups. Thirty min later, the mice were given subcutaneous injections of graded doses of KCN. In untreated control mice, the 24-hr median lethal dose (LD50) of KCN was 11 mg/kg of body weight (potency ratio, PR = 1.0). In comparison, pro

Treatment of vascular calcification.

A variety of potential therapies for vascular calcification, based either on the underlying biology or physical chemistry or solely on empiric observations in patients, may be effective but lack rigorous testing. Pasch et al. provide convincing evidence that sodium thiosulfate prevents medial vascular calcification in uremic rats. Although this provides some scientific basis for the clinical use of thiosulfate, uncertainty about mechanism of action and safety still remains.

Effects of protein-free diet and food deprivation on hepatic rhodanese activity, serum proteins and acute cyanide lethality in mice.

This study was undertaken to evaluate changes in hepatic and extra-hepatic cyanide detoxication resulting from nutritional deficits and any parallel changes in cyanide sensitivity. Male CD-1 mice were fed control diet, control diet in reduced quantities or protein-free diet. After 14 d body weight and relative liver size were lower in the protein-free diet and the food-deprived groups than in the control group. Hepatic and serum protein and albumin concentrations were lower in the protein-free diet group and hepatic protein and albumin concentrations were higher in the food-deprived group than

Properties of Azotobacter vinelandii rhodanese.

Rhodanese (thiosulfate: cyanide sulfurtransferase, E.C. 2.8.1.1) was purified from the nitrogen fixing organism Azotobacter vinelandii, and its amino acid composition was determined. The enzyme is a single polypeptide chain of M(r) 29,000 which showed an apparent pI of 4.7 and no presence of isoenzymes. Steady-state initial velocity measurements indicate that the enzyme catalyzes the transfer of sulfane sulfur of thiosulfate either to cyanide or to dihydrolipoate by way of a double displacement mechanism.

Effects of oxygen on the antagonism of cyanide intoxication: cytochrome oxidase, in vitro.

Since oxygen was reported to be an effective cyanide antagonist in vivo, particularly in the presence of the classic antidotal combination of sodium nitrite and sodium thiosulfate, in vitro studies were initiated in an attempt to investigate the mechanism of oxygen-mediated cyanide antagonism. The effect of oxygen on cyanide-inhibited cytochrome oxidase with and without cyanide antagonist(s) was investigated in a purified membraneous enzyme system prepared from rat liver mitochondria. Cyanide produced a concentration dependent inhibition of cytochrome oxidase, and 100% oxygen did not alter the

Syntrophic Interaction between an Anoxygenic Photosynthetic Bacterium and a Tetrathionate-reducing Bacterium in Anaerobic Benzoate Degradation.

The present study exami-ned bacteria that anaerobically degrade the aromatic compound, benzoate, and obtained enrichment cultures from marine sediments under illumination. The enrichment culture contained anoxygenic photosynthetic bacteria and non-photosynthetic bacteria. The photosynthetic strain PS1, a purple sulfur bacterium in the genus Marichromatium, was unable to utilize benzoate; however, when combined with the non-photosynthetic bacterial isolate, Marinobacterium sp. strain BA1, the co-culture grew anaerobically on benzoate in the presence of thiosulfate or tetrathionate. Based on the

Diagnosis and treatment of calciphylaxis.

Calciphylaxis occurs due to calcium deposition in arterioles, which leads to ischemic ulceration of overlying skin. Two-year mortality rates from sepsis ranges from 50% to 80%. Calciphylaxis is most common in hyperparathyroidism secondary to chronic renal impairment and rarely occurs in the setting of normal renal function. Biopsy of the calciphylaxis ulcer reveals calcium deposits lining the vascular intima. Tissue calcification may also be seen on plain radiographs. Calcium-phosphate metabolism should be normalized by treating any underlying hyperparathyroidism with bisphosphonates, parathyr

Structural and Mechanistic Insights into Hemoglobin-catalyzed Hydrogen Sulfide Oxidation and the Fate of Polysulfide Products.

Hydrogen sulfide is a cardioprotective signaling molecule but is toxic at elevated concentrations. Red blood cells can synthesize H2S but, lacking organelles, cannot dispose of H2S via the mitochondrial sulfide oxidation pathway. We have recently shown that at high sulfide concentrations, ferric hemoglobin oxidizes H2S to a mixture of thiosulfate and iron-bound polysulfides in which the latter species predominates. Here, we report the crystal structure of human hemoglobin containing low spin ferric sulfide, the first intermediate in heme-catalyzed sulfide oxidation. The structure provides mole

Cyanide poisoning from propionitrile exposure.

A 55-year-old worker was exposed to propionitrile by both the inhalational and dermal routes. He rapidly lost consciousness and developed metabolic acidosis consistent with cyanide poisoning. Treatment with hydroxycobalamin and sodium thiosulfate produced complete symptom resolution within an hour. Serial blood cyanide and thiocyanate levels documented a rapid decrease in cyanide and concomitant increase in thiocyanate. Serious cyanide poisoning can occur from propionitrile exposure. Treatment with hydroxycobalamin and sodium thiosulfate can produce rapid clinical improvement.
