Stoking the drug target pipeline for human African trypanosomiasis.

Trypanosoma brucei is the causative agent of African sleeping sickness, putting at risk up to 50 million people in sub-Saharan Africa. Current drug therapies are limited by toxicity and difficult treatment regimes and as the development of vaccines remains unlikely, the identification of better drugs to control this deadly disease is needed. Strategies for the identification of new lead compounds include phenotypic screening or target-based approaches. Implementation of the latter has been hampered by the lack of defined targets that are both essential and druggable. In this issue of Molecular

Risk for human African trypanosomiasis, Central Africa, 2000-2009.

Comprehensive georeference records for human African trypanosomiasis in Cameroon, Central African Republic, Chad, Congo, Equatorial Guinea, and Gabon were combined with human population layers to estimate a kernel-smoothed relative risk function. Five risk categories were mapped, and ≈3.5 million persons were estimated to be at risk for this disease.

Regulatory T cells prevent control of experimental African trypanosomiasis.

African trypanosomes are single-cell, extra-cellular blood parasites causing profound immunosuppression. Susceptible BALB/c mice infected s.c. into a footpad with 10(4) Trypanosoma congolense die with fulminating parasitemia within 10 days. We injected BALB/c mice 2 days before such an infection with different doses of a depleting mAb specific for CD25, a surface marker of regulatory T cells (Tregs). Pretreatment with a low, optimal dose of anti-CD25 resulted in a dramatic effect, in that the infected mice did not develop parasitemia, as well as eliminated all parasites and showed no signs of

Experimental trypanosomiasis in Yankasa ewes: the body weight response.

Sleeping sickness (African Trypanosomasis) is an anthropozoonosis transmitted primarily by the tsetse fly. It is associated with a host of clinical indices ranging from fever, aneamia and anorexia to reproductive failures in man and his domestic animals. The main objective of this study is to appraise the responsiveness of the body weight as a clinical indicator of sleeping sickness in experimentally infected Yankasa ewes. Twelve mature Yankasa sheep (6 infected and 6 control ewes) were used in this study. Weekly body weights and daily rectal temperature were taken while blood samples for haem

Epidemiology and diagnosis of African trypanosomiasis using DNA probes.

The accurate identification of trypanosome species has been a challenging problem in the epidemiology of African trypanosomiasis, both human and animal. The last 10 years have seen great progress through the application of deoxyribonucleic acid (DNA) probe technology, although this has also revealed greater complexity than supposed. While a single repetitive DNA probe can identify all members of the subgenus Trypanosoma (Trypanozoon), including the human pathogens T. brucei gambiense and T. b. rhodesiense as well as the non-tsetse-transmitted trypanosomes T. evansi and T. equiperdum, at least

African trypanosomiasis in two travelers from the United States.

African trypanosomiasis is a rare but well-documented cause of fever in United States travelers returning from areas where it is endemic. We report two recently diagnosed cases that involved tourists who went on safari in Tanzania. Review of these and 29 other published cases indicates that disease in returning United States travelers is nearly always of the East African form, a fulminant illness for which prompt diagnosis is necessary. In the United States, timely and appropriate therapy for this disease has resulted in favorable outcomes for most patients. Chemoprophylaxis for East African t

Plasma melatonin rhythm is maintained in human African trypanosomiasis.

In human African trypanosomiasis (sleeping sickness), sleep and wake episodes are sporadically distributed throughout the day and the night. Plasma melatonin, sleep-wakefulness and rectal temperature rhythms were studied in 9 Congolese patients suffering from sleeping sickness compared to 6 healthy controls submitted to the same light/dark regime. The circadian distribution of the sleep-wake cycle was disturbed in relation to the severity of the disease. As controls, patients maintained a very distinct plasma melatonin nyctohemeral rhythm which displayed a significant phase advance (1:08 +/- 0

[Epidemiology of bovine trypanosomiasis (Trypanosoma vivax) in French Guiana].

About 3,000 European and Zebu cattle sera from various rearing areas of French Guiana were examined by enzyme-linked immunosorbent assay (ELISA) for detection of specific circulating antigens of Trypanosoma vivax and for detection of antibodies to Trypanosoma species with reagents provided by ILRAD. For the whole country, the seroprevalence rate is 29%. Infection was detected everywhere in French Guiana, but seroprevalence rates were different from place to place, depending on the epidemiological situation, the abundance of vectors (stable-flies and horse-flies) and the management. We could ge

[Cerebral trypanosomiasis: epidemiology, clinical aspects, therapy and illness course].

