There is a strong association between Helicobacter pylori and peptic ulcer and chronic gastritis. It has been shown, furthermore, that the relapse rate after a healed duodenal ulcer is much reduced when the treatment has also eradicated H pylori from the antrum ventriculi. We have tried two simple and rapid tests for diagnosing H pylori infection on biopsies taken at gastroscopy. Cocaine is a potent topical vasoconstrictor that causes nasal mucosal and dermal ulceration. Use of the purified cocaine derivative "crack" has reached epidemic proportions in large cities. We have shown previously that acid is one factor, although not the only one, in the pathogenesis of duodenal ulcer pain. In the present study patients with gastric ulcer were endoscoped without sedation or premedication. Under direct vision the ulcer craters were infused sequentially with 0.1 N HCl and normal saline, the sequence of infusion being randomized and double blind. Bacterial corneal ulcer is a potentially blinding emergency which should ideally be treated by an ophthalmologist aided by slit lamp biomicroscopy, microbial stain and cultures, and then selected fortified topical antibiotics. We suggest an emergency corneal ulcer kit for the initial treatment of patients with suspected corneal ulcers who are unable to readily travel to an ophthalmologist.

The authors retrospectively describe the computed tomographic (CT) findings in 35 patients with peptic ulcer disease. Three of eight patients with gastritis or duodenitis had bowel-wall thickening. Ten of the remaining 27 patients had CT evidence of ulcer perforation (n = 2) or penetration (n = 8), four cases of which were unsuspected clinically. Experimental gastric ulcer formation was performed in the pika and compared with that in the rat. Gastric ulcers were formed in pika that were subjected to water restraint for 4-5 days for 2 hours each day. Operated on were 168 patients with gastric ulcer disease. The preference was given to Billroth-I gastric resection with the artificial sphincter formation at the site of anastomosis and proximal gastric resection with the antireflux gastroesophageal anastomosis creation. Of 333 duodenal ulcer (DU) patients 75 (22.5%) were aged 65-93 years (study group). We have prospectively evaluated 16 episodes of bacteremia induced by decubitus ulcers. The most commonly isolated microorganisms were Staphylococcus aureus, Proteus mirabilis, Pseudomonas aeruginosa and Bacteroides fragilis. Bacteremia was polymicrobial in 5 cases (31%). In a study of patients attending a rheumatology clinic 230 unselected patients, 185 with rheumatoid arthritis (RA) and 45 with other rheumatic disorders (non-RA), were examined by endoscopy and a detailed history of symptoms referable to the gastrointestinal tract was taken. Mooren's ulcer is a rare disease of presumed autoimmune aetiology. Some cases run a chronic severe course and fail to respond to local and systemic therapy. We report here such a case with bilateral Mooren's ulcer that failed to respond to local therapy with topical corticosteroids, silver nitrate, and conjunctival resection, as well as systemic immunosuppression with corticosteroids, cyclophosphamide, and azathioprine.
