Closing fasciotomy wounds using plastic bands: an alternative simple and cheap method.

Fasciotomy incisions are essential to relieve pressure on the neurovascular structures in the limbs. However, closing such wounds often becomes a challenge to the surgeon. The aim of this study is to describe a simple and cheap method of closing these wounds. Plastic bands were used to gradually close fasciotomy wounds in five patients. All fasciotomy wounds in the five patients closed successfully within 4-12 days. Only two patients developed minimal complications in the form of minor wound infection and a hypertrophic scar. We conclude that the plastic band method used here is cheaper and si

A case of popliteal pterygium treated along with nerve expansion.

The presence of a short sciatic nerve in the free edge of a popliteal pterygium makes this syndrome a surgical challenge. We present a case of popliteal pterygium that was treated by nerve expansion. The range of motion of the patient's knee joint was between 30 and 120 degrees. A 75-cc tissue expander was placed under the sciatic nerve and filled with 5 cc of saline solution weekly. When a total of 60 cc was reached, wound dehiscence was observed, and the procedure had to be stopped. The maximum extension obtained was 160 degrees. Since the expansion process had to be stopped early, the elong

[Indications for the use of a skin-stretching device].

In the management of surgical or traumatic skin loss the Authors consider an alternative method which involves the use of a tool called skin stretching device. The latter seems easy to apply, atraumatic and the least troublesome for the isolation and detaching of tissues employed in skin repair.

Management of a giant omphalocele with an external skin closure system.

BACKGROUND/PURPOSE: The management of neonates with giant omphalocele remains challenging and multiple strategies have been described. We present the case of a 34-week-old neonate with isolated giant omphalocele managed with an external surgical skin closure system as a component of a staged closure strategy. CASE PRESENTATION: An Inuit boy of 34 weeks gestation was born by urgent Caesarean delivery at an affiliated obstetrical hospital with a giant ruptured omphalocele and loss of abdominal domain. He was transferred to our institution and a silastic silo was fashioned and placed in the oper

A new surgical approach to exomphalos.

This report describes the use of tissue expansion for late reconstruction of giant exomphalos. An unexpected advantage was reconstruction of the anterior rectus sheath using the posterior capsule formed by the tissue expanders.

Surgical Techniques for the Repair of Webbed Neck: A Scoping Review.

Webbed neck is a rare condition characterized by bilateral subcutaneous bands, which extend from approximately the mastoid to the acromion. … dures described included: butterfly correction, V-Y Plasty, posterior cervical lift, skin excision, tissue expansion plus skin excision, T to M rearrangement, and subcutaneous fascial excision. Documented complications included hypertrophy of procedure scars or webbed neck recurrence. In summary, we present the first full literature review of the surgical techniqu …

An Overview of Pre-expanded Perforator Flaps: Part 2, Clinical Applications.

Pre-expanded perforator flaps have several advantages over their traditional counterparts owing to the thin, more pliable nature, larger size, and minimum morbidity of the donor site. Recently, plastic surgeons have begun to use pre-expanded perforator flaps to reconstruct defects of almost the entire body, including the cervicofacial region, axilla, trunk, and extremities resulting from scar, congenital melanocytic nevi, hemangiomas, and neurofibromas. Such a versatile flap is especially appropriate for face and neck resurfacing, which requires more optimal functional and cosmetic outcomes.

Osteophyte-induced rupture of a scalp tissue expander.

Tissue expansion is in widespread use for the reconstruction of congenital and acquired defects. Complications of tissue expansion are well documented, the most common being infection or extrusion of the expander. Although the complications associated with scalp tissue expansion are similar to those experienced elsewhere in the body, site-specific complications can occur. These include bone thinning and erosion of the cranial vault. In this study, we report an unusual case of multiple osteophyte formation beneath a scalp tissue expander, resulting in its rupture and necessitating removal.

Restoration of accommodation.

PURPOSE OF REVIEW: Pediatric cataract surgery remains a very important and difficult problem to manage. While dramatic advances have occurred in this field over the past 10 years, some technical aspects of surgery, changing refraction and functional outcome continue to pose significant problems. The aim of the present review is to update the reader on advances reported on the topic during the past year. RECENT FINDINGS: Manual capsulorhexis still remains a gold standard for the successful outcome of pediatric cataract surgery. Primary management of the posterior capsule is mandatory depending

[Skin expansion on the extremities].

The indications for skin expansion in the limbs are now well defined. This technique is particularly irreplaceable below the knee, and allows the resection of very large lesions despite a minimal skin capital. More than in other sites, it is a difficult technique which requires rigour and a sound basis in reconstructive surgery. The operative plan must include precise assessment of the tissue gain with selection of prostheses adapted to local conditions. The techniques of insertion, filling and resection must be irreproachable. The functional and aesthetic indications are numerous and the tech

Xiphoomphaloischiopagus tripus conjoined twins: successful separation following abdominal wall expansion.

A case of xiphoomphaloischiopagus tripus conjoined twins is reported. Particular emphasis was paid to the large abdominal wall defect that would be produced by separation, and the complications resulting from the use of intraperitoneal Silastic tissue expanders are described. As a result of adequate tissue expansion and the use of a vascularized pedicle graft from the fused limb, primary abdominal wall and pelvic closure was possible without complications of wound healing. The consistent finding of a shared lower urinary tract with one ureter from each infant entering each bladder should be an…

Nipple malposition following nipple sparing mastectomy: Evaluation of secondary correction in prepectoral implant-based breast reconstruction.

Ptosis can be a relative contraindication to nipple sparing mastectomy (NSM). Although a mastopexy-type approach can be performed during mastectomy, this requires very well perfused skin flaps and possibly intraoperative adjustments with the oncological breast surgeon. The purpose of this study is to assess risk factors that result in the nipple surgically raised secondarily in standard NSM with prepectoral tissue expander (TE) reconstruction to help guide surgical decision-making and patient counseling. Patients at our center who underwent NSM and immediate prepectoral TE reconstruction were

Biomechanical and histological outcomes of a cervical expander capsule.

BACKGROUND: An expander capsule is a fibrous membrane that forms around a tissue expander. However, its outcome is still unclear. Here we investigated the biomechanical and histological outcomes of cervical capsules that were left in vivo after expanders were removed. METHODS: The deep and superficial capsules of 29 human cervical expanders were collected to serve as an experimental group. All 29 patients sustained facial and neck burn scars and underwent scar excision and expanded skin flap transfer. These capsules were divided into four groups based on the in vivo persistence time of the ca

Investigation of Microsurgical Technique Combined With Skin Flap Expansion for Ear Reconstruction in Treating Hunter Type III Congenital Microtia.

OBJECTIVE: This study aims to observe the efficacies of microsurgical technique combined with the Zhuang's skin soft tissue expander in treating Hunter type III congenital microtia. METHODS: Fifty-eight patients (61 ears) were enrolled from 2003 to 2012; the skin tissue expander was embedded subepidermally in the first stage via the intrahairline longitudinal incision in the postauricular mastoid area, the diseased-side rib cartilage was then taken for preparing the ear bracket in the second stage, and the tragus was surgically reconstructed in the third stage. RESULTS: The mean follow-up la