More than 20,000 people are infected with one of the two forms of African trypanosomiasis each year. Without treatment the outcome is almost always fatal. There are, however, problems involved in drug treatment. Reactive encephalitis is recorded in up to 18% of all cases treated for cerebral trypanosomiasis. Recently a lack of response to treatment has been reported with increasing frequency from endemic areas. Relapses occur in up to 10% of cases. Reasons for relapses include insufficient treatment due to poor compliance or lack of drugs, reinfection in endemic areas, pharmacokinetic problems

Pathophysiology of ovine trypanosomiasis: ferrokinetics and erythrocyte survival studies.

The haematological changes, erythrokinetics and ferrokinetics of sheep were investigated after infection with Trypanosoma congolense. Following the detection of parasites in blood, the infected sheep developed macrocytic hypochromic anaemia. Studies with 51Cr-red cells, 125I-albumin and 59Fe as ferric citrate 11 weeks after infection revealed that infected sheep had significantly lower mean circulating red cell volumes but higher plasma and blood volumes than control sheep. The infected sheep also had enhanced erythropoietic activity as judged by significantly higher plasma iron turnover rates

Trypanosomiasis in the black rhinoceros (Diceros bicornis Linnaeus, 1758).

A black rhinoceros (Diceros bicornis) moved from a tsetse-free to a tsetse-infested area in Kenya was monitored for two months following translocation. The animal acquired a Trypanosoma vivax infection from natural tsetse challenge, but survived without requiring treatment with trypanocides. The infection was characterised by moderately high parasitaemia, with symptoms of anaemia, leukopaenia and thrombocytopaenia. Although confirmed to be T. vivax through deoxyribonucleic acid hybridisation and parasite development in tsetse in the proboscis only, the parasite had unusual morphology and motil

Serum lipid and lipoprotein abnormalities in human African trypanosomiasis.

We have studied the serum lipoprotein system in human African trypanosomiasis (Trypanosoma brucei gambiense infection). The study was carried out on 74 Congolense patients suffering from sleeping sickness and 34 Congolense control subjects living in the endemic region of Boko Songho. We have determined the serum concentrations of lipids (triglycerides, cholesterol, phospholipids) and apolipoproteins (apolipoprotein A-I and B), and the separation of serum lipoproteins by electrophoresis. For the patients infected with T. b. gambiense, in comparison with control subjects, the results have shown

[Epidemiological aspects of human African trypanosomiasis in south Chad].

The authors are recording results of a clinical, parasitological and immunological sample survey about african human trypanosomiasis in the South Chad where old focus has been reexplored. In 1988 the situation ever remains as a problem in some areas (Tapol, Ranga) where high prevalence rates are found again.

[Endocrine function and African trypanosomiasis. Evaluation of 79 cases].

Although the high frequency of hypogonadism was found during the sleeping sickness, the physiological properties of the gonads have been poorly studied. We reported here the exploration of the gonadotropic axis in 79 patients infected by Trypanosoma brucei gambiense. It has been noted a decrease in oestradiol for 50% of women, in testosterone levels for 50% of men whereas no decrease of pituitary gonadotropins (FSH and LH) was found. The results of the gonad adenohypophyseal tests suggested that the dysfunction may be of central origin. The studies of thyreotropic axis were also examined: a de

[Trypanosomiasis caused by T. b. gambiense: methods of control].

This paper reviews the different ways currently available for screening sleeping sickness. These ways are then integrated and discussed by the author in proposing different strategic and methodologic solutions after emphasizing the role of each of the technical stages. Altogether, there isn't one standard solution but adaptations around a main plan, according to contexts and having regard to efficiency and profitability. At all events, success depends on political will, community participation and the motivation and good training of the personnel.

Hyperendemic West African trypanosomiasis in a rural hospital setting.

Over 60 cases of trypanosomiasis presented to Eku Hospital between May 1983 and May 1985. During this time, within this previously unreported hyperendemic focus, 47 patients were documented microscopically. Their clinical characteristics are reported here. Seventy-four per cent were felt to have CNS involvement on presentation. Compliance with the therapeutic regimen was a major problem, requiring adjustments in the usually accepted management of this disorder.

Socio-economic problems related to animal trypanosomiasis in Africa.

Animal trypanosomiasis is one of the major constraints of rural development in Africa. If trypanosomiasis did not exist in these areas, it is estimated that some regions could carry 3-5 times more livestock. Chemotherapy and chemoprophylaxis are old methods used to tackle the problem and are still valuable, provided the number of interventions is limited. Tse-tse fly control is a very useful tool to combat trypanosomiasis but expensive. The sterile male release technique is not economically feasible on a wide scale. Immunization or vaccination is not yet available. Trypanotolerance is a biolog
