Characterizing changes to older adults' care transition patterns from hospital to home care in the initial year of COVID-19.

BACKGROUND: Skilled home healthcare (HH) provided in-person care to older adults during the COVID-19 pandemic, yet little is known about the pandemic's impact on HH care transition patterns. We investigated pandemic impact on (1) HH service volume; (2) population characteristics; and (3) care transition patterns for older adults receiving HH services after hospital or skilled nursing facility (SNF) discharge. METHODS: Retrospective, cohort, comparative study of recently hospitalized older adults (≥ 65 years) receiving HH services after hospital or SNF discharge at two large HH agencies in Bal

Care needs and level of care difficulty related to hip fractures in geriatric populations during the post-discharge transition period.

The objective of this study was to understand the following: (1) the specific care needs of geriatric hip fracture patients; (2) the level of care difficulty during the transition period following discharge; and (3) the correlation amongst demographics, the physical function status of the elderly, and care needs. Purposive sampling was used, and a total of 71 hip fracture cases and their caregivers were selected. A survey was carried out to collect data just before discharge and one week following discharge. Statistical methods included descriptive statistics, correlation analysis, one-way ANO

The Role of a Transitional Pain Approach to Surgical Care in a Patient With an Opioid Use Disorder: A Case Report.

INTRODUCTION: The management of postoperative pain in patients with an opioid use disorder (OUD) is challenging. Hospitals are frequently not equipped to respond effectively to the pain of these patients following surgery. For example, many caregivers are reluctant to prescribe opioids to these patients because a relapse in their misuse of drugs may occur. Limited research is available on the management of pain in this population, particularly in relation to the transition from the hospital to the community. CASE REPORT: A 28-year-old male with a long-standing OUD requiring cardiothoracic sur

A Web-Based Peer-Patient Navigation Program (Compassionate Online Navigation to Enhance Care Transitions) for Youth Living With Childhood-Acquired Disabilities Transitioning From Pediatric to Adult Care: Qualitative Descriptive Study.

BACKGROUND: Studies have highlighted significant challenges associated with the transition from pediatric to adult health and social care services for youth living with childhood-acquired disabilities and their caregivers. Patient navigation has been proposed as an effective transitional care intervention. Better understanding of how patient navigation may support youth and their families during pediatric to adult care transitions is warranted. OBJECTIVE: This study aims to describe the preferred adaptations of an existing web-based platform from the perspectives of youth with childhood-onset…

Transitioning from child and adolescent psychiatry to follow-up care: a concept mapping study.

Youth with severe and enduring mental health problems often require follow-up care after treatment in child and adolescent psychiatry (CAP). In this study, follow-up care is defined as services beyond CAP, including basic mental health support, primary mental health care, and practical or parenting support delivered by community-based teams. However, transitions to such care are often fragmented, poorly coordinated, and experienced as abrupt. This study explores perspectives of CAP clinicians, follow-up care providers, parents, and youth to identify factors considered both important and feasib

Transition to adult diabetes care for young adults with type 2 diabetes: Challenge and opportunity.

Transition from pediatric to adult care is a critical period for young adults with type 2 diabetes mellitus (T2DM). This study included a retrospective chart review of 139 patients aged 17-25 years with T2DM followed in pediatric endocrinology between 2021 and 2022. Patients were categorized as transitioned to adult care, remained in pediatric care, or lost to follow-up. Of the cohort, 33.8% transitioned to adult care, 27.3% remained in pediatric care, and 38.9% were lost to follow-up. Among those who transitioned to adult care, 48.9% established care with family medicine, 27.7% with adult end

Are You PREPAREd? A Structured Approach to Support Safe and Confident Transitions to After-Hours Care.

BACKGROUND AND NEED FOR INNOVATION: Early after-hours work represents a demanding transition for newly graduated doctors. Preparation is often fragmented, with limited attention to the integrated clinical, organisational, and supervisory demands of after-hours care, contributing to uncertainty that may affect both patient safety and professional development. GOAL OF INNOVATION: PREPARE (PREParing for After-hours caRE) is an eight-week programme designed to support new graduates during their early postgraduate transition by enabling safe and confident participation in after-hours care. STEPS

Cystic Fibrosis Transition From Pediatrics to Adult Care in Low-Middle Income Countries.

