Hypothenar hammer syndrome is a rare entity secondary to ulnar artery damage in the wrist, affecting mainly those exposed to repeated hand-palm trauma. Surgery is discussed in case of severe symptoms, resistant to medical treatment, and/or when anatomical lesions with emboligenic potential are demonstrated in the radiological exams. In this case, resection of the pathological zone with revascularization by autologous vein graft is the best option. We report the case of a 60-year-old patient who had a recurrence of symptoms more than 10 years after the completion of a surgical treatment. There

BACKGROUND: Recurrent ulnar nerve compression after primary anterior subcutaneous transposition is relatively rare, and revision surgery is challenging. This study retrospectively evaluated the clinical outcomes of revision anterior subcutaneous transposition for recurrent ulnar nerve compression.
METHODS: Eight patients who underwent revision anterior subcutaneous transposition for recurrent ulnar nerve compression were enrolled in this study. The outcomes were based on preoperative and postoperative symptoms, physical examination findings, and electromyographic evaluation.
RESULTS: Ulnar n

OBJECT: Surgical transposition of the ulnar nerve to alleviate entrapment may cause otherwise normal structures to become new sources of nerve compression. Recurrent or persistent neuropathy after anterior transposition is commonly attributable to a new distal compression. The authors sought to clarify the anatomical relationship of the ulnar nerve to the common aponeurosis of the humeral head of the flexor carpi ulnaris (FCU) and flexor digitorum superficialis (FDS) muscles following anterior transposition of the nerve.
METHODS: The intermuscular septa of the proximal forearm were explored i

PURPOSE: To evaluate the dynamic anatomy of the ulnar nerve at the elbow.
METHODS: We studied 11 fresh cadavers. We placed metal clips on the ulnar nerve at three locations: at the medial epicondyle (point A), 3 cm proximal to the epicondyle (point B), and 14 cm proximal to the epicondyle (point C). The distances from the medial epicondyle to points A, B, and C on the ulnar nerve and between each pair of points were measured in full elbow extension and flexion.
RESULTS: With full elbow flexion, there was no movement of the ulnar nerve at point A (adjacent to the medial epicondyle). Point A a

INTRODUCTION/AIMS: Nerve cross-sectional area (CSA) is the most robust and sensitive ultrasonographic indicator of nerve pathology. The study aimed to identify predictors of maximal CSA (CSAmax) in ulnar neuropathy at the elbow (UNE) and to assess its utility in elucidating the mechanism of UNE, which is potentially important for patient referral to a surgeon.
METHODS: In a group of prospectively recruited UNE patients, 11 demographic, clinical, and electrodiagnostic features were recorded. CSAmax in the elbow segment was measured ultrasonographically. Explanatory variables for CSAmax were de

BACKGROUND: The neurological component of hand-arm vibration syndrome (HAVS) uses the Stockholm Workshop Scale sensorineural (SWS SN) stages for classification. Proximal compressive neuropathies are common in HAVS and the symptoms are similar to SN HAVS. The SWS may not be a valid staging tool if a patient has comorbid proximal compression neuropathy.
AIMS: To evaluate the prevalence of proximal compression neuropathy in patients presenting for HAVS assessment and examine the association between compressive neuropathies and SWS SN.
METHODS: A standardized assessment protocol was used to asse

Background: Coincident carpal and cubital tunnel syndromes present a diagnostic challenge, exacerbated by the limitations of nerve conduction study (NCS) for confirming cubital tunnel syndrome. This study develops a diagnostic scoring system, the Koh-Benhaim (KB) score, to identify patients with coincident compression neuropathies. Methods: A retrospective review of 515 patients was performed from patients surgically treated for carpal and/or cubital tunnel release. These patients were divided as patients with isolated carpal tunnel syndrome (n = 337) or coincident carpal and cubital tunnel sy

Peripheral nerve compression of the upper extremity is a common pathology often necessitating surgical intervention, much is known, but much more is left to understand. For the more common pathologies, carpal tunnel syndrome, cubital tunnel syndrome, and ulnar tunnel syndrome, research and clinical efforts directed toward standardization and reduction of resource use have been attempted with varied success. Diagnosis of many of these syndromes is largely based on a proper history and physical examination. Electrodiagnostic studies continue to have value, but proportionally less than previous d

A 72-year-old chronically depressed man developed bilateral ulnar, median, and peroneal nerve palsies. Chronic depression predisposes to pressure neuropathies because of prolonged immobility and because of postures with leg crossings, resting arms on elbows, and resting chin on hyperextended wrist.

During routine anatomical dissection of the left upper limb of a 73-year-old female cadaver, a unique coexistence of variant muscles was found. In the forearm region, a largely developed reversed palmaris longus was discovered. Its short distal tendon was in close relation to the median nerve. In the neighboring hypothenar region, an unusual abductor digiti minimi was also observed. Its muscular body was composed of two portions - medial and lateral one, arising from the reversed palmaris longus tendon. The lateral portion passed over the ulnar nerve and artery in the canal of Guyon. In the li

Increasing reports of fractured hamate hooks have provided a sharper focus on causes, diagnosis, treatment, and associated anomalies such as ulnar neuropathy, bipartition, and accessory ossicles. This case report, presenting ulnar tunnel syndrome with bilateral hamulus bipartition, illustrates the relation between these anomalies.

The carpal tunnel is a tightly bounded space, making the median nerve prone to compression and eventually leading to carpal tunnel syndrome. Carpal tunnel release surgery transects the transverse carpal ligament to expand the tunnel arch space, decompress the median nerve, and relieve the associated symptoms. However, the surgical procedure unavoidably disrupts essential anatomical, biomechanical and physiological functions of the wrist, potentially causing reduced grip strength, pillar pain, carpal bone instability, scar tissue formation, and perineural fibrosis. It is desirable to decompress

Peripheral nerve injuries have the potential to cause significant disability and can be commonly associated with recreational and occupational activities. Acute nerve injuries are mainly related to violent trauma, while repeated mechanical trauma due to external forces or repetitive motions can produce chronic nerve compression injury. This chapter will present a narrative review of the existing evidence of the association between peripheral compressive nerve disorders and work-related risk factors. Carpal tunnel syndrome (CTS) is the most common peripheral neuropathy in the general population

BACKGROUND: Recurrent compression of the ulnar nerve in the cubital tunnel is a difficult problem and many solutions have been tried with variable success. Autologous vein graft wrapping is an alternative technique and it is recommended for the treatment of recalcitrant ulnar nerve compression in which 2 or more previous surgical procedures have failed to resolve the problem.
METHODS: Seventeen patients with recurrent cubital tunnel syndrome were treated with autologous saphenous vein wrapping. These patients had previously undergone simple decompression, decompression combined with medial ep
