Cervicomediastinal Hematoma: Atypical Presentation of a Parathyroid Carcinoma.

Parathyroid carcinoma (PC) is a rare endocrine neoplasm that typically presents with osteopenia/osteoporosis, nephrolithiasis, asthenia, and neuropsychiatric symptoms. We describe the case of a 48-year-old woman, presenting with a large painful hematoma in the cervicomediastinal area. The neck ultrasound (US) demonstrated a solid lesion measuring 40 × 80 × 55 mm, markedly hypoechoic, which extended from the right thyroid lobe to the mediastinum. The blood tests showed elevated serum calcium and parathyroid hormone (PTH) concentrations, consistent with hypercalcemic primary hyperparathyroidism.

Intrathyroidal Clear Cell Tumor of Parathyroid Origin with Review of Literature.

Water-clear cell adenoma (WCCA) of the parathyroid gland is an exceedingly rare neoplasm. To date, 17 cases have been reported in the literature, with only one of them being intrathyroidal. Here we report a case of a 34-year-old woman who presented for evaluation of a goiter and was found to have a thyroid nodule and abnormal thyroid function tests (TFT). Fine needle aspiration biopsy of the nodule revealed thyroid follicular cells without atypia and subsequent Afirma® Gene Expression Classifier (GEC) testing results were suspicious for malignancy. As a result, the patient underwent a right th

Struma ovarii maligna.

INTRODUCTION: Malignant struma ovarii is a rare ovarian neoplasm composed predominantly of mature thyroid tissue. CASE REPORT: A right ovarian tumor was discovered at ultrasound examination in a 20-year-old woman. Complete right ovariectomy was done - histopathological examination revealed papillary thyroid carcinoma arising in struma ovarii (malignant struma ovarii). Patient underwent subsequent total thyroidectomy - the thyroid was found to be without any pathological lesions. After operations the patient received ablative radioiodine treatment (200 mCi 131I). An 131I posttherapeutic whole-

Adenochondroma of the Thyroid in a 3-Year-Old Female: A Case Report.

Follicular adenomas are the most common benign thyroid neoplasm but are unusual in children. However, other rare degenerative lesions and those of developmental origin can also present as thyroid masses. This article reports the first described pediatric thyroid adenochondroma. A 3-year-old female presented with a hard mass in the right lobe of her thyroid with nondiagnostic imaging and cytology findings. She underwent a right thyroid lobectomy uneventfully. Final histopathology examination confirmed an adenochondroma. To the best of our knowledge, an adenochondroma of the thyroid gland in a c

Hashimoto's Disease and Thyroid Cancer in Children: Are They Associated?

Hashimoto's thyroiditis (HT) is the most common cause of thyroid disease in children and adolescents. Along with significant modifications of thyroid function, HT in pediatric age can be accompanied by relevant thyroid structural alterations. Over time, benign thyroid nodules, carcinoma and, rarely, primary non-Hodgkin lymphoma can develop. However, the relationships between HT and neoplasms are poorly defined. The main aim of this paper is to discuss what is presently known regarding the coexistence of HT and thyroid tumors. Moreover, we attempt to define the pathogenesis of cancer developmen

Actinomycotic thyroiditis in a child.

The authors present a unique case of actinomycosis occurring in a 10-year-old girl who presented with a painless, noninflammatory left neck mass of 3 weeks duration. Physical examination findings showed a firm, nontender mass within the left thyroid lobe, and a presumptive diagnosis of thyroid neoplasm was made. Inflammatory changes subsequently occurred and surgical drainage was performed revealing sulphur granules. Histological examination confirmed actinomycosis of the thyroid gland.

Secondary carcinoma of the thyroid gland.

When the cervical tumor mimics thyroid carcinoma, some cases are shown to be the primary carcinoma of other site. Four cases of secondary thyroid carcinoma, of which primary sites are adjacent organ, are studied. The characteristics of histology are squamous cell carcinoma. Squamous cell carcinoma of which primary organ seems to be thyroid often discloses metastatic neoplasm originated from the adjacent organs. Since chronically inflammatory lesion is recognized concurrently, abnormal thyroid is suggested to be congenial soil for malignant growth. Our experience indicates that the secondary ca

[Update on follicular thyroid cancer-What is relevant for surgeons?].

Follicular thyroid cancer (FTC) is the second most frequent form of differentiated thyroid cancer, accounting for approximately 5-15% of all thyroid malignancies. According to the World Health Organization (WHO) classification from 2022 FTC is divided into three subtypes, which clearly differ in the overall prognosis and probability of recurrence. Furthermore, knowledge about benign follicular tumors and low-grade neoplasms is important. Prognostic factors for malignant alterations include patient age, tumor size, invasive growth, the presence of angioinvasion and the occurrence of distant met

Primary Mammary Analog Secretory Carcinoma (MASC) of the Vulva With ETV6-NTRK3 Fusion: A Case Report.

