Phrenic nerve injury following scalenectomy in a patient with thoracic outlet obstruction.

We present a case in which a patient with normal pulmonary reserve experienced orthopnea and hypoxia secondary to unilateral diaphragmatic paralysis following right scalenectomy. This operation was performed in an attempt to relieve neurovascular compromise at the thoracic outlet. To our knowledge, this association has not been previously described in the literature.

[Changes in systemic pressure during experimental injury of the thoracic spinal cord].

The hemodynamic changes in experimental injury of spinal medulla were known as studied under the general anesthesia. Authors performed the thoracic spinal cord impact trauma in rabbits. They stated the character of this "sympathetic shock" is the same as in narcosis. Immediately after the injury, the phase of increase in systemic pressure occurred which lasted for 10 s, then the phase of decrease in pressure followed lasting 10 min. In spite of significant pressure changes (p less than 0.05) which occurred through the experiment, these changes were not so large to override the autoregulatory c

Sarns centrifugal pump for repair of thoracic aortic injury: case reports.

A new centrifugal pump (Sarns), originally designed for ventricular assist, was successfully used in two patients during repair of traumatic pseudoaneurysm of the descending thoracic aorta. The distal thoracic aorta was perfused without heparinization to avoid spinal cord and visceral ischemia, reduce afterload on the heart, and avoid clamp injury to the aorta. Distal mean aortic pressure was maintained above 50 mm Hg with a mean pump flow of 1.75 liter/minute. Proposed structural advantages of the Sarns centrifugal pump for perfusion of the distal thoracic aorta without heparin are resistance

Prevention of spinal cord injury after cross-clamping of the thoracic aorta.

Paraplegia has been a devastating and unpredictable complication following surgical procedures involving temporary occlusion of the thoracic aorta. This study was undertaken to determine the effect of the pressure gradient between the aortic pressure distal to the occluding aortic clamp and cerebrospinal fluid pressure, defined as "Relative spinal cord perfusion pressure" (RSPP) on the development of the ischemic spinal cord injury. In twelve mongrel dogs, the thoracic aorta just distal to the left subclavian artery was cross-clamped. Somatosensory evoked potentials (SEP) were generated by per

Interhospital transfer of older adults with thoracic injury in Türkiye: a nationwide study.

INTRODUCTION: The increasing older adult population is raising the burden of trauma-related healthcare services, with thoracic trauma resulting in high mortality and long hospital stays in this group. Therefore, appropriate triage and interhospital transfer are critical for improving clinical outcomes. The aim of this study is to evaluate the data on the referral patterns of older adults with thoracic trauma in Turkey who were transferred by road and air. MATERIALS AND METHODS: The patients' age, gender, nationality, referring-receiving province and region, referral time, distance, and ICD-10

Multicompartment Injury Risk in High-Rate Nonpenetrating Blunt Thoracic Impacts.

The occurrence of blunt abdominal injuries resulting from thoracic high-rate nonpenetrating blunt impacts (NPBIs) is often missed and associated with increased mortality and morbidity. Diagnosis of penetrating gunshot wounds successfully predicts injury locations using bullet trajectory, but no similar correlation has been applied for blunt impacts. Historically, thoracic NPBIs have been studied on ovine subjects and have reported thoracic only and thoracoabdominal injuries for impacts at the same location. The purpose of this study is to investigate if the finite element ovine thorax model (F

Long-Term Survival following TEVAR for Blunt Thoracic Aortic Injury.

BACKGROUND: Blunt thoracic aortic injury (BTAI) is a leading cause of death in blunt chest trauma. Thoracic endovascular aortic repair (TEVAR) has emerged as the preferred treatment, offering reduced morbidity and improved survival compared with open repair. This study evaluates long-term outcomes of TEVAR for BTAI at a single Canadian center. METHODS: We retrospectively reviewed 46 patients with BTAI treated with TEVAR at McGill University from 2002 to 2025. Data collected included demographics, associated injuries, Society for Vascular Surgery (SVS) grade, aortic zone, procedural details, a

Paraplegia caused by a thoracic angiolipoma obscured by cord injury: a case report.

