Management of Adult Condylar and Subcondylar Mandibular Fractures in a Level 1 Trauma Center.

Management of mandibular condylar and subcondylar fractures is heavily debated due to the proximity of important structures and complication risks. This study reviews the outcomes of 4 different treatment approaches: conservative, closed reduction with maxillomandibular fixation, open reduction and internal fixation, and open reduction and internal fixation with maxillomandibular fixation. A retrospective chart review of all patients with a facial fracture seen at a level one trauma center between 2012 and 2022 was performed. Demographics, fracture etiology, fracture type, treating team (plast

Quantifying the Drivers of Survival: A Matched Cohort Study of Trauma Center-Level Differences.

INTRODUCTION: Tertiary trauma centers (levels I and II) offer comprehensive care and are equipped to manage complex injuries. Trauma patients treated at tertiary centers have better survival outcomes. However, the specific interventions driving this advantage remain unknown. This study aimed to identify interventions contributing to differences in short-term mortality between tertiary trauma centers (TTCs) and nontertiary trauma centers (NTCs). METHODS: This was a retrospective cohort study using Trauma Quality Improvement Program data reported between 2017 and 2022. Propensity score matching…

Traumatic Venous Sinus Thrombosis: Patient and Practice Patterns at a Major Trauma Center.

BACKGROUND: Traumatic brain injury can lead to venous sinus injury and thrombosis, which may be associated with elevated intracranial pressure and poor outcomes. We sought to examine the risk factors, management, and clinical outcomes of traumatic venous sinus thrombosis (tVST). METHODS: We conducted a comprehensive search of our institutional radiology database for final radiology reports from 2013 to 2022 that contained the terms "venous sinus thrombosis," "sinus thrombosis," or "venous occlusion." tVST was detected on computed tomography and confirmed by a board-certified neuroradiologist.

Comparative outcomes of blunt and penetrating diaphragmatic ruptures: A single trauma center study.

OBJECTIVES: Diaphragma rupture is an insidious case following thoracoabdominal trauma with significant morbidity and mortality rates, if left untreated. Its symptoms might be masked by other associated injuries, which are often present, especially following blunt trauma. Modern imaging modalities might overlook a present diaphragmatic rupture. The real challenge, therefore, lies in diagnosis rather than treatment. To shed more light on this entity, we shared our experience in this regard. MATERIAL AND METHODS: A total of 51 patients were enrolled in the study between January, 2008 and October

Outcomes of resuscitative and emergent thoracotomies following injury at the largest trauma center in Estonia.

BACKGROUND: An emergency department thoracotomy (EDT) is performed in critically injured patients after a recent or in an imminent cardiac arrest following trauma. Emergent thoracotomy (ET) or operation room thoracotomy is reserved for more stable patients. However, the number of these interventions performed in an European settings is limited. Thus, we initiated the current study to investigate outcomes and risk factors for mortality of patients required EDT or ET at the largest trauma center in Estonia. METHODS: All patients admitted after trauma to the North Estonia Medical Centre between

A Descriptive Analysis of Patients With Attempted Suicide at a Rural Level I Trauma Center.

OBJECTIVES: For patients with self-harm, suicide attempt, or suicide completion, the trauma bay is often the single point of contact. Regional differences and patterns exist for suicide that should be studied to enhance preventive strategies. Our goal was to critically evaluate the suicidal population of Southeast Georgia over a 9-year period. METHODS: A retrospective review of our trauma database from January 2010 through December 2019 was conducted at a Level I Trauma Center. All ages were included. All patients arriving with attempted suicide or death due to a suicidal complication were in

Association of Trauma Center Designation With Postdischarge Survival Among Older Adults With Injuries.

IMPORTANCE: Trauma centers improve outcomes for young patients with serious injuries. However, most injury-related hospital admissions and deaths occur in older adults, and it is not clear whether trauma center care provides the same benefit in this population. OBJECTIVE: To examine whether 30- and 365-day mortality of injured older adults is associated with the treating hospital's trauma center level. DESIGN, SETTING, AND PARTICIPANTS: This prospective, population-based cohort study used Medicare claims data from January 1, 2013, to December 31, 2016, for all fee-for-service Medicare benefi…

Reconstructive Management of Gunshot Wounds to the Frontal Sinus: An Urban Trauma Center's Perspective.

INTRODUCTION: In the last decade, we have seen a steady increase in the incidence of frontal sinus trauma due to gunshot wounds and a decrease in motor vehicle trauma. Penetrating gunshot wounds to the frontal sinus present a unique challenge to the reconstructive surgeon because they require careful consideration of the management principles of plastic surgery. Despite previous reviews on frontal sinus trauma, there are no studies examining the management techniques of frontal sinus fractures due specifically to gunshot wounds. In this study, we aim to retrospectively evaluate the use of a va

Factors associated with pediatric drowning admissions and outcomes at a trauma center, 2010-2017.

BACKGROUND: Drowning is a leading cause of preventable mortality and morbidity in children. Its high fatality rate and frequent severe sequelae (e.g. brain damage and permanent loss of functioning) place a premium on preventive efforts. METHODS: A retrospective analysis of patients ≤21 years of age admitted between 2010 and 2017 to a pediatric trauma center was conducted to identify factors associated with drowning admissions, fatal drowning, and severe outcome (ventilator use, ICU admission, or death). Outcomes were modeled and estimated by use of logistic regression and Poisson regression.