BACKGROUND: As survival improves for individuals with cystic fibrosis (CF) worldwide, the number of adolescents requiring transition from pediatric to adult care is rapidly growing. In addition to limited resources and lack of uniform access to medical care, traditional treatments, and advanced therapies, the increasing number of transitioning young adults in low- and middle-income countries (LMICs) face barriers and challenges to safe transition from pediatric to adult healthcare that are both universal and unique. This shift in demographics presents distinct challenges due to an underdevelop

Codesigning a pharmacist-led transition of care program in the emergency department: a qualitative participatory study.

INTRODUCTION: Transitions from the emergency department (ED) to home are high-risk periods for medication-related harm. Pharmacist-led interventions may improve medication safety and care continuity, however, co-designed approaches to developing such interventions remain limited. AIM: This study aimed to co-design a pharmacist-led transition-of-care (ToC) program to improve medication management for adults discharged from the ED, with the intention of subsequently evaluating it in a pilot randomized controlled trial. METHOD: A two-phase qualitative study was undertaken at a general teaching

Evolving Burn Care: The Transition from Life Preservation to Life Restoration-A Narrative Review.

Over the past years, burn care has evolved from a discipline focused on survival to one centered on restoring long-term health, function, and quality of life. Significant advances in critical care, early excision and grafting, infection control, and metabolic support have transformed survival outcomes for even the most severe injuries. As a result, the field now faces a new frontier: understanding and managing the long-term physical, psychological, and systemic sequelae of survival. This review traces the evolution of burn care over the last century and outlines the challenges and priorities f

Follow-up and transition of care for low recurrence risk thyroid cancer patients in Canada.

The incidence of differentiated thyroid cancer (DTC) has increased significantly in recent decades. Following initial diagnosis, DTC patients are classified according to the American Thyroid Association (ATA) as low, intermediate, and high risk for recurrence. Patients in the ATA low recurrence-risk category have a recurrence risk of ≤5%, with 20-year disease-specific mortality of <1%. Accordingly, there has been a shift to de-escalating initial treatment, including the relaxation of thyroid-stimulating hormone suppression. In addition, fewer low-risk patients undergo total thyroidectomy or ra

Lost in Transition: Challenges in the Journey from Pediatric to Adult Care for a Romanian DMD Patient.

BACKGROUND: The transition from pediatric to adult care in Duchenne Muscular Dystrophy (DMD) is challenging due to the disease's complexity and the need for lifelong, comprehensive management. In Romania, ongoing efforts aim to enhance multidisciplinary collaboration, though systemic barriers, such as fragmented healthcare services, persist. Nonsense mutations, including those in exon 30 described here, are often associated with more severe disease progression. METHODS: We present the case of a 17-year-old Romanian DMD patient with a nonsense mutation in exon 30 of the dystrophin gene. The pa

Impact of Prospective Audit and Feedback by Transitions of Care Pharmacists on the Management of Community-Acquired Pneumonia.

Background: The 2019 Center for Disease Control and Prevention's Core Elements of Hospital Antibiotic Stewardship identifies community-acquired pneumonia (CAP) as a crucial area for improving antibiotic use. Transitions of care (TOC) pharmacists can optimize medication therapy for CAP patients. Methods: This was a retrospective, multi-hospital analysis of discharge antibiotic prescribing. Between December 1, 2022, and June 30, 2023, TOC pharmacists intervened to decrease discharge days of therapy (DOT) for CAP therapy. The study compared records of CAP patients with a TOC antibiotic stewardshi

The impact of providing intensive care and practical help in mid-life on employment transitions in Europe.

This paper examines how caregiving influences employment transitions among employed mid-life adults (50-69 years) who began providing non-professional care on a daily basis to someone inside or outside their household. Using data from the Survey of Health Ageing and Retirement in Europe (SHARE) from 2004 to 2017, we apply a difference-in-difference model with propensity score weighting to estimate probabilities of various employment change outcomes for each care status. These outcomes include reducing working hours, exiting the labour market, and retiring. Results are compared to those who con