BACKGROUND: Spinal epidural angiolipomas are rare benign tumors comprising 0.5-1% of all spinal neoplasms and typically arise in the thoracic spine, causing progressive myelopathy due to spinal cord compression. They feature mature adipocytes and vascular proliferation, show heterogeneous magnetic resonance imaging (MRI) enhancement, and may mimic posttraumatic or degenerative myelopathy, particularly in patients with prior spinal cord injury. CASE DESCRIPTION: A 56-year-old man presented with a 3-month history of progressive numbness and weakness in both lower limbs. He had sustained a cervi

Thoracic Duct Injury in a Patient Undergoing Axillary Lymphadenectomy: A Case Report.

A 56-year-old female patient, with no significant comorbidities, presented with abnormal breast exam findings. Imaging revealed a 5.4-cm irregular nodule in the left breast, diagnosed as invasive breast carcinoma (NST, Grade 2). Neoadjuvant chemotherapy was initiated, leading to a reduction in lesion size. Surgical intervention included quadrantectomy, sentinel lymph node biopsy, and axillary lymphadenectomy, which revealed residual carcinoma and positive lymph nodes. Postoperatively, chylous drainage through a Portovac drain was observed, prompting reoperation, during which the injured lympha

Endovascular Repair Blunt Thoracic Aortic Injury in Adolescent: A Case Series.

BACKGROUND: Blunt traumatic aortic injury (BTAI) is a rare occurrence in adolescents, yet it is associated with a high mortality rate necessitating immediate treatment. Although endovascular repair has become the preferred treatment for such injuries in adults, its effectiveness in adolescents remains uncertain. CASE SUMMARY: Blunt traumatic aortic injury typically presents with concomitant injuries to other organs and carries a high perioperative mortality rate with operative repair (OR). In this report, we describe the treatment of 3 clinical cases of BTAI in adolescents using thoracic endo

A non-lethal left sided thoracic impalement injury: A case report.

INTRODUCTION AND IMPORTANCE: Impalement injuries are those injuries that result from the injuring object or weapon being stuck on to the victim's body parts. Such cases occur rarely and when they do they pose a great challenge starting from transportation to anesthesia induction and surgical decision. The extremities are the commonest parts of the body where this occurs. Only a few reports of thoracic impalement injuries have been documented in the literature. CASE PRESENTATION: Here we present a case of a 25 years old male patient who presented 36 hours after sustaining an impalement injury

Survival Model of Thoracic Contusion Spinal Cord Injury in the Domestic Pig.

Spinal cord injury (SCI) frequently results in motor, sensory, and autonomic dysfunction for which there is currently no cure. Recent pre-clinical and clinical research has led to promising advances in treatment; however, therapeutics indicating promise in rodents have not translated successfully in human trials, likely due, in part, to gross anatomical and physiological differences between the species. Therefore, large animal models of SCI may facilitate the study of secondary injury processes that are influenced by scale, and may assist the translation of potential therapeutic interventions.

Costal cartilage fracture: A commonly missed thoracic injury in trauma patients.

In this report, we describe a case of a 43-year-old male who sustained a displaced mid chondral costal cartilage fracture along with multiple ipsilateral rib fractures following a jet ski collision. Costal cartilage fractures (CC) are commonly missed on imaging evaluation, and accordingly underreported in the literature. High-energy blunt chest trauma represents the most common mechanism for CC injuries. Computed Tomography (CT) is the modality of choice to diagnose CC fractures along with the associated cardiopulmonary and abdominal injuries in polytrauma patients. There is currently no conse

A Rare Case of Spinal Cord Injury Following Thoracic Radiofrequency Ablation.

Medial branch radiofrequency ablation (RFA) has become a common treatment for facet-related back pain. While this procedure is often performed in the lumbar and cervical spinal segments, it can also be applied to the thoracic spine. Complications of spinal RFA at any level are scarce in the literature and are often mild. The patient is a 37-year-old male with a family history of multiple sclerosis in his siblings who underwent thoracic RFA at the T2-T5 nerve root levels. Within one week of the procedure, the patient experienced paresthesias below the nipple line and progressive lower-extremity

[Rupture of the descending thoracic aorta following sudden traumatic deceleration injury].

Acute post-traumatic rupture of the thoracic aorta is the second cause of road traffic deaths; 80% of patients die at the scene of the accident. The involvement of its descending portion outside the isthmus is rare and it is estimated to be less than 3%. We report the case of a 60-year old man, victim of road traffic accidents (RTA) whose mechanism was: the motorcyclist was struck by a car; the points of impact included the right upper limb and the chest. This RTA caused a fracture in both bones of the forearm and a rupture of the descending thoracic aorta (ATD). Treatment was based in the fir