Mammary analog secretory carcinoma is a primary salivary gland neoplasm with histologic, immunophenotypic, and molecular features identical to those of secretory carcinoma of the breast. Similar neoplasms have now been reported to occur in various nonmammary sites including the parotid gland, submandibular gland, sinuses, lip, skin, thyroid gland, and lung. We report, to our knowledge, the first example of a primary vulvar neoplasm with pathologic features identical to secretory carcinoma of the breast and an ETV6-NTRK3 fusion.

[Thyroid maltoma: report of a case without coexistent Hashimoto's thyroiditis].

Maltomas of the thyroid are rare neoplasms that originate from parafollicular B lymphocytes and are often associated with autoimmune thyroiditis (Hashimoto's disease). The authors describe a case of a primary thyroid lymphoma in which no evidence for a previous thyroiditis could be found during clinical evaluation (clinical progression of the tumor, diagnostic studies, preoperative cytology, histology of the surgical specimen). Noteworthy was the development of a severe form of pure red cell aplasia (PRCA) about 6 months after the operation without detectable autoantibodies or signs of previou

Sclerosing mucoepidermoid carcinoma with eosinophilia of thyroid gland in a male patient: a case report and literature review.

Sclerosing mucoepidermoid carcinoma with eosinophilia (SMECE) was first described by Chan et al in 1991. It is characterized by nest or strands of epidermoid tumor cells with squamous differentiation, rare mucous cells, prominent sclerotic stroma, eosinophilic and lymphoplasmacytic infiltration, and a background of chronic lymphocytic thyroiditis in the non-neoplastic thyroid gland. It is important to recognize SMECE of thyroid and differentiate it from squamous cell carcinoma or other neoplasms with squamous differentiation/metaplasia. In published cases, the SMECE of thyroid gland predominan

Unusual tumors of the thyroid gland.

Thyroid neoplasms are classified into three major categories: epithelial, nonepithelial, and secondary. Most primary epithelial tumors of thyroid are derived from follicular cells. These include follicular adenoma and carcinoma (Hürthle and non-Hürthle), and papillary carcinoma and its variants. Other primary epithelial tumors include medullary carcinoma, mixed medullary and follicular carcinomas, insular and poorly differentiated carcinoma, anaplastic carcinoma, and the least common squamous carcinoma and related tumors. The nonepithelial tumors are rare; the most common include malignant lym…

Fine needle aspiration cytology of thyroid gland diseases.

From 1982 to 1987, 2,433 lesions of the thyroid gland in 1,796 patients were examined by fine needle aspiration (FNA). Cytopathology classified 66.91% of the aspirates as benign, 10.76% as thyroiditis, 4.89% as suspected (unspecified) neoplasia, 1.31% as positive for malignancy and 16.11% (392) as unsatisfactory. The histologic diagnoses in 257 cases were compared with cytologic diagnoses to determine the accuracy of FNA cytology of thyroid lesions, yielding a sensitivity of 71.43%, a specificity of 100% and an accuracy of 95.09%. This data strongly supports thyroid FNA as an important preoper

A close look at our cases with parathyroidectomy: 11 years of experience.

BACKGROUND: Hyperparathyroidism is caused by parathyroid adenoma, hyperplasia or carcinoma. Parathyroid adenomas are the most common cause of parathyroid disease (85-88%) while atypical parathyroid adenomas and carcinomas are the least frequently seen parathyroid neoplasms which cause diagnostic difficulty. This series aims to identifiy lesions of parathyroidectomy specimens in our center and draw attention to borderline cases in between parathyroid adenoma and carcinoma. METHODS: The study included 638 parathyroidectomy materials diagnosed in our center between the years 2005 and 2016 and ex…

Fine needle aspiration and the Bethesda system: Correlation with histopathology in 1,228 surgical patients.

BACKGROUND: Fine-needle aspiration combined with the Bethesda System for Reporting Thyroid Cytopathology is indispensable in the diagnostic evaluation of thyroid nodules. Their increased detection over the last few decades mandates the determination of which thyroid nodules require surgical management for malignancy. This study examines the correlation of fine-needle aspiration to final histopathology of dominant thyroid nodules in a large series of surgical patients undergoing thyroidectomy at a single academic institution. METHODS: A retrospective review of prospectively collected data of 1

An Uncommon Thyroid Tumor: The Diagnostic Challenge of Solitary Fibrous Tumors.

Background: Solitary fibrous tumors are uncommon fibroblastic neoplasms. These tumors are characterized by the recurrent NAB2-STAT6 gene fusion, which is a hallmark of solitary fibrous tumors (SFTs), particularly those arising in the thoracic cavity. While SFTs are mostly found in the abdomen and pleura, they can occur in various locations, including the head and neck region (6% of cases of SFTs). Solitary fibrous tumors of the thyroid (SFTTs) are extremely rare, accounting for only 0.1% of all thyroid tumors. The gold standard imaging modality for thyroid tumors is ultrasonography, even thoug
